The manifestation of Onychia (Jia Ju) is characterized by thickened skin around the wound, with the nail plate becoming rough, peeling, and lifting. The wound may also be itchy and painful, causing the patient to constantly desire to scratch it, accompanied by the discharge of pus. Its initial onset is caused by Wind pathogen invading and obstructing the flow of Blood and Qi, and parasites may also exist within the wound.
Formula for treating Onychia with thickened skin, swelling, and pain: Xiamu San (Toad Powder).
Bufo Skin Ash (Ban Liang / Half Tael), Apricot Kernel (Qi Mei / Seven seeds, stir-fried until black and ground into paste), Coptidis Rhizoma (Half Fen / Half Qian, powdered), Realgar (Ban Qian / Half Qian, finely ground), Alum Ash (Ban Qian / Half Qian), Calomelas (Half Fen / Half Qian), Deer Antler (Seven Cun / Seven inches, calcined until cooked and finely ground), Moschus (Ban Qian / Half Qian, finely ground), Bungarus Caeruleus Gallbladder (Ban Qian / Half Qian).
Mix all the above ingredients and grind them finely. Blend with lard from the twelfth lunar month to form a paste. First, wash the wound with a decoction prepared from Glycyrrhizae Radix et Rhizoma (Licorice), Cnidii Fructus (Shechuangzi), and Cortex Sophorae Flavescentis (Huai Baipi). Dry the area thoroughly, apply the medication, wrap with oiled paper, and then cover with cotton cloth. After three days, the excess granulation tissue and loose nail fragments should fall off naturally. Change the dressing every three days.
Formula for treating Onychia and Osteomyelitis:
Alum (Liang / One Tael, calcined until liquid evaporates), Moschus (Ban Liang / Half Tael, finely ground), Aloe (Ban Liang / Half Tael), Bungarus Caeruleus Gallbladder (Size of a soybean).
Grind all ingredients together into a powder. For each application, first wash the wound with warm rice water, dry it, and apply the powder. In severe cases, recovery occurs within three to four applications.
Formula for treating bluish-black spots on fingers leading to Onychia:
Aloe (Ban Liang / Half Tael), Moschus (Ban Liang / Half Tael), Green Vitriol Ash (Liang Er / Two Taels, calcined to ash).
Grind all ingredients together into a powder. Place the powder in a silk bag. Insert the affected finger into the bag and tie it securely with thread to prevent movement. Continue until healed.
Formula for treating Onychia:
Bola Snake Slough (Place in a clean porcelain vessel, burn evenly over candle flame until charred, use Liang Yi / One Tael), Chouhuang (One Tael), Green Vitriol (Yi Fen / One Qian, calcined).
Grind all ingredients finely into a powder. Store in a copper box. First, take two Sheng (approx. 2 liters) of hot urine, place it in a copper basin, add twenty or thirty Apricot Kernels, chew them (or process them) until soft, and remove the residue. *Note: The text implies caution as the medicine might "bite" and damage the tendons of the wound.* Strictly avoid sexual intercourse and strenuous physical exertion. Avoid wheat products, garlic, and alcohol; other foods are permissible.
For men and women suffering from Onychia on the fingers of hands and feet due to Wind-Blood toxicity attacking the area, which has persisted for a long time, presenting with pain where the flesh envelops the nail and irregular bleeding:
Alum (Ban Liang / Half Tael), Melanterite (Ban Liang / Half Tael), Moschus (Yi Fen / One Qian), Cinnabar (Yi Fen / One Qian), Dragon's Blood (Yi Fen / One Qian).
Take the Alum and Melanterite and calcine them together in an iron vessel. Then add Moschus, Dragon's Blood, and Cinnabar, and grind everything finely. Sprinkle a small amount of this powder directly onto the wound. Bind tightly with cloth. Change the dressing two to three times daily.
Formula for treating Onychia:
Portulaca Oleracea (Ban Liang / Half Tael, dried), Costi Rhizome (Yi Fen / One Qian), Prepared Salt (Yi Fen / One Qian), Cinnabar (Yi Fen / One Qian, finely ground).
Pulverize all ingredients through a fine sieve and mix thoroughly. Apply the powder three to four times daily.
Miraculous Formula for treating Onychia wounds:
Arsenic Sulfide (Half Fen / Half Qian), Bola Snake Slough (Yi Fen / One Qian, calcined to ash).
Grind together into a powder. For each application, first soak and wash the wound with warm starch water until soft. Use a sharp knife to cut away the dry nail corners. Apply the powder and cover with soft cloth. Replace the dressing when it becomes damp, approximately half a day later. Pain will be relieved within one day.
Formula for treating Onychia where red granulation tissue grows along the nail edge and envelops the nail:
Calcine Alum to ash, grind finely, and apply to the affected area.
Formula for treating swollen and ulcerated Onychia with red tissue protruding from the side of the toenail, which heals and recurs intermittently:
Phellodendri Cortex (Liang Er / Two Taels, sliced). *Note: Original text has a missing character after 'Huang', likely Phellodendri Cortex based on context.*
Soak in wine overnight. Add five He (approx. 500ml) of lard and simmer over low heat until reduced to three He. Strain out the dregs. Apply to the affected area two to three times daily. The swelling and ulceration will subside.
Immediate Effective Formula for treating ulcerated Onychia:
Boil Amaranthus tricolor (Ren Xian) in concentrated juice, add a little salt, and soak the foot for one to two days until healed. Dried plant material and roots can also be used.
Formula for treating Onychia with polypoid growth at the nail margin causing unbearable pain:
Calcine Melanterite until smoke ceases, grind into a fine powder, and apply to the wound.
Formula for treating swelling and pain adjacent to the nail following trimming of fingernails or toenails:
Dry Sour Plum (Gan Yan Mei), including peel and pit, pound into a paste, and apply as a compress to the swollen area. After one day, mild itching may occur. Remove the plum paste after another half day, and healing will ensue.
