The physician is the one who holds the life and death of human beings; pulse diagnosis is the greatest undertaking in medicine. If a physician does not understand the pulse, it is like groping for a path in the dark, inevitably leading to stumbling and falling.
For internal medicine (Da Fang Mai), gynecology, pediatrics, and wind disease departments, one must first diagnose the pulse before prescribing medication according to symptoms. Only in the department of sores and ulcers do many practitioners fail to examine the pulse, focusing exclusively on external treatments; or when encountering difficult cases, they summon internal medicine specialists for consultation, thereby accepting the reputation of being shallow and ignorant. Alas! How limited their perspective is.
Investigating the origin, the emergence of sores and swellings is all due to the disharmony of Yin and Yang, and the stagnation of Qi and Blood. Without pulse diagnosis, how can one know the strength or weakness of Yin and Yang, and the gathering or dispersal of Qi and Blood? From this, it is clear that in treating sores and swellings, the method of pulse diagnosis cannot be omitted. Reviewing ancient and modern times, there are many medical texts on treating sores and swellings, yet the methods of pulse diagnosis within them are often brief and undetailed, appearing very negligent compared to other specialties. Although I am untalented, I have selected discussions on diagnosing sores and swellings from the prescriptions and theories of famous physicians past and present, including *Huangdi Suwen* (Yellow Emperor's Inner Canon: Basic Questions), *Nanjing* (Classic of Difficult Issues), *Ling Shu* (Spiritual Pivot), *Jia Yi Jing* (A-Behavior Classic), as well as those by Wang Shuhe, Zhang Zhongjing, Bian Que, Hua Tuo, *Qianjin Fang* (Essential Prescriptions Worth a Thousand Gold), *Waitai Miyao* (Secret Essentials of the Outer Terrace), *Taiping Shenghui Fang* (Holy Grace Formulas of the Taiping Era), and *Tai Ping Hui Min He Ji Ju Fang* (Formulas of the Taiping People's Welfare Bureau). I have compiled and arranged these excerpts into chapters. First, I record the standard methods of pulse diagnosis; second, I discuss the principles of correlating blood, Qi, complexion, and pulse; finally, I clarify the names and appearances of pulses, the syndromes they govern, and the methods for determining favorable and unfavorable outcomes in sores and swellings. It is hoped that practitioners in the field of sores and swellings, upon reading this, will find it clearly understandable. When facing diseases at dawn, having reached this level of diagnosis, distinguishing between favorable and unfavorable outcomes, and deciding success or failure, will be as easy as distinguishing black from white!
The way of Heaven and Earth lies in Yin and Yang; within humans, Yin and Yang manifest as Blood and Qi. Generally speaking, Blood is Ying (Nutritive) Qi, and Qi is Wei (Defensive) Qi; Ying nourishes internally, while Wei protects externally. Therefore, Ying circulates within the vessels, and Wei circulates outside the vessels; the vessels are the pioneers of Blood and Qi. Without vessels, how can Blood nourish internally? Without vessels, how can Wei protect externally? These two rely on each other to circulate, connecting internally with the Five Zang and Six Fu organs and the Twelve Meridians; externally moistening the Nine Orifices, four limbs, joints, and hairs. They circulate day and night like a circle without beginning or end, establishing their proper rhythm. They converge at the Cun Kou (Radial Artery Pulse Point) and manifest changes in the pulse. Thus it is said: Blood and Qi are the spirit of humanity; the pulse is the spirit of Blood and Qi. Therefore, the beginning of treatment takes the Five Determinations (Wu Jue) as its guideline. The so-called Five Determinations refer to the Five Zang organs.
Complexion and pulse should correspond with each other. Thus it is said: One who can combine the analysis of complexion and pulse can achieve perfection in ten thousand cases. If one is not proficient in these aspects, even if intelligent, wise, eloquent, and knowledgeable, one is not worthy of trust.
