论述说：《内经》指出，风、寒、湿三种邪气混杂侵袭人体，结合而形成痹证。又说：在冬季遇到这些邪气的，称为骨痹。骨痹如果不愈，再次感受邪气，邪气内传并停留在肾脏，这就称为肾痹。其症状表现为容易腹胀，臀部代替脚跟行走（形容下肢严重挛缩，无法直立），脊背弯曲代替头部（形容脊柱严重变形）。因为肾是胃的关口，如果关门不利，胃气就不能正常运行，所以容易腹胀；筋骨受到拘束压迫，所以下肢挛急，上身躯体蜷曲，因此说“代踵代头”。
治疗肾脏虚衰，长期感受寒湿，因而形成痹证。采用补益元气的方法。远志丸方。

治疗肾脏虚冷，邪气乘虚而入，身体寒冷麻木不仁，手足牵引僵硬，活动困难；或者肌肉跳动，牵引腰脊及左右侧筋脉拘急，不能进食；或者因房事诱发。防风丸方。

治疗肾脏受风湿侵袭，腰痛、脚膝偏枯（半身不遂或局部瘫痪），皮肤麻木不仁。说话声音艰涩，双耳空虚鸣响，全身乏力，面色无华，情绪低落，骨节酸痛。茵芋散方。

Commentary states: The *Neijing* (Inner Canon) asserts that wind, cold, and dampness pathogenic factors mix and invade together, combining to form Bi syndrome (obstruction syndrome). It further states: contracting these pathogens in winter results in Bone Bi. If Bone Bi is not resolved and the body is re-exposed to pathogens, the pathogens move inward and lodge in the Kidney, which is termed Kidney Bi. Clinical manifestations include frequent abdominal distension, walking with the buttocks replacing the heels (indicating severe lower limb contracture preventing upright posture), and the spine replacing the head (indicating severe spinal deformity). This is because the Kidney serves as the gate of the Stomach; if this gate is obstructed, Stomach Qi cannot circulate freely, leading to frequent distension. Due to the constraint and compression of sinews and bones, the lower limbs become cramped and the upper body curls up, hence the description of "heels replaced by buttocks" and "head replaced by spine."
Treatment for Kidney deficiency and exhaustion, where long-term exposure to cold and dampness leads to Bi syndrome. The therapeutic method is tonification and replenishing Qi. Yuanzhi Wan (Polygala Pill) Formula:

Treatment for Kidney deficiency with Wind-Damp invasion, flaccidity and weakness of waist and legs, stubborn numbness and insensitivity, dark complexion, turbid voice, unstable will, dizziness and blurred vision, stiff and painful lower back, cramped limbs, heavy body and weakness. The above seventeen ingredients are chopped like hemp seeds, placed in a raw silk bag, and soaked in one dou of wine. Soak for three days in spring/summer, five days in autumn/winter. Take half a zhan on an empty stomach, warm, twice daily.
