Gan Ju (Sweet Carbuncle) arises from Qi stagnation caused by worry and overthinking. It manifests on the chest (Ying), appearing initially like a grain of millet with a purple, trichosanthes-like shape. Chills, fever, hardness, and pain are present; it should be allowed to suppurate quickly. Delayed suppuration requires vigilance against toxic invasion inward.
[Note] This condition is caused by Qi stagnation due to worry and overthinking. It occurs on the upper chest, specifically the fleshy prominence on both sides, located below the Zhongfu acupoint (LU-1) of the Lung Meridian. It can affect either the left or right side. Initially, it resembles a millet grain in color (greenish), gradually becoming purple like a trichosanthes fruit, hard and painful, accompanied by aversion to cold and high fever. Rapid suppuration with thick pus indicates a favorable prognosis (Shun). If chills and fever persist beyond ten days without pus formation, and the pulse is floating and rapid, one must guard against toxic invasion inward, which leads to malignant complications and indicates an unfavorable prognosis (Ni). Initially, Jingfang Baidu San (Atractylodes and Saposhnikovia Decoction for Overcoming Toxins) is suitable to disperse cold and heat, followed by Neituo Huangqi San (Internal Support Astragalus Decoction). If it does not suppurate when expected, Shiquan Dabu Tang (Perfect Ten Tonifying Decoction) should be taken urgently to support the expulsion of toxins. For other internal and external treatments, refer to the sections on Carbuncles and Ulcers (Swelling/Ulcerated stages).

Danzhong Ju (Carbuncle at Danzhong) begins as a millet-grain shape, purple and hard, gradually becoming painful. It arises from Fire-Toxin triggered by emotional disturbances, affecting the Ren Mai (Conception Vessel). Urgent treatment according to symptoms is required; delay leads to ominous outcomes.
[Note] This condition occurs above the heart pit, in the center between the two breasts, corresponding to the Danzhong acupoint (CV-17) of the Ren Mai (Conception Vessel). It is formed by disharmony of the Zang-Fu organs and irregular emotions, leading to the condensation of Fire-Toxin. Initially, it appears like a millet seed, purple and hard, gradually developing redness, heat, swelling, and pain, accompanied by aversion to cold and high fever. Xianfang Huoming Yin (Immortal Life Decoction) added with Perilla Leaf (Zi Su Ye) and Mint Leaf (Bo He Ye) should be taken urgently to induce sweating. If restlessness, vomiting, dry lips, and extreme thirst occur, Duoming Dan (Life-Snatching Pill) should be used to clear heat. Once exterior symptoms have subsided, Tuoli Tou Nong Tang (Interior-Supporting Pus-Penetrating Decoction) should be taken urgently. If the lesion fails to rise, indicating deficiency, Shiquan Dabu Tang (Perfect Ten Tonifying Decoction) should be used to support the expulsion of toxins. However, Danzhong is the residence of Qi and Sea of Qi; treatment must be early. Delay allows toxin to invade inward, damaging the membranes and obstructing Qi flow, which is an unfavorable prognosis (Ni). All other internal and external treatments follow the protocols for Carbuncles and Ulcers (Swelling/Ulcerated stages).
