颠倒散(参见“肺风粉刺”条)
Jiu Bi (Alcoholic Rhinophyma) occurs on the nasal bridge and sides. It is caused by stomach fire steaming the lungs combined with external cold invasion. Initially, blood stasis presents as redness; after a long time, it turns purplish-black. Treatment requires dispersing stagnation and activating blood circulation to achieve gradual recovery.
[Note] This condition arises at the nasal tip and alae nasi. It results from stomach fire steaming the lung, compounded by wind-cold constraining the exterior, leading to blood stasis and coagulation. Thus, it first appears red, then purple, and eventually black if prolonged, making it particularly persistent and difficult to cure. The treatment should focus on diffusing lung qi stagnation and resolving stagnant blood. Formulas such as Mahuang Xuanfei Jiu (Ephedra Decoction for Diffusing Lung Qi) and Liangxue Siwu Tang (Cooling Blood Four Substances Decoction) can be selected to promote the circulation of Ying and Wei (nutritive and defensive qi), thereby nourishing new blood. Additionally, Diandao San (Inverted Powder) should be applied topically to the affected area. If the condition persists for a long time without healing, Zhizi Ren Wan (Gardenia Seed Pill) can be taken orally for gradual improvement.
Mahuang Xuanfei Jiu (Ephedra Decoction for Diffusing Lung Qi)
Ingredients: Ephedrae Herba (Mahuang, 麻黄) and Radix Ephedrae (Mahuanggen, 麻黄根) (each 2 liang).
Preparation and Administration: Use 5 hu (ancient liquid measure) of first-press wine. Place the herbs in the wine, boil in a double boiler for the duration of three incense sticks, and leave exposed overnight. Drink 3–5 cups morning and evening. After 3–5 days, pus will form and sores will appear; after more than 10 days, the pus will drain completely, the redness will fade, turning yellow then white, leading to recovery.
[Formula Song]: Mahuang Xuanfei Jiu treats Jiu Bi, where blood heat ascends and external cold causes stasis. Soaking Mahuang and its root in wine and boiling it yields reliable efficacy.
Liangxue Siwu Tang (Cooling Blood Four Substances Decoction)
Ingredients: Angelicae Sinensis Radix (Danggui, 当归), Rehmanniae Radix Preparata (Shengdi, 生地), Chuanxiong Rhizoma (Chuanxiong, 川芎), Paeoniae Radix Rubra (Chishao, 赤芍), Scutellariae Radix (Huangqin, 黄芩) (stir-fried with wine), Poria (Chifuling, 赤茯苓), Citri Reticulatae Pericarpium (Chenpi, 陈皮), Carthami Flos (Honghua, 红花) (stir-fried with wine), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) (raw, each 1 qian).
Preparation and Administration: Add 2 zhong (ancient volume measure) of water and 3 slices of Zingiberis Rhizoma Recens (Shengjiang, 生姜). Decoct until 8 fen remains. Add 1 cup of wine and 2 qian of powder of Trogopteri Dung (Wulingzhi, 五灵脂). Take hot. For patients with Qi deficiency, add 2 qian of Astragali Radix (Huangqi, 黄芪) stir-fried with wine for immediate effect.
[Formula Song]: Liangxue Siwu Tang treats redness in Jiu Bi, dispersing stasis, resolving stagnation, and regulating Ying and Blood. Huangqin, Fuling, Siwu Tang, Chenpi, Honghua, and Gancao are decocted with Shengjiang, adding wine and Wulingzhi.
Zhizi Ren Wan (Gardenia Seed Pill)
Preparation: Grind Gardeniae Fructus (Zhizi, 栀子) seeds into powder. Melt Carnauba Wax (or beeswax/Huangla, 黄蜡) and mix with the powder to form pills the size of a bullet.
Administration: Take one pill per dose, chewed down with clear tea water. Avoid spicy foods.
[Formula Song]: Zhizi Ren Wan treats reddish Jiu Bi, applicable even when purplish-black and persistent. Gardenia powder mixed with melted wax forms bullet-sized pills taken with tea.
