\r尾闾痈\p04-a55a39.bmp\r
\r臀痈\p04-a55a40.bmp\r
\r骨痈\p04-a55a41.bmp\r

\r脾痈\p04-a55a44.bmp\r
Wei Li Carbuncle (Carbuncle at the Wei Li Acupoint)
This carbuncle occurs at the Wei Li acupoint. This area belongs to the Du Meridian (Governing Vessel). The pain is extremely severe and unbearable, and suppuration is difficult to achieve. It is appropriate to use Da Tuo Li Tang (Great Support Interior Decoction) added with Ruxiang (Olibanum, 乳香) and Moyao (Myrrha, 没药) to drain pus and relieve pain. External application of poultices should be used.
One must abstain from anger and sexual intercourse; otherwise, it may lead to a fistula that is difficult to cure.
Buttock Carbuncle (Dan Carbuncle)
The buttocks belong to the Foot-Taiyang Bladder Meridian, which is characterized by abundant blood and scarce Qi. Since essence, flesh, Qi, and blood rarely reach this area, the pain is most severe. If weakness is observed, one should immediately employ the interior-supporting method to tonify blood and Qi. If the sore tends to spread towards the hip bone and the Huantiao acupoint, it involves the Foot-Shaoyang Gallbladder Meridian, which has scarce blood and abundant Qi; adding meridian-guiding herbs would be even more effective. The situation is similar on both sides, involving the Chengfu acupoint.
Bone Carbuncle (Gu Carbuncle)
This sore persists for one or two years without healing, often shedding a piece of bone or a thin bone fragment. There may be carious holes, or pieces may shed every three to five months. This is named "Duog Chuang" (Sore with Multiple Bones). It is not generated by the failure of Ying-Qi (Nutritive Qi) to flow smoothly, but rather results from the mother's conception followed by the reception of Jing-Qi (Essence-Qi). Using Feizhi Tuoseng (Calcined Zinc Oxide, 飞过陀僧) mixed with Tongyou (Camellia Seed Oil, 桐油) to make a paste, spread on cloth, and applied externally yields good results.
Waist Carbuncle (Yao Carbuncle)
This sore develops below the soft ribs near the Belt Vessel (Dai Mai), specifically around the Wushu and Weidao acupoints. It belongs to the Foot-Shaoyang Gallbladder Meridian, characterized by abundant Qi and scarce blood. Symptoms include alternating chills and fever, swelling, and pain. I treated one patient who discharged over a bucket of pus; he recovered through interior support and strong tonification of Qi and Blood. The condition is similar on both sides.
Internal Carbuncle (Nei Carbuncle)
The origin of carbuncles and sores has no fixed location. Internally, it arises from the combination of the Seven Emotions (Seven Affects, 七情) and rich/flavorful foods leading to stagnation. Sores appearing on the external body are easy to treat. However, if pain occurs in the blind membranes beside the body cavities and does not respond to medication, heavy palpation of the pulse reveals a Kou (Hollow) and Shu (Rapid) quality. This is not a pain pulse but indicates the presence of pus. Administer Siwu Tang (Four Substances Decoction, 四物汤) added with Jiegeng (Platycodon Root, 桔梗), Chaihu (Bupleurum Root, 柴胡), Xiangfu (Cyperus Rhizome, 香附), and Zaocis Spina (Gleditsia Sinensis Thorn, 皂角刺) for several doses. After pus is discharged via the stool, the patient recovers.
Spleen Carbuncle (Pi Carbuncle)
This carbuncle arises at the Zhenyu acupoints on the shoulder. It belongs to the Hand-Taiyang Small Intestine Meridian, characterized by abundant blood and scarce Qi. It is generated by the Spleen being affected by rich/flavorful foods. It is appropriate to induce sweating. As the classics state: "Inducing sweat stops the sore." The toxin disperses through sweat. It is appropriate to use Xianfang Huoming Yin (Immortal Life-Extending Powder, 仙方活命饮) with modifications, along with herbs that guide the action into the Spleen Meridian, to push the toxin outwards, combined with external topical applications for efficacy.
