Tongue disorders originate from the Heart and Spleen meridians, and all such conditions arise from accumulated heat. Heavy tongue (Chong She) presents with swelling of the blood vessels under the tongue; Phlegm nodule (Tan He) presents with a nodule growing on the tongue; Chong Ding manifests in the upper oral cavity, accompanied by frequent mental restlessness, resembling a plum shape; Ling Ding grows on the tongue, appearing as purple spots, shaped like beans, with aggravated chills and fever.
[Note] This disorder can occur in both adults and children. Heavy tongue is caused by heat accumulation in the Heart and Spleen, which follows the meridians upward to the root of the tongue, causing the blood vessels under the tongue to swell and protrude like a small tongue, hence the name "Heavy Tongue." It is suitable for topical application of Bingpeng San. Phlegm nodule results from phlegm-fluids stagnating and transforming into heat in the Heart and Spleen, forming a hard, painful nodule on the tongue. It is suitable to puncture the nodule with a clothing needle, apply Bingpeng San topically, and administer Jiawei Erchen Tang orally. Chong Ding is caused by heat in the Heart and Spleen leading to sores in the upper oral cavity, resembling a plum. There are no external chills or fever, but internal mental restlessness occurs frequently; this belongs to extreme heat. Needle puncture is strictly prohibited. It is suitable to take Huanglian Jiedu Tang with Jiegeng, and constantly dissolve Zixue San in the mouth. Ling Ding is caused by fire-toxin in the Heart and Spleen, producing purple spots on the tongue shaped like beans, which are hard, accompanied by chills, fever, and severe pain radiating to the heart. In the early stage, it is suitable to hold Chansu Wan under the tongue, swallowing it as it dissolves, or taking three more pills to resolve internal toxins; for severe cases, puncture is required, followed by taking Huanglian Jiedu Tang, applying Zixue San externally, and slowly swallowing it until recovery.
Zixue San Formula: Xijiao (Rhinocerotis Cornu, sliced) 1 liang, Lingyangjiao (Capricornis Subviridis Cornu, sliced) 1 liang, Shigao (Gypsum Fibrosum) 1 liang, Hanshuishi (Hanshui Shi) 1 liang, Shengma (Cimicifugae Rhizoma) 1 liang, Yuanshen (Scrophulariae Radix) 2 liang, Gancao (Glycyrrhizae Radix et Rhizoma, raw) 8 qian, Chenxiang (Aquilariae Lignum Resinatum, ground) 5 qian, Muxiang (Aucklandiae Radix, ground) 5 qian.
Decoction method: Use five bowls of water, decoct until one bowl of liquid remains. Filter out the dregs using silk cloth. Boil the liquid again, add Tijing Puxiao (Natrii Sulfas purificatus) 3 liang 6 qian, and simmer slowly over low heat. When the water is nearly evaporated and the liquid is about to congeal, pour it into a bowl. Add Zhusha (Cinnabaris) and Bingpian (Borneolum Syntheticum), each 3 qian, and 100 sheets of Jinbo (Folium Aurii), previously ground fine and mixed evenly. Place the medicine bowl in a basin of cold water and wait until it cools and solidifies like snow. Adults should take 1 qian per dose, children 2 fen, and ten-year-old children 5 fen. Swallow slowly for effect. Alternatively, decoct Danzhuye (Phyllostachys Viridis Folium) and Dengxincao (Junci Medullae) in water to dissolve and take the powder. Blowing the powder into the throat is also effective for sore throat and swelling.
[Formula Song] Zixue San effectively treats accumulated heat; Chenxiang, Muxiang, Xijiao, Lingyangjiao, Yuanshen, Gancao; add Hanshuishi, Shengma, Shigao, Puxiao; grind Zhusha, Jinbo, Bingpian and mix inside.
