Carbunculus Apiformis resembles the structure of a honeycomb and typically arises on the posterior aspect of the shoulder or beside the spine. This condition strictly contraindicates the upward direction of the abscess head; urgent clearing of Heart Fire is required to prevent the internal invasion of pathogenic toxins.
[Note] This lesion commonly occurs behind the shoulders and alongside the spinal column, resembling a honeycomb. It is caused by accumulated heat in the Spleen Meridian compounded by the coagulation of Heart Fire into toxicity. Initially, it presents as high swelling resembling a turtle shape; if the swelling occupies only half of the back, the prognosis is mild. If the lesion spreads horizontally and covers a large area, the condition is severe. The appropriate prescription is Neishu Huanglian Tang (Internal Draining Coptis Decoction). If the abscess head is pointed and directed upward, it indicates extreme excess of Heart Fire, necessitating prevention against the internal attack of toxic fire on the Zang-Fu organs. There is also a type where the lesion extends up to one chi in length with roots spreading across the entire back, named Jingti Ju (Whole-Body Carbuncle), which belongs to highly dangerous toxicity. Upon initial sensation, one should immediately take Huanglian Xiaodu Yin (Coptis Detoxification Decoction) to clear the Heart and detoxify, thereby avoiding internal invasion. All other internal and external treatment methods follow the chapters on Carbuncles and Ulcers (Swelling/Ulceration phases).

Neishu Huanglian Tang (See Chapter on Swelling Phase)
Huanglian Xiaodu Yin (See Section on Baihui Ju of the Head)
Yinyang Erqi Ju (Yin-Yang Qi Carbuncle) arises beside the spinal column; the swelling fluctuates, and the texture alternates between soft and hard. This is generated by internal disharmony of the Seven Emotions, leading to disordered Ying and Wei (Nutritive and Defensive) Qi. Initial onset causes alternating chills and fever. If accompanied by intense thirst, clear consciousness, high swelling, and a surging pulse, pus forms after fourteen days, and rupture indicates a favorable course (Shun). If there is no thirst, coma, diffuse swelling without a head, a fine pulse, absence of pus at the expected time, and loss of appetite, it indicates an unfavorable course (Ni). Initially, administer Duoming Dan (Life-Restoring Pill) to resolve chills and fever, followed by Xianfang Huoming Yin (Immortal Formula for Preserving Life Decoction). All other internal and external treatments follow the chapters on Carbuncles and Ulcers.

Duoming Dan
Qingfen (Calomelas), Shexiang (Moschus), Baipi (Arsenicum Album wrapped in flour and roasted, each 5 fen), Baijian (Alumen), Chensha (Cinnabar used as coating), Xuexie (Draco Sanguis, each 1 qian), Xionghuang (Realgar, 2 qian), Chansu (Bufo Toxin, dried, dissolved in wine for use, 2 qian), Ruxiang (Olibanum), Moyao (Myrrha), Hanshuishi (Aqua Calcitae), Tonglv (Chalcanthite, each 2 qian), Woniu (Helix Aspersa, with shell, 21 pieces).
Grind all ingredients into fine powder. First, mash the snails into a paste and mix uniformly with the aforementioned medicines. If difficult to form pills, add a small amount of good yellow wine and beat three to five hundred times to form pills, sized like mung beans. Take two or three pills per dose. Take one cun (approx. 3.3 cm) of scallion white, have the patient chew it thoroughly, spit it into their palm (men use left hand, women use right hand), wrap the pill in the scallion paste, and swallow with a large cup of warm, unadulterated rice wine. Cover with blankets until sweating occurs. For severe cases, do not exceed three doses; do not overuse.
[Formula Song] Duoming Dan contains Calomelas and Moschus, combined with Arsenic, Alumen, Cinnabar, Draco Sanguis, and Realgar; Bufo Toxin, Olibanum, Myrrha, and Aqua Calcitae are added, while Chalcanthite and Helix Aspersa are most efficacious.
