The toxic pathogen of Xuan Yong (Hanging Abscess) arises at the Huiyang acupoint. Initially, it is as small as a lotus seed, gradually enlarging to the size of a peach. This condition results from deficiency of the Three Yin meridians and stagnation of damp-heat; if it ulcerates and fails to heal for a long time, forming a fistula, it evolves into Chuang Lao (chronic consumptive sore).
[Note] This syndrome is also known as Qima Yong (Horse-Riding Abscess), occurring in the perineum (specifically at the Pingyi acupoint located behind the anterior orifice and in front of the posterior orifice, which is also the Yinhui acupoint, the first point of the Ren Mai meridian). It initially appears like a lotus seed, with slight itching and significant pain. Over time, it swells and takes on the shape of a plum or peach. It is caused by deficiency of the Three Yin meridians combined with emotional worry leading to Qi stagnation, resulting in the accumulation and stagnation of damp-heat. The color becomes red as pus forms and rupture approaches. If the rupture leads to a deep wound that persists for a long time, it forms a fistula, exhausting Qi and Blood, and transforming into Chuang Lao. In the early stage, if the patient's constitution is strong and solid, but no pus has formed yet, accompanied by difficult and scanty urination, Jiulong Dan (Nine Dragons Pill) should be used to purge the root of the disease. For patients with slightly deficient constitutions, Xianfang Huoming Yin (Immortal Life Elixir Decoction) should be used to drain damp-heat; treating according to this method can resolve three or four out of ten cases. If after more than ten days the swelling has fully formed and cannot be resolved internally, Tuoli Xiaodu San (Interior-Supporting Detoxification Powder) or Tuoli Tou Nong Tang (Interior-Supporting Pus-Releasing Decoction) should be taken to induce spontaneous rupture. If it does not rupture spontaneously, and the swelling is high, shiny, and distendingly painful, a horizontal needle incision should be performed. If foul pus drains out and the swelling completely subsides, this is a favorable outcome (Shun). Take Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) in the morning and Shiquan Dabu Tang (Ten-Ingredient Great Tonifying Decoction) at noon to warm-tonify and nourish Yin. For those who have consumed excessive rich and greasy foods and have a robust constitution, Longdan Xiegan Tang (Gentian Liver-Draining Decoction) should be taken initially, followed by Ziyin Bawu Tang (Yin-Nourishing Eight-Ingredient Decoction) after rupture. For those with excessive sexual activity and a emaciated constitution, Bazhen Tang (Eight-Treasure Decoction) should be taken initially, followed by Shiquan Dabu Tang after rupture; for those with Spleen deficiency and poor appetite, Liujunzi Tang (Six-Gentleman Decoction) should be used. For chronic cases that have formed fistulas, Guolao Gao (Old Man's Paste) dissolved in water should be used to deliver Hubo Lafan Wan (Amber and Alum Pill). External treatment methods follow the protocols in the "Ulcerative Sores and Carbuncles" section. One must abstain from sexual intercourse, anger, and stimulating foods such as fish and seafood, and manage care prudently.
Jiulong Dan (Nine Dragons Pill)
Muxiang (Aucklandiae Radix), Ruxiang (Olibanum), Moyao (Myrrha), Ercha (Catechu), Xuexie (Dragon's Blood), Badou (Crotonis Fructus, oil not removed).
Grind all ingredients into equal parts powder. Mix with raw honey to form a paste, and store in a porcelain jar. When needed, roll into pills the size of pea seeds. Take nine pills each time, swallowed with a cup of hot wine on an empty stomach. After four or five bowel movements, one may consume thin porridge; for severe swelling, administer another dose every other day until it resolves spontaneously.
[Fang Ge - Formula Song] Jiulong Dan treats the toxin of Xuan Yong, when Qi is excess and pus has not yet formed; Muxiang, Ruxiang, Moyao, Ercha, and Xuexie, mixed with honey and taken with wine, are effective.
Ziyin Bawu Tang (Yin-Nourishing Eight-Ingredient Decoction)
Danggui (Angelicae Sinensis Radix), Shengdihuang (Rehmanniae Radix Recens), Baishaoyao (Paeoniae Radix Alba, stir-fried with wine), Chuanxiong (Ligustici Chuanxiong Rhizoma), Danpi (Moutan Cortex), Fengfen (Bletillae Rhizoma) (each 1 qian), Zexie (Alismatis Rhizoma) (0.5 qian), Gancao Jie (Scutellariae Radix node-like part? Note: Text says Gancao Jie, usually refers to licorice nodes or specific preparation, here likely Licorice Root nodes/sections) (1 qian).
Add two cups of water and fifty cun of Dengxin Cao (Junci Medulla). Decoct until eight fen remains. Take before meals. For constipation, add 1 qian of Mahuangda Huang (Rhei Radix et Rhizoma stir-fried with honey).
[Fang Ge - Formula Song] Ziyin Bawu treats cases following rich diet, take this formula for ruptured Xuan Yong; Siwu, Danpi, Fengfen, and Zexie drain heat, add Da Huang for constipation with Gancao Jie.
