To treat swelling and pain in the cheek and jaw, or ulceration into sores.
Take *Hibisci Mutabilis Folium* (Furong Ye) [Cotton Rose Leaf] (quantity not restricted).
Mash it into a paste and apply externally to the affected area, secure with cloth, and change once daily.
Take *Realgar* (Xionghuang) above, grind into powder, mix with vinegar for topical application, and simultaneously take mixed with wine orally. This is applicable for snake bites as well as injuries from bees, scorpions, centipedes, venomous insects, and mad dogs.
To treat sores arising around the anus that persist and do not heal for a long time.
Take *Gallus gallus domesticus Brisson* (Ji Nei Jin) [Chicken Gizzard Lining], specifically the inner yellow membrane removed from the gizzard, quantity not restricted. Burn it to ash while retaining its properties, grind into an extremely fine powder, and apply dry directly to the lesion each time; the effect is miraculous.
In the Renwu year of the Zhiyuan era, a person named Wang Bolu, aged fifty-seven, suffered from severe swelling of the upper arm, extending upwards to the shoulder and downwards to the fingers. The skin color changed, and the skin was cold. The six pulses were deep, fine, and faint; both pulse and symptoms indicated a cold syndrome. Yan He, a surgeon, examined him and said, "This is a deep-rooted abscess (Fugu Yong). The pus has matured and is flowing, but it is clear, thin, and diffuse. Wait for a few days until the area below the elbow is opened again." Subsequently, the patient developed incessant hiccups. Yan He administered Dingxiang Shidi Tang (Clove and Persimmon Calyx Decoction) for two doses, providing slight relief. A few days later, the hiccups became even more severe, accompanied by spontaneous diarrhea, cold pain in the umbilical and abdominal regions, abdominal fullness, reduced food intake, and episodes of sudden dizziness and confusion. Yan He applied moxibustion for fourteen moxa cones at the point where blackness ended under the left breast. He then prescribed a formula containing *Zingiberis Rhizoma Recens* (Ganjiang), *Aconiti Lateralis Radix Praeparata* (Fuzi), *Lithospermum Erythrorhizon* (Muxiang - *Note: Context implies Aucklandia/Changpu or similar aromatic, but text says Muxiang which is Litsea/Aucklandia root, often confused in ancient texts or referring to specific variety. Standard Muxiang is Aucklandiae Radix*), *Aquilariae Lignum Resinatum* (Chenxiang), *Foeniculum Vulgare* (Huixiang), and *Notopterygii Rhizoma et Radix* (Qianghuo). These were cut into pieces totaling one tael and half, intended for administration. Someone remarked, "All pains, itching, and sores belong to Heart Fire." Moreover, it is the height of summer heat; is it appropriate to use Ganjiang and Fuzi? I replied, "It is logically correct." The *Neijing* states: "Fine pulse, cold skin, diarrhea before and after, inability to eat—this is called the 'Five Deficiencies'. Hiccups indicate extreme deficiency of Stomach Qi." Although it is a general principle that all swellings and sores belong to Heart Fire, this case presents contradictory internal and external manifestations. One must abandon the seasonal convention and follow the syndrome. Without urgent treatment using a large dose of pungent and hot agents, recovery is impossible. Thus, the medicine was administered, all symptoms disappeared, food intake resumed, the sore became warm, and the pus color normalized. Yan He then administered Wuxiang Tang (Five Fragrances Decoction) for several doses, and the patient recovered fully after more than a month. Alas! Adhering to conventions is the view of the masses; knowing how to adapt is crucial for major matters. Those who know the constant but fail to understand adaptation often suffer defeat, especially in medicine. Can adhering to convention and knowing adaptation be compared on the same day?
