鼻疔生在鼻孔中，鼻窍肿引脑门疼，甚则唇腮俱浮肿，肺经火毒蟾离宫。
【注】此证生于鼻孔内，鼻窍肿塞，胀痛引脑门，甚则唇腮俱作浮肿，由肺经火毒，凝
结而成。宜蟾酥丸汗之，再用蟾酥丸研细末，吹入鼻窍。若肿硬外发，用离宫锭涂之。此
证初起之时，须当速治，迟则毒瓦斯内攻，以致神昏、呕哕、鼻肿如瓶者逆。

鼻渊浊涕流鼻中，久淋血水秽而腥，胆热移脑风寒火，控脑砂因蚀脑虫。
【注】此证内因胆经之热，移于脑髓，外因风寒凝郁，火邪而成。鼻窍中时流黄色浊
涕，宜奇授藿香丸服之。若久而不愈，鼻中淋沥腥秽血水，头眩虚晕而痛者，必系虫蚀
脑也，即名控脑砂，宜天罗散服之。但此证久则必虚。当以补中益气汤兼服之即效。
奇授藿香丸
藿香连枝叶（八两）
研细末，雄猪胆汁和丸，如梧桐子大。每
服五钱，食后苍耳子汤下，或黄酒送下。
【方歌】奇授藿香鼻渊流，浊涕淋漓久不休，猪胆汁合藿香末，苍耳汤下患可瘳。
天罗散
丝瓜藤（近根处者，烧存性）
为末，每用三钱，食后黄酒送下。
【方歌】天罗虫蚀脑髓中，头痛鼻流血水腥，丝瓜根烧研细末，黄酒调服惯杀虫。
补中益气汤（见溃疡门）
鼻疔生长在鼻孔内部，鼻孔肿胀堵塞并牵引头部前额疼痛，严重时嘴唇和腮部都会出现浮肿，这是肺经火毒蕴结于蟾离宫所致。
【注】此证候生于鼻孔内，鼻孔肿胀闭塞，胀痛放射至脑门（前额），严重时嘴唇和腮部均发生浮肿，由肺经火毒凝结而成。适宜使用蟾酥丸使其发汗，再用蟾酥丸研成细末，吹入鼻孔。如果肿胀坚硬并向外扩散，用离宫锭涂抹患处。此证在初起之时，必须迅速治疗，若延误则毒气内攻，导致神志昏迷、呕吐呃逆、鼻子肿得像瓶子一样的情况属于逆证（预后不良）。
鼻渊表现为浑浊的鼻涕流出鼻孔，日久淋漓不止，流出腥臭的血水，病机为胆热移入脑髓，兼感风寒与火邪，因虫蚀脑髓而得名控脑砂。
【注】此证内因为胆经之热，移行至脑髓，外因为风寒凝滞郁结，化火邪而成。鼻孔中时常流出黄色浊涕，适宜服用奇授藿香丸。如果日久不愈，鼻中淋漓流出腥臭的血水，伴有头晕目眩、虚性眩晕且头痛者，必定是虫蚀脑髓所致，即名为控脑砂，适宜服用天罗散。但是此证日久必然导致正气虚弱。应当配合服用补中益气汤即可见效。
奇授藿香丸
藿香连枝叶（八两）
研成细末，用雄猪胆汁调和制成丸药，大小如梧桐子。每次服用五钱，饭后用苍耳子汤送服，或用黄酒送服。
【方歌】奇授藿香治鼻渊流，浊涕淋漓久不停，猪胆汁合藿香末，苍耳汤下患可愈。
天罗散
丝瓜藤（靠近根部者，烧至存性）
研成细末，每次用三钱，饭后用黄酒送服。
【方歌】天罗杀虫蚀脑髓中，头痛鼻流血水腥，丝瓜根烧研细末，黄酒调服惯杀虫。
补中益气汤（见溃疡门）
Bi Ding (Nasal Carbuncle) arises within the nasal cavity; swelling of the nasal orifice radiates pain to the forehead. In severe cases, both lips and cheeks become swollen. This is caused by Fire-Toxin in the Lung Meridian condensing in the Chan Li Palace.
[Note] This condition arises inside the nostrils, with swelling and blockage of the nasal orifice, distending pain radiating to the forehead (naomen). In severe cases, the lips and cheeks all swell. It is formed by the condensation of Fire-Toxin in the Lung Meridian. It is appropriate to use Chansu Wan (Toad Venom Pill) to induce sweating, and then grind Chansu Wan into fine powder and insufflate it into the nasal orifice. If the swelling becomes hard and spreads outward, apply Ligong Ding (Ligong Paste) to the affected area. At the initial stage of this condition, rapid treatment is required. Delay allows Toxin Qi to attack inward, leading to coma, vomiting, and hiccups, or a nose swollen like a bottle; these are adverse signs (poor prognosis).
Bi Yuan (Nasal Sinusitis) presents with turbid discharge flowing from the nose. Prolonged dripping results in foul-smelling blood and water. The pathogenesis involves Gallbladder Heat transferring to the brain marrow, combined with external Wind-Cold stagnation and Fire-Evil. It is named Kongnao Sha (Brain-Controlled Sand) due to worm erosion of the brain.
[Note] Internally, the cause is Heat in the Gallbladder Meridian transferring to the brain marrow. Externally, it is caused by Wind-Cold stagnation and郁 (stagnation), transforming into Fire-Evil. Yellow turbid discharge flows frequently from the nasal orifice; Qishou Huoxiang Wan (Qishou Agastache Pill) is appropriate for oral administration. If prolonged without healing, with dripping of foul-smelling bloody water from the nose, accompanied by dizziness, vertigo, and headache, it must be due to worm erosion of the brain marrow. This is named Kongnao Sha, and Tianluo San (Tianluo Powder) is appropriate for oral administration. However, this condition leads to deficiency over time. It should be taken concurrently with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) for efficacy.
Qishou Huoxiang Wan
Huoxiang Lian Zhiye (Agastache with stems and leaves) (8 liang)
Grind into fine powder, mix with bile from a male pig to form pills, sized like wutong seeds. Take 5 qian per dose, after meals, washed down with Xiancao Zi Tang (Xanthium Seed Decoction) or delivered with yellow wine.
[Formula Song] Qishou Huoxiang treats Bi Yuan flow, turbid discharge drips long without ceasing, Pig bile mixed with Huoxiang powder, Xiancao Tang down the illness can heal.
Tianluo San
Sigua Teng (Silk Gourd Vine) (from near the root, burned to preserve its nature/charred)
Grind into powder. Use 3 qian per dose, after meals, delivered with yellow wine.
[Formula Song] Tianluo kills worms eroding the brain marrow, Headache, nose bleeds, bloody water foul-smelling, Silk gourd root burned ground fine powder, Yellow wine mixed delivery habitually kills worms.
