Intestinal abscess (Chang Yong) arises from improper regulation of cold and warmth, and irregularity in joy and anger, causing pathogenic factors to interfere with Ying and Wei Qi (Defensive and Nutritive Qi) and remain within the intestines. Upon encountering heat, Qi and blood stagnate and accumulate, forming a mass. Symptoms include frequent urination resembling strangury (Lin syndrome). A slow and tight pulse indicates that pus has not yet formed; severe cases present with abdominal distension, audible fluid sounds when turning over, or sores around the umbilicus that rupture and discharge pus, or spontaneous passage of pus and blood. Urgent treatment is required. It is also stated that passing pus and blood in stool, resembling dysentery with red and white discharge, constitutes intestinal abscess. Patients suddenly afflicted with intestinal abscess are often treated incorrectly. If the Cun-kou pulse is slippery and rapid, slipperiness indicates excess, while rapidity indicates heat; slipperiness reflects the state of Ying Qi, and rapidity reflects the state of Wei Qi. If heat rises and interferes with Ying and Wei due to heat, blood becomes exhausted alone, presenting with fullness and stuffiness in the lower abdomen, difficulty in urination, sweating, or recurrent aversion to cold, indicating that pus has already formed. If the pulse is slow and tight, it signifies coagulation of stasis; if blood passes downward, the disease resolves; mature pus draws out white discharge. Furthermore, floating and rapid pulses typically accompany fever, but if there is instead a sensation of shivering aversion to cold and fixed pain, pus accumulation should be suspected. A pulse that is both slippery and choppy indicates intestinal abscess with expectoration of pus.
