Whenever there are carbuncles, ulcers, or toxic sores, fever occurs at specific times, worsening in the late afternoon and subsiding in the morning. If during the summer months, there is redness and swelling on the head, face, and back of the neck, or sore throat and swelling, or severe swelling of the legs and feet extending several inches, making walking impossible, people often suspect it to be a sore. However, accompanied by headache and internal dryness/heat, with continuous fever day and night, it naturally differs from typical sore conditions. By using Baidu San (Decoction for Defeating Toxins) added with Shigao (Gypsum Fibrosum), Huanglian (Coptidis Rhizoma), and other herbs, once the heat syndrome resolves, the redness and swelling will subside spontaneously, with absolutely no pus or blood. This is named Shuyang (Summer-Heat Carbuncle), where a difference of a hair's breadth leads to an error of a thousand miles.
Lin's Commentary: In the syndrome of Shuyang, when Heat pathogen prevails, there is redness, swelling, hardness, and pain; when Wind pathogen prevails, there is itching; when Dampness pathogen prevails, there is abundant pus and discharge. Treatment should focus on clearing and venting to drain heat, dispelling wind and removing dampness, adjusting according to the severity of the condition.
In the height of summer, Fire can scorch Metal (Lung). If one does not abstain from pungent foods and alcohol, Spleen Fire may surge violently, causing labor-like heat and dryness disturbing the organs, stirring up Fire in the Heart and Lungs. This causes coughing, wheezing, sudden hematemesis (vomiting blood) or epistaxis (nosebleed), cloudiness of the head and eyes, and restlessness and irritability in the chest and diaphragm. Even if children or the elderly occasionally suffer from this, ignorant practitioners mistake it for Laozhi (Consumption/Tuberculosis). They do not realize that the Fire is above the Blood, which is not equivalent to deficiency consumption caused by depletion of True Yin. It is suitable to use Siwu Tang (Four Substances Decoction), Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity), and Erchen Tang (Two Aged Herbs Decoction). From these three formulas, remove Chuanxiong (Cyperi Rhizoma), Baishao (Paeoniae Radix Alba), and Huangbai (Phellodendri Cortex); replace Banxia (Pinelliae Rhizoma) with Beimu (Fritillariae Bulbus); add Jiegeng (Platycodonis Radix) to restrain, Bohe (Menthae Haplocalycis Herba) to disperse, Maidong (Ophiopogonis Radix) and Wuweizi (Schisandrae Fructus) to astringe, and self-healing will occur. Alternatively, add Tongbian (Urine of Child) or Ouji (Nelumbinis Nuciferae Rhizoma Juice), or take one or two doses of Huanglian Xiangru Yin (Coptis and Elsholtzia Drink). Rest quietly for several days, avoid alcohol and fried foods, and peace will return. This is named Shuzhai (Summer-Heat Consumption), which should be considered and applied wisely. Using Dongyuan's Shenling Tiaozhong San (Ginseng and Atractylodes Powder for Regulating the Middle) is also excellent.
Lin's Commentary: Shuzhai arises because Yin Qi fails to generate and Yang Qi fails to hide. Symptoms include hemoptysis and hematemesis, feeling hot and irritable in the late afternoon, clarity in the morning, white tongue coating with thirst, head distension, and body ache. These are all due to Summer-Heat pathogenic factors invading internally, depleting Yin and damaging collaterals. Even if there are chronic ailments, one must first clear the heat in the collaterals. Suitable herbs include Shashen (Glehniae Radix), Tian Xingren (Sweet Apricot Seed), Chuan Bei (Fritillariae Cirrhosae Bulbus), Loupi (Trichosanthis Pericarpium), Lianqiao (Forsythiae Fructus), Maidong (Ophiopogonis Radix), Zhuye (Bambusae Folium), and Xian Shengdi (Rehmanniae Radix Recens). Mix with one cup of Xian Heye Zhi (Fresh Lotus Leaf Juice) for administration, which yields remarkable results. After bleeding stops, discuss nourishing Yin. If the sequence is reversed, or if warming and tonifying herbs are mistakenly used, the latent heat in the collaterals cannot reach the exterior, inevitably becoming an incurable condition. Physicians seeing a white tongue coating often suspect latent Cold and dare not directly use cooling herbs, leading to delays and errors. They fail to understand that a white tongue coating is due to Summer-Heat damaging Qi; the Lung governs Qi and belongs to Metal, and the color of Metal is white.
