Translation focuses on the anatomical locations and nomenclature of sores and ulcers. Although yin and yang (deficiency and excess) have not yet been distinguished, students should be familiar with these various names. For instance, toxins arising on the vertex are named Shan Fa Ju (善发疽); those on the neck are called Luo Tou (落头), Dui Kou (对口), or Nao Ju (脑疽). Ya Yan (鸭蜒) occurs when toxins are trapped in the axilla; Yu Du (鱼肚) arises on the calf muscles; Shi Rong (失荣) is located exclusively in the neck region; Jia Ju (夹疽) appears bilaterally on the sides of the throat; sores on the foot bones are termed Jia Gun (夹棍); if skin and flesh ulcerate, they are called Lv Yan (驴眼). The Yongquan acupoint (Yongquan, KI1) is at the sole of the foot; if toxins hide beneath the foot skin, they form Niu (牛 - likely referring to a specific type of lesion like 'Niu Dou' or similar, kept as per text). Abdominal abscesses refer to midline lesions, while Jian Dai (箭袋) refers to lateral ones. Chou Tian Luo (臭田螺) denotes rotting of the big toe; Bian Dan Guai (扁担怪) refers to boils in shoulder acupoints; Bin Qian Ju (鬓前疽) appears behind the ears near the hairline. Leg sores are called Tui Yong (腿痈); lower leg lesions are known as Kua Ma (跨马); white-colored ones are called Dong Gua (冬瓜). Shou Bei (手发背) affects the hand back; Jiao Ya Ju (脚丫疽) affects the toes; Tou Fen Lao Shu (偷粪老鼠) is also known as Xuan Yong (悬痈). Lou (漏) is called Hai Di (海底); finger sores are She Tou (蛇头); nail bed infections are Jia Ju (甲疽). Knee swelling is He Xi (鹤膝); testicular pain is Zi Yong (子痈). Ma Dao (马刀) sores appear on the face; Gu Cao Feng (骨槽风) affects the gums; Jing Quan Ju (井泉疽) affects the chest/heart area; Tie Gu Ju (贴骨疽) toxins reside at the Huantiao acupoint (Huantiao, GB30). Buttock toxin accumulation is Bi Ju (臀疽); arm abscesses are Bi Du (臂毒). Names are deduced from locations; treatments differ based on redness or whiteness. In the early stage before ulceration, observe the current appearance; after prolonged ulceration, inquire about the initial color. If initially red, apply formulas for Yang sores (Yong); if initially white, apply formulas for Yin ulcers (Ju). Specific treatments follow below to help students distinguish symptoms and refine their practice.
Ma states: Early-stage lesions belong to Yang syndromes. If they ulcerate for a long time, leading to Qi and Blood deficiency and transforming into Yin-Cold syndromes, one must absolutely avoid using formulas for Yang sores.
If chronic ulceration results in severe rotting, administer Dong Tian Jiu Ku Dan (Cave Heaven Saving Bitter Pill) three doses, each dose being 3 qian, taken with aged wine until drunk and sweating under covers. After two days, administer another dose; after another two days, administer a third dose. During the two-day intervals, take Xing Xiao Wan (Wake-Up Eliminating Pill) 3 qian daily with aged wine. Once toxic fluids have drained completely, take Xing Xiao Wan twice more after seven days, followed by Da Zao Wan (Jujube Pill), 5 qian morning and evening. Even in critical cases, this can yield results.
Ma states: This pill formula treats syphilis and damp-toxin conditions where sores ulcerate into large patches with dozens of holes discharging foul fluid. However, it should not be used for general sores and ulcers that have ulcerated into large patches with dozens of holes, unless specifically indicated for such toxic manifestations.
