【论述】破伤风是由于突然遭受创伤，风邪乘虚侵袭所致。病邪传变至经络，导致寒热交替发作，身体强直反张，口噤不能张开。严重时，邪气深入脏腑，则难以治愈。此外，各种疮疡久不愈合，导致营卫之气虚弱，肌肉不生，疮口不合，风邪也能从外部侵入，形成破伤风的证候。
治疗破伤风，症见身体僵硬如铁石，或呈角弓反张状，口噤不开者，宜采用撬开牙齿灌药的方法。方用羌活汤：
羌活（去芦头）、防己、羚羊角（镑片）、升麻、黄芩（去黑心）、蔓荆实（去皮），以上各一两半；犀角（三分）；麻黄（去根节）。
以上十二味药，粗捣筛碎。每次服用三钱匕，加水一盏，加入地黄汁半合、薤白二寸，煎煮至八分，去渣。空腹、中午及睡前温服。若病情紧急，不拘时间服用。服药后若能微微汗出，即可痊愈。
治疗破伤风，症见身体强直、口唇颤抖者，方用雄黄丸：
雄黄（研细）、羌活（去芦头）、独活（去芦头），以上各一分；腻粉（研细一钱）；人参、芦荟（研细）、乌蛇肉（炮制）、天南星（炮制）、干姜（炮制），以上各一两；全蝎（去土炒）、甘草（炙）、防风（去叉）、白附子（炮制）、川芎、白僵蚕（炒）、藿香叶、薄荷、苍耳子（炒）、细辛（去苗叶）、当归（切焙）、本（去苗土）、秦艽（去苗土）、丹参、乌梢蛇（酒浸去皮骨炙）、龙脑（研细）、麝香（研细）、生金（锉为末）、水银（一分，入坩埚内与金末同结砂子）。
以上十五味药，将前十味捣罗成粉末，加入后五味研细的和匀。用酒煮和根葱二茎，煮熟后去掉葱，另加入槐胶末一两，煎成稠膏，调和药粉制成丸剂，如绿豆大。每次服用二丸，空腹、午时及睡前用温酒送下，逐渐加量至四五丸。
治疗破伤中风，方用附子丸：
附子（炮裂去皮脐）、乌头（炮裂去皮脐）、天麻、天南星（炮制），以上各一两。
以上四味药，粗捣筛碎，装入绢袋中，用好酒三升浸泡。冬季七日，夏季一日一夜，不得透气。到期后将袋子绞干，去渣取汁，在瓷石器中用慢火熬成膏。加入以下药物：
雄黄（研细）、丹砂（研细）、铅霜（研细）、白僵蚕（炒捣各一分）、蝎梢（炒捣）、鹿角霜（研细）、鹿角胶（烊化）、龙脑（研细）、麝香（研细）、生金（锉为末各一钱）、水银（一分，入坩埚内同金末结沙子）。
以上八味药，将前四味同捣，后四味同研，令其极细。
龙脑（研细）、麝香（研细）、生金（锉为末各一钱）、水银（一分，入坩埚内同金末结沙子）。
以上三项，共十六味药，分别捣研后混合均匀。用炼蜜制成丸剂，如绿豆大。每次服用一至二丸，空腹、午时及睡前用温酒送下，逐渐加量至三五丸。
治疗破伤中风，症见牙关紧急、口眼歪斜、身体或硬或软，以及小儿惊风，均适用此方。方用走马散：
天麻、天南星（炮制）、半夏（汤洗七遍，与生姜半两同捣焙干）、白附子（炮制）、附子（炮裂去皮脐各半两）、丹砂（研细）、雄黄（研细）、牛黄（研细）、麝香（研细）、犀角（镑片各一分）、腻粉（研细三分）。
以上十一味药，将六味捣罗成粉末，加入五味研细的和匀。每次服用半钱匕，用豆淋酒调下。若出汗则病愈，若未出汗再服一字量，过一会儿用热生姜稀粥送服。若小儿患病，每次服一字，用荆芥汤或熟开水调下。若要制成丸剂，即用新炊米饭制作。
治疗破伤中风，方用牛黄散：
牛黄（研细）、干蝎（酒炒）、麝香（研细）、雄黄（研细各三分）、白附子（炮制三两半）、天南星（炮制一两）、白僵蚕（炒）、天麻、半夏（汤洗七遍，与生姜一两同捣焙干各一两半）、丁香、丹砂（研细）、犀角（镑片）、羌活（去根节先煎掠去沫焙干）、附子（炮裂去皮脐）、蔓荆实（去皮）、防风（去叉）、当归（切焙各一两）。
以上二十一味药，将十七味捣罗成散剂，与研细的四味药和匀。每次服用半钱匕，用温酒调下。若出现角弓反张、牙关紧急，用豆淋热酒调下一钱匕，盖衣保暖良久，出汗即愈。
治疗打击造成的破疮，或洗头、挑齿、灸疮、狗咬等引起的中风，方用羚羊角散：
羚羊角（镑片）、石斛（去根）、川芎、知母（焙制）、山茱萸、薏苡仁、白芷、曲棘针（生用）、枳壳（去瓤）、双仁炒、麻黄（去根节煎掠去沫焙干）、蔓菁花（炒）、赤茯苓（去黑皮）、葛根、羌活（去芦头）、苍耳心（炒）、车前子、桑白皮（锉）、菊花（未开的）、酸枣仁（炒）、当归（切焙）、本（去苗土）、秦艽（去苗土）、细辛（去苗叶）、丹参、乌梢蛇（酒浸去皮骨炙各一两）。
以上四十味药，捣罗成散剂。每次服用一钱半至二钱匕，空腹、午时及睡前用温酒调下。若要制成丸剂，用炼蜜制成如梧桐子大的丸剂，每次服用十丸，用豆淋酒送下。
治疗破伤中风，方用莽草散：
莽草（浸洗过二两半）、石斛（去根二两）、萆薢、柏子仁（生用）、石龙芮、泽泻、牛膝（酒浸）、杜仲（去粗皮炙）、羌活（去芦头）、乌头（去尖）、黄芩（去黑心）、本（去苗土）、防风（去叉）、当归（切焙各二两）。
以上二十三味药，将二十一味捣为细末，与牛黄、菟丝子末拌匀。每次服用一钱匕，用温酒调下，日服二次，夜服一次，出汗即愈。
治疗破伤中风，方用大乌犀散：
犀角（镑片）、羚羊角（镑片）、龙脑（研细）、麝香（研细）、雄黄（研细）、熊胆（研细）、牛黄（研细）、乳香（研细）、全蝎（酒炒）、猪牙皂荚（酥炙）、天仙子（炮制）、干姜（炮制各一分）、天麻、升麻、独活（去芦头）、狗脊（去土）、细辛（去苗叶）、秦艽（去苗土）、川芎、杜仲（去粗皮切炙）、黄芪（锉炙）、本（去土）、莨菪子（淘去浮者炒）、蜀椒（去目并闭口炒出汗）、白鲜皮（锉）、防风（去叉）、天南星（炮制）。
以上三十五味药，将二十六味捣罗成散剂，与九味研细的药拌匀，再过筛一遍。每次服用一钱半至二钱匕，用豆淋酒调下，生姜酒送服。重症者用米醋与水同煎服之。
治疗破伤中风，症见牙关紧急、四肢强硬、无法进食，方用金乌散：
乌鸦（一只，去嘴足并毛翅）、狐肝（一具，同乌鸦入罐子内用蚯蚓泥封固烧烟尽，用三两捣为末，加入以下药物）、天麻、白附子、天南星（炮制）、白僵蚕（炒）、乌梢蛇（酒浸去皮骨炙）、藿香叶、桑白皮。
以上九味药，同捣罗成散剂。每次服用一钱匕，用温酒调下，昼夜服用五次。
治疗破伤中风，方用乳香散：

熊胆（研细）、天麻、紫菀（去土）、防风（去叉）、丹砂（研细）、牛黄（研细）、麝香（研细）、龙骨（研细）。
以上八味药，将四味捣罗成粉末，与别研的四味药和匀。用炼蜜制成如梧桐子大的丸剂，每次服用二十丸，用桑槐根汤送下，不拘时间。
治疗破伤中风，方用白僵蚕丸：
白僵蚕（炒）、麝香（研细）、乌梢蛇（酒浸去皮骨炙）、牛黄（研细）、干蝎（酒炒）、木香、龙骨（研细各半两）。
以上十二味药，将八味捣罗成粉末，与别研的四味药和匀。用炼蜜制成如绿豆大的丸剂，每次服用二丸，用温酒送下。重症者加服以祛涎水。
治疗破伤风，方用白丸子：
安息香、胡桃仁（与安息香同研各一两）、白胶香（研细）、牛黄（研细）、麝香、丹砂（研细）、川芎（各一分）、当归（切炒半两）、干蝎（七枚全者酒炒）、巴豆（三粒去皮心膜研如膏压去油）。
以上十味药，将三味捣为细末，与别研的七味药和匀。用炼蜜制成如梧桐子大的丸剂，每次服用一至二丸，用温酒送下。量力饮酒，只需微醺，不可大醉。兼治中风手脚挛缩及半身不遂，日服二次，夜服一次，不可过多。
治疗破伤风，方用麝香丸：
麝香（研细）、牛黄（研细各一分）、防风（去叉）、乌梢蛇（酒浸去皮骨炙）、干姜（炮切）、桂枝（去粗皮）、全蝎（去土炒）、白附子（炮制）、天南星（炮制）、白僵蚕（炒）。
以上十味药，将七味捣罗成粉末，与别研的三味药和匀。用炼蜜制成如梧桐子大的丸剂，每次服用七丸，用温酒送下，日服二次，夜服一次。重症者用豆淋酒送下并服两次，过一会儿用热生姜稀粥送服。出汗后仍要避风寒。
治疗一切破伤风，方用乌蛇丸：
乌梢蛇（酒浸去皮骨炙）、山栀子（去皮各半两）、防风（去叉）、独活（去芦头）、枳壳（去瓤）、川芎、白附子（炮制）、天南星（炮制）、白僵蚕（炒）、全蝎（去土炒）、干姜（炮制）、甘草（炙）。
以上十二味药，捣罗成粉末。用炼蜜制成如梧桐子大的丸剂，每次服用七丸，用温酒送下，逐渐加量至二十丸。
治疗破伤风牙关紧急、口噤不开、口眼歪斜、肢体弛缓，方用天南星丸：
天南星（一枚及三分者炮制）、地龙（五条醋炙）、土虺蛇（一条去头尾肠皮骨醋炙）。
以上三味药，捣罗成粉末。用醋煮面糊制丸，如绿豆大。每次服用三至五丸，用生姜酒送下，稀葱粥送服，出汗即愈。
治疗破伤风，方用干蝎丸：
干蝎（酒炒）、天麻（各半两）、蟾酥（二钱，汤浸化如稀糊）。
以上三味药，将二味捣罗成粉末，用蟾酥糊制丸，如绿豆大。每次服用一至二丸，用豆淋酒送下。重症者加量至三至五丸。
治疗破伤风，方用麝香散：
麝香（研细）、干蝎（各一分）。
以上二味药，捣罗成粉末。有疮口者，将药粉敷于疮口，可加速风邪排出，促进痊愈。
治疗破伤风，方用夺命散：
天南星、防风（各一两）。
以上二味药，捣罗成粉末。先用童子小便清洗疮口，然后用此药末调酒贴敷于疮口。
治疗破伤风，症见项颈紧硬、身体强直，方用定命散：
蜈蚣（一条全者）、乌梢蛇（项后取）、白花蛇（项后取各二寸，先酒浸去骨并酒炙）。
以上三味药，制成细散。每次服用二至三钱匕，煎酒小沸调服。
治疗破伤风，方用独圣散：

以上十六味药，将十三味捣罗成粉末，与别研的三味药和匀。每次服用一钱匕，用温酒调下，不拘时间。
治疗破伤风牙关紧急、失音不语、口吐涎沫、喉中作声，方用六味追风散：
附子（生用）、白附子（去尖）、乌头（去尖）、天南星、半夏（汤洗七遍去滑，姜制用各半两）、干蝎（全者炒一分）。
以上六味药，捣罗成散剂，加入腻粉一分研匀。每次服用一字至半钱匕，用豆淋酒调下，不拘时间。疮口用药末干掺封裹。
治疗破伤风，症见身如角弓反张，方用守宫丸：
守宫（炙干去足七枚）、天南星（一两，酒浸三日曝干，以上二味捣为末）、腻粉（半分）。
以上三味药，同研匀细。用薄面糊制丸，如绿豆大。每次撬开口腔，用酒灌下七丸。过了一会儿出汗即解，再给一服，再次出汗即愈。
治疗破伤风身肿、牙关不开，方用白僵蚕散：
白僵蚕（直者不拘多少）。
以上一味药，生研为粉末。每次用生姜自然汁调匀，以鸡翎蘸药扫于疮口，勿令干燥。须臾后肿塌皮皱即为有效。仍用生姜汁调半钱匕内服。
治疗破伤风发热头痛、恶心烦闷，方用乌头散：
草乌头（去芦头生用）、白矾（生用）、蜀椒（去目并闭口生用各一分）。
以上三味药，捣研为细末。疮口未合时，以津唾调涂。感觉有清水流出即为有效。仍服用后三味追风散：
乌头（生去皮脐一枚）、雄黄（研细一分）、麝香（研细一字）。
以上三味药，捣研为细散。每次服用一钱匕，用温酒调下。
治疗破伤风身体强直、口噤不开，方用黑神丸：
硫黄（研细）、丹砂（研细）、雄黄（研细各一两）、水银（二两，以上四味同结砂子）、铅丹（研细）、硝石、麻黄（去根节）、麝香（研细各一两）。

治疗破伤风寒热烦闷，状似伤寒，方用追风白丸子：
蜈蚣（赤足者姜汁浸一宿）、白花蛇项肉（酒浸一宿炙）、乳香（研细）、麝香（研细）、附子尖（...）。
以上五味药，捣研为末。用炼蜜制成如萝卜子大的丸剂，根据人大小，用薄荷熟水研下一丸至五丸或十丸。要出汗，以桃柳汤浴后，用热米饮送服，出汗即愈。
治疗破伤风打扑内损，方用银花散：
天南星（大者半生半炮）、白附子（半生半炮）、防风（去叉等分）。
以上三味药，为细散。每次服用二钱匕，重者用童子小便调下，稍轻者用热酒调下。如外伤用此药贴疮口。
[Discussion] Traumatic tetanus (Pòshāngfēng) arises from sudden injury, allowing wind pathogen (Fēngxié) to invade. The pathogen transmits through the meridians and collaterals (Jīngluò), causing alternating chills and fever, severe stiffness of the body, and trismus (Kǒujìn). In severe cases, the pathogen enters the zang organs, making it incurable. Furthermore, chronic non-healing sores lead to deficiency of Ying and Wei Qi (Yíngwèi), failure of muscle regeneration, and unclosed wound apertures; wind pathogen can also enter externally, resulting in traumatic tetanus.
For traumatic tetanus presenting with body stiffness like iron or opisthotonos (jiǎogōngfǎnzhāng) and trismus, it is appropriate to pry open the teeth for medication administration. Use Qianghuo Tang:
Qianghuo (Notopterygii Rhizoma et Radix, remove rhizome head), Fangji (Stephaniae Tetrandrae Radix), Lingyangjiao (Saigae Tataricae Cornu, sliced), Shengma (Cimicifugae Rhizoma), Huangqin (Scutellariae Radix, remove black heart), Manjingzi (Viticis Fructus, remove skin), each 1.5 liang; Xijiao (Rhinocerotis Cornu, 3 fen); Mahuang (Ephedrae Herba, remove roots).
The above twelve ingredients are coarsely crushed and sifted. Take 3 qianbi per dose, add 1 sheng of water, 0.5 he of Rehmanniae Juice (Dihuangzhi), and 2 cun of Scallion White (Xiebai). Decoct until 0.8 sheng remains, strain, and take warm on an empty stomach, at noon, and before bed. If the condition is urgent, take at any time. Recovery occurs if mild sweating ensues.
For traumatic tetanus with body stiffness and lip tremors, use Xionghuang Wan:
Xionghuang (Realgar, ground), Qianghuo (remove rhizome head), Duhuo (Angelicae Pubescentis Radix, remove rhizome head), each 1 fen; Nifen (Calomel, ground, 1 qian); Renshen (Ginseng Radix), Lvhui (Aloe), Wushao She (Zaocys, prepared), Tiannanxing (Arisaematis Rhizoma, prepared), Ganjiang (Zingiberis Rhizoma, prepared), each 1 liang; Quanxie (Scorpio, remove earth and stir-fry), Gancao (Licorice Root, honey-fried), Fangfeng (Saposhnikoviae Radix, remove forked tips), Baifuzi (Typhonii Rhizoma, prepared), Chuanxiong (Ligusticum Chuanxiong Rhizoma), Baijiangcan (Bombyx Batryticatus, stir-fried), Huoxiangye (Agastaches Folium), Bohe (Menthae Haplocalycis Herba), Can'erzi (Xanthii Fructus, stir-fried), Xixin (Asari Radix et Rhizoma, remove seedlings and leaves), Danggui (Angelicae Sinensis Radix, cut and dry-roasted), Bencao (Ligustici Wallichii Rhizoma et Radix, remove seedlings and soil), Qinjiao (Gentianae Macrophyllae Radix, remove seedlings and soil), Danshen (Salviae Miltiorrhizae Radix et Rhizoma), Wushao She (Zaocys, wine-soaked, skin and bones removed, roasted), Longnao (Borneolum Syntheticum, ground), Shexiang (Moschus, ground), Shengjin (Gold, filed into powder, 1 qian), Shuiyin (Hydrargyrum, 1 fen, placed in a crucible with gold powder to form cinnabar-like particles).
Of the fifteen ingredients, crush and sieve the first ten into powder. Mix with the remaining five finely ground ingredients. Boil two stalks of scallion with roots in wine until cooked, remove scallions, add 1 liang of Huaijiao (Acacia Gum), and decoct into a thick paste. Mix with the medicine to form pills the size of mung beans. Take 2 pills per dose, warm wine with empty stomach, noon, and before bed. Gradually increase to 4-5 pills.
For traumatic stroke, use Fuzi Wan:
Fuzi (Aconiti Lateralis Radix Praeparata, cracked, peel and navel removed), Wutou (Aconiti Radix, cracked, peel and navel removed), Tianma (Gastrodiae Rhizoma), Tiannanxing (Arisaematis Rhizoma, prepared), each 1 liang.
Crush and sift the four ingredients, place in a silk bag, and soak in 3 sheng of good wine. Soak for 7 days in winter or 1 day and night in summer; do not allow air penetration. Squeeze the bag dry, strain, and take the juice. Slowly boil in a porcelain vessel to form a paste. Add the following:
Xionghuang (Realgar, ground), Dansha (Cinnabar, ground), Qianshuang (Plumbum Subcarbonate, ground), Baijiangcan (Bombyx Batryticatus, stir-fried and pounded, 1 fen), Xieshao (Scorpion tail, stir-fried and pounded), Lu Jiaoshuang (Deer Antler Glue Frost, ground), Lu Jiaojiao (Deer Antler Glue, melted), Longnao (Borneolum Syntheticum, ground), Shexiang (Moschus, ground), Shengjin (Gold, filed into powder, 1 qian), Shuiyin (Hydrargyrum, 1 fen, placed in a crucible with gold powder to form cinnabar-like particles).
Of these eight ingredients, pound the first four together and grind the last four separately until extremely fine. Mix all sixteen ingredients evenly. Form pills with refined honey, size of mung beans. Take 1-2 pills per dose, warm wine with empty stomach, noon, and before bed. Gradually increase to 3-5 pills.
For traumatic stroke with trismus, facial deviation, body stiffness or flaccidity, and pediatric convulsions, use Zouma San:
Tianma (Gastrodiae Rhizoma), Tiannanxing (Arisaematis Rhizoma, prepared), Banxia (Pinelliae Rhizoma, washed seven times in hot water, pounded with 0.5 liang of fresh ginger, dried), Baifuzi (Typhonii Rhizoma, prepared), Fuzi (Aconiti Lateralis Radix Praeparata, cracked, peel and navel removed, 0.5 liang each), Dansha (Cinnabar, ground), Xionghuang (Realgar, ground), Niuhuang (Calculus Bovis, ground), Shexiang (Moschus, ground), Xijiao (Rhinocerotis Cornu, sliced, 1 fen each), Nifen (Calomel, ground, 3 fen).
Pound six ingredients into powder, mix with five ground ingredients. Take 0.5 qianbi per dose, mixed with Doulinjiu (bean-leavened wine). Sweating indicates cure. If no sweat, administer another 'yi zi' dose, followed by hot ginger thin rice gruel. For children, take 'yi zi' dose, mixed with Jingjie Tang (Schizonepeta Decoction) or boiled water. To make pills, use fresh rice.

Wuya (Corvus Corvax, one, remove beak, feet, feathers, wings), Hulu (Vulpis Hepatic, one, place with crow in jar, seal with earth mud, burn until smoke ceases, use 3 liang, pound into powder, add the following): Tianma (Gastrodiae Rhizoma), Baifuzi (Typhonii Rhizoma, prepared), Tiannanxing (Arisaematis Rhizoma, prepared), Baijiangcan (Bombyx Batryticatus, stir-fried), Wushao She (Zaocys, wine-soaked, skin and bones removed, roasted), Huoxiangye (Agastaches Folium), Sangbaipi (Mori Cortex).

Ten ingredients. Pound three into fine powder, mix with seven ground ingredients. Form pills with refined honey, size of paulownia seeds. Take 1-2 pills per dose, warm wine. Drink moderately to achieve slight intoxication, not drunkenness. Also treats stroke with limb contracture and hemiplegia. Twice daily, once at night. Do not overdose.

Sixteen ingredients. Crush thirteen into powder, mix with three ground ingredients. Take 1 qianbi per dose, warm wine, anytime.
For traumatic tetanus with trismus, aphonia, salivation, and throat sounds, use Liuwei Zhuifeng San:
Fuzi (Aconiti Lateralis Radix, raw), Baifuzi (Typhonii Rhizoma, remove tip), Wutou (Aconiti Radix, remove tip), Tiannanxing (Arisaematis Rhizoma), Banxia (Pinelliae Rhizoma, washed seven times, ginger-processed, 0.5 liang each), Ganxie (Dried Scorpio, whole, stir-fried, 1 fen).
Six ingredients. Crush into powder, mix with 1 fen Nifen (Calomel). Take 'yi zi' to 0.5 qianbi per dose, bean-leavened wine, anytime. Apply dry powder to sores.
For traumatic tetanus with opisthotonos, use Shougong Wan:
Shougong (Gecko, roasted dry, remove feet, 7 pieces), Tiannanxing (Arisaematis Rhizoma, 1 liang, wine-soaked 3 days, dry-dried, pound these two into powder), Nifen (Calomel, 0.5 fen).
Three ingredients. Grind together finely. Make pills with thin flour paste, size of mung beans. Pry mouth open, give 7 pills with wine. After a while, sweating occurs. Give another dose; second sweating indicates cure.
For traumatic tetanus with swelling and trismus, use Baijiangcan San:
Baijiangcan (Bombyx Batryticatus, straight ones, any amount).
One ingredient. Raw grind into powder. Mix with fresh ginger juice, sweep onto sore with chicken feather, keep moist. Swelling reduction and wrinkling indicate effect. Also take 0.5 qianbi mixed with ginger juice internally.
For traumatic tetanus with fever, headache, nausea, and restlessness, use Wutou San:

For traumatic tetanus with chills, fever, restlessness resembling cold syndrome, use Zhuifeng Bai Wanzi:
Wugong (Centipeda, red legs, soak in ginger juice overnight), Baihuashe Neck Meat (wine-soaked overnight, roast), Ruxiang (Olibanum, ground), Shexiang (Moschus, ground), Fuzi Jian (Aconiti Lateralis Radix Praeparata tip)...
Five ingredients. Grind into powder. Form pills with refined honey, size of radish seeds. Based on patient size, dissolve 1-5 or 10 pills in mint boiled water. Induce sweating, bathe in peach/willow soup, follow with hot rice drink. Sweating indicates cure.
For traumatic tetanus with blunt trauma internal injury, use Yinhua San:
Tiannanxing (Arisaematis Rhizoma, large, half raw half prepared), Baifuzi (Typhonii Rhizoma, half raw half prepared), Fangfeng (Saposhnikoviae Radix, remove forked tips, equal parts).
Three ingredients. Fine powder. Take 2 qianbi per dose. Severe: young boy's urine. Mild: hot wine. For external wounds, apply powder to sore.
