Guo Jizhong stated: When excessive sweating occurs postpartum and transforms into Jing disease (spasm), it is due to blood deficiency after childbirth, causing the Couli (skin pores) to be loose, leading to profuse sweating. Upon encountering wind pathogen invasion and binding, Jing disease develops. Jing disease refers to lockjaw where the mouth cannot open, stiffness and straightness of the back, resembling an epileptic seizure, with head shaking, sounds like a horse neighing, opisthotonos (body bending backward), recurring in moments, with breath appearing as if cut off. One should rapidly administer Xiao Xuming Tang (Minor Continue Life Decoction). If treatment is delayed slightly, resulting in sweat pouring like rain and hands groping in the air, it becomes incurable.
Xue stated: Postpartum Jing disease arises from excessive blood loss, extreme depletion of Yuan Qi (primordial qi), or interaction with external pathogens, leading to tightness of the jaw, spasticity and rigidity of the four limbs, or opisthotonos of the waist and back, and limb convulsions. If there is sweating but no aversion to cold, it is called Rou Jing (Soft Spasm); if there is no sweating and aversion to cold, it is called Gang Jing (Hard Spasm). This is caused by excessive blood loss, leaving the sinews without nourishment. Therefore, patients with Taiyang disease who have sweated or purged excessively, or those with ulcers suffering massive discharge of pus and blood, are prone to this condition, which is a critical failure syndrome. One must urgently use Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction) to tonify Qi and Blood. If ineffective, add Fuzi (Aconite Lateral Root) urgently, and safety may be ensured. If one attacks the wind pathogen, death is inevitable.
Daquan states: Postpartum wind-stroke, with spasms and urgency of the sinews and four limbs, indicates insufficiency of Qi and Blood, and deficiency of all Zang-Fu organs. Rising early for labor, wind-cold pathogen lodges in the skin and meridians, causing numbness and insensitivity, emaciation, fatigue, and shortness of breath. When wind pathogen enters the sinews and binds with cold, it causes spasms and urgency. Xue stated: The Liver belongs to Wood and governs the sinews. If the aforementioned symptoms are due to Wind-Heat and Blood Dryness in the Liver Meridian, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder). If ineffective, employ Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) to tonify Kidney Water. The Classic states: "When wind lodges and disrupts Qi, Essence is lost; evil harms the Liver."
The Classic states: The meridian of the Large Intestine spreads under the tongue. It also states: "When the meridian of the Spleen moves, disease manifests as a stiff tongue base and inability to speak." It further states: "The divergent meridian of the Kidney enters upward into the Heart and ties to the root of the tongue; when deficient, one cannot speak." I surmise that the aforementioned symptoms, due to postpartum weakness, often result from retained stasis blocking the Heart orifices, preventing clear consciousness, thus causing muteness. Generally, for Heart and Kidney Qi deficiency, use Shizhen San (Seven Treasures Powder); for Kidney deficiency with Wind-Heat, use Dihuang Yinzi (Rehmannia Decoction); for Large Intestine Wind-Heat, use Jiawei Xiaoyao San plus Fangfeng (Saposhnikovia Root) and Baizhi (Angelica Dahurica); for Spleen Meridian Wind-Heat, use Qinjiao Shengma Tang (Gentian and Cimicifuga Decoction); for Liver Meridian Wind-Heat, use Chaihu Qinggan San (Bupleurum Clearing Liver Powder) plus Fangfeng and Baizhi; for Spleen Qi stagnation, use Jiawei Guipi Tang (Modified Restore the Spleen Decoction) plus Shengma; for excessive Liver Wood, use Xiaochaihu Tang (Minor Bupleurum Decoction) plus Gouteng (Uncaria Rhynchophylla); for Spleen oppressed by Liver Wood, use Liujunzi Tang (Six Gentlemen Decoction) plus Shengma, Baizhi, and Gouteng; for Liver and Spleen Blood deficiency, use Foshou San (Finger Citron Powder); for Spleen Qi deficiency, use Sijunzi Tang (Four Gentlemen Decoction); for combined Qi and Blood deficiency, use Bazhen Tang (Eight Treasure Decoction). If ineffective, use Dan Shen Tang (Single Ginseng Decoction); if still ineffective, urgently add Fuzi to tonify Qi and generate Blood. Using only blood-tonifying herbs would be an error. Daquan states: Postpartum inverted speech, or manic talking and delirium, seeing ghosts and spirits, has various origins. First, due to postpartum Heart deficiency, retained stasis accumulates and harasses the Heart, causing manic solitary speech (seek treatment in the 'Seeing Ghosts Suddenly' section). Second, postpartum organ deficiency leads to Heart Spirit fright and palpitations, unstable will, confused speech, unawareness, and unsettled spirit (seek treatment in the 'Palpitations' section). Third, pre-existing wind-toxin, combined with postpartum Heart deficiency and weak Qi, causes stiff waist and back, singing, crying, anger, laughter, and chaotic speech; treat as Wind-Jing (seek treatment in the 'Heart Fright Wind-Stroke' section). Fourth, postpartum Heart deficiency with wind-stroke causes vague Heart Spirit and confused speech (seek treatment in the 'Wind-Stroke Vagueness' section). Fifth, postpartum conditions often involve stasis confusing the Heart Meridian leading to mania, erratic speech, or fainting (seek treatment in the 'Blood Syncope' category). Sixth, postpartum contraction of wind-cold (none of the above sections mention phlegm), sudden cessation of lochia, aversion to cold and fever resembling malaria, clarity during the day but delirium at night resembling seeing ghosts (treat as Heat Entering the Blood Chamber), suitable for Hupo Dihuang Wan (Amber Rehmannia Pill) and Siwu Tang (Four Substance Decoction) (do not use Taiyang disease treatment methods); decoct equal parts of Shengdi (Rehmannia Root) and Beichaihu (Bupleurum Root); if unresolved, use Xiaochaihu Tang plus Shengdi. Although these symptoms generally have specific methods for pregnancy and postpartum, differing by a hair, postpartum treatment first requires expelling stasis and generating new blood, then carefully distinguishing the disease pattern. Arbitrary naming and modification of formulas should be symptom-specific. Following the formula for treatment ensures stability.
Xue stated: For the aforementioned symptoms, the primary approach should be consolidating Stomach Qi, assisted by herbs addressing the presenting symptoms. Attacking phlegm exclusively would be an error.
