In the context of cold damage (Shanghan), when pathogenic heat and toxic qi are vigorous, this type of sore is prone to occur. The sores may appear white or red on the skin surface, forming pus at the apex. If the pus head is white, the toxicity is considered mild; if the root appears purplish-black and lies hidden within the muscle layer, the toxicity is severe. In extreme cases, sores may manifest in the five zang organs and seven orifices, resembling peas in shape, hence the name "pea sore" (Wandou Chuang).
