87. Zhuwei Gao (Pig Tail Ointment). Indicated for early-stage chickenpox and smallpox that has not yet fully erupted, as well as cases where the eruption is complete but internal and external heat-toxins are flooding the system. It is used when blood stagnates like bean-sized nodules on the skin surface without forming scabs in either scenario. Furthermore, when rashes are latent with heat-toxin attempting to emerge but failing to do so, severe heat may disturb the Heart, causing mental restlessness, mania, delirious speech, or hallucinations of ghosts and spirits; these are all signs of Heat in the Heart. Since the Heart governs the Blood, Blood Heat leads to stasis, and stasis causes discoloration. Un-erupted lesions become "Ghost Sores," turning black upon eruption. Erupted lesions are called "Black Crusts." Generally, pockmarks develop from red macules into bloody points; by the fifth to seventh day, they should form pustular bases. If no pustular base forms by the sixth or seventh day, it indicates that toxins are pervasive, risking re-entry into the interior. Ancients used Longnao Xiang (Borneol) to cool Heart Blood and promote the circulation of Ying and Wei Qi. Pig Heart Blood was used because "Heart returns to Heart"; Pig Tail Blood was chosen for its dynamic nature, symbolizing dispersion outward. In cases with preceding mania and unconsciousness, administering with warm wine aims to disperse and circulate Ying and Wei Qi. These methods treat excessive Heat-Toxin. Later practitioners found Zhuwei Gao, also known as Longnao Gao (Borneol Ointment), effective. However, it was occasionally misused for deficiency-type collapsed eruptions, causing much harm, which is astonishing. Some prescribed one qian (approx. 3-5g) instead of the required yi zi (approx. 0.3-0.5g), resulting in insufficient potency or perceived harm. For instance, if a dose of one yi zi is indicated, half may be given initially, similar to treating severe thirst in Cold Damage where a patient capable of drinking one dou (approx. 10L) might start with five sheng (approx. 5L). Moreover, if Heart Channel Heat coexists with other deficiency-cold patterns, it should not be taken lightly. For patients with vomiting/diarrhea, fever, irritability, thirst, and dark urine, treatment should not assume pure cold. For irritable patients with spontaneous loose stools and clear urine, treatment should not assume pure heat. Physicians must balance these conditions, alleviate suffering, harmonize Yin and Yang, avoid extremes, and ensure safety.
Preparation: Take approximately half a yi zi of Longnao (Borneol), grind finely, mix with fresh Pig Heart Blood to form pills. Dissolve in Zicao Tang (Arnebia Decoction). For mania and suffocation with unconsciousness, take with warm wine. Qian Yi (Qianshi) used Pig Tail Blood in this formula; the rationale is consistent. It is also named Longnao Gao.
88. Kongxin San (Controlling Heart Powder). For pediatric exanthems; one dose promotes eruption.
Quanxie (Scorpio, 24 pieces, stir-fried), Xionghuang (Realgar), Mahuang (Ephedrae Herba, de-stemmed, each 1 fen).
Grind into fine powder. Take 1 qian per dose, mixed with Huixu Jianjiu (Coriander Decoction Wine). Scorpions treat various wind-induced urticaria, hemiplegia from stroke, facial paralysis, dysarthria, and spasms; they penetrate meridians and sinews. Realgar treats malignant sores, carbuncles, hemorrhoids, penetrates joints, treats scabies and ulcers, and aids in expelling toxins from exanthems that fail to erupt. Mahuang releases the muscle layer, expels pathogenic qi, and eliminates red/black toxic spots. These three herbs treat exanthems that should erupt but do not. If eruption occurs naturally and smoothly, administration is unnecessary.
89. Duwei Mahuang Tang (Single-Ingredient Ephedra Decoction). Treats inverted crusts (retracted exanthems).
Take 30 cun of Mahuang (Ephedrae Herba), de-stemmed, stir-fried with honey until fragrant and purple. Decoct in one sheng of water until reduced to six-tenths. Administer. Historically, an official in Xianyuan County treated a critical case of inverted meat-sores with one dose, resulting in immediate eruption, proving miraculous. It is treatable before gastric ulceration and hematochezia occur.
90. Nanjin San (Southern Gold Powder). For pediatric exanthems that erupt then retract, with severe dangerous heat unresponsive to other drugs; infallible.

Symptoms: chills/jaw clenching, yellow/swollen/purple ears. Purge urgently with Baixiang Wan. If worsening chills, cold sweat, and ear/buttock heat persist after purging, it is fatal. Warm body/thirst indicates treatability. Another view: Black collapse without chills means unstable disease. Use Baixiang Wan assisted by Niuli Gao (large dose each). Dong Ji used this for severe pediatric exanthems (subcutaneous non-eruption, slow growth, purple-black dry collapse).
