Knee Carbuncle (Xi Yong)
A carbuncle occurring on the lateral aspect of the knee is caused by toxic pathogens in the Foot-Shaoyang Gallbladder Meridian (Zu Shao Yang Jing). This area is characterized by abundant Qi and scarce Blood. A carbuncle occurring on the medial aspect of the knee is related to the Foot-Taiyin Spleen Meridian (Zu Tai Yin Jing), which also features abundant Qi and scarce Blood. The knee joint serves as a pivotal articulation for movement; therefore, it requires careful treatment. Neglect may lead to disability. One must exercise caution.
Breast Carbuncle (Ru Yong)
The most severe swelling of the breast is termed "Ru Fa" (Breast Erysipelas/Severe Breast Abscess), followed by "Ru Yong" (Breast Carbuncle). If a nodule appears at the onset, it is called "Ru Ju" (Breast Deep-Seated Abscess), accompanied by aversion to cold, high fever, and nausea. The breast tissue belongs to the Foot-Yangming Stomach Meridian (Zu Yang Ming Wei Jing), which is rich in both Qi and Blood. The nipple belongs to the Foot-Jueyin Liver Meridian (Zu Jue Yin Gan Jing), which is rich in Blood but poor in Qi. When occurring during pregnancy, it is called "Nei Chui" (Internal Blowing); when occurring during lactation, it is called "Wai Chui" (External Blowing). It should be treated promptly to dissipate toxicity, soothe Liver Qi, and clear Stomach Qi. Once the Stomach Qi ulcerates, pus will discharge. In women over fifty years old, with declining Qi and Blood and chronic depression, nodules may form in the breasts that hurt on rainy days; this is called "Ru He" (Breast Nodule). After one and a half to two years, if it ruptures and discharges continuous pus without healing, it is called "Ru Lou" (Breast Fistula). Some women who adopt children but lack milk may force infants to suckle, leading to chronic ulcers that persist for years, sometimes rotting halfway like a broken lotus pod, causing unbearable pain. Because necrotic tissue remains inside, healthy flesh cannot regenerate, resulting from damp-heat in the Yangming Stomach; this is called "Ru Gan" (Breast Chancre/Gan). Treatment involves clearing Stomach heat, administering Da Bu Xue Qi Tang Wan (Major Qi and Blood Tonifying Decoction/Pills), and applying external Qi and Blood tonifying plasters with Hong Fen Shuang (Red Powder Frost) for better effect. Another condition involves hard, stone-like breast masses that do not ulcerate for months and cause frequent pain, known as "Ru Yan" (Breast Rock/Cancer). Urgent dispersal of stagnation, reduction of swelling, and detoxification are required, otherwise it is difficult to treat. Jiang Shuang (Falling Frost) can be applied topically. If there is redness and swelling on the nipple, about two or three cun in diameter, round and headless, it is called "Ru Ji" (Breast Furuncle). These ten types of breast conditions have been detailed and clarified.
Left and Right Genital Abscess (Zuo You Bian Yong)
This sore arises in the inner thigh region near Henggu (Henggu, GB29) and Yinlian (Yinlian, LR11) acupoints, involving the Liver and Kidney meridians, an area rich in Blood but poor in Qi. Patients experience alternating chills and fever, with local swelling and pain. It may occur unilaterally (left or right) or bilaterally. Treatment should focus on dissipating damp-heat from the Liver meridian and detoxifying. Moxibustion is effective in the early stages.
