Spontaneous abortion (early miscarriage) often occurs at three months of pregnancy, as well as at five or seven months. Excluding cases caused by falls or physical trauma, if a previous pregnancy ended in miscarriage at three months, the next pregnancy will likely result in miscarriage at the same gestational age. This is because injury occurred at that specific time previously, so the consequence must manifest later; this is due to bodily deficiency. Therefore, one must pre-administer herbs that strengthen the spleen, tonify qi, and nourish blood and essence from the day pregnancy is confirmed. These must be taken daily without interruption to be effective.
If there is insufficiency of the fetal origin leading to fatigue and listlessness, or restless fetus, or slight fever with reduced appetite, use the following prescription: Ren Shen (Ginseng Radix), Dang Gui (Angelicae Sinensis Radix), Huai Shu Di (Prepared Rehmanniae Radix) (each 2 qian), Bai Zhu (Atractylodis Macrocephalae Rhizoma) (2.5 qian), Tiao Qin (Scutellariae Baicalensis Radix) (8 fen), Chen Pi (Citri Reticulatae Pericarpium), Su Ye (Perillae Folium) (each 4 fen), Sha Ren (Amomi Fructus Rotundus) (3 fen). If symptoms of irritability and thirst occur, add Mai Dong (Ophiopogonis Radix) (1 qian) and Zao (Jujubae Fructus) (5 pieces). Decoct in water and take orally. If lower back pain or pain in the spleen/leg area occurs, three doses must be taken within one day and night to stabilize the condition.
For symptoms such as vomiting of phlegm-water, mental restlessness and oppression, heavy head, vertigo, aversion to food odors, restless fetus, lower back and abdominal pain, and vaginal bleeding, use Si Wu Tang (Four Substances Decoction) added with Huang Bo (Phellodendri Cortex), Bai Zhu, Gan Cao (Licorice Root), Huang Qin (Scutellariae Radix), E Jiao (Donkey-hide Gelatin), Ren Shen, Sang Ji Sheng (Taxilli Herba). (If genuine Sang Ji Sheng is unavailable, replace it with Xu Duan [Dipsaci Radix].)
Nausea and obstruction of food intake occurring at three months of pregnancy constitute E Zu (Hyperemesis Gravidarum/Pregnancy Vomiting). Some women suffer from vomiting even at six or seven months; the treatment is the same. However, for obese individuals, the etiology is attributed to Phlegm; for thin individuals, it is attributed to Fire. All these cases are suitable for Er Chen Tang (Two-Aged Powder) added with Bai Zhu and Huang Qin, or alternatively added with Xiang Fu (Cyperi Rhizoma), Sha Ren, Jiang Zhi (Zingiberis Rhizoma Recens Juice), and Zhu Ru (Bambusae Caulis in Taenia). This aligns with the treatment for general vomiting. If caused by Qi stagnation, the pulse will likely be deep and choppy; treatment should address soothing depression, adding Fu Xiong (Chuan Xiong/Achyranthis Bidentatae Radix variant) and Xiang Fu. Caution against excessive use of acrid/warm herbs. Pregnant women with innate constitutional weakness may develop "Zu Bing" (obstruction disease), presenting like intoxication but with normal complexion and smooth pulse, yet feeling heavy limbs, dizzy head and eyes, selective eating or anorexia, and craving sour/salty flavors. In severe cases, alternating chills and fever occur, mental oppression below the heart, vomiting of phlegm-water, and mental confusion with inability to sustain oneself. Ancient texts name this "E Zu". Treatment should first regulate the Spleen and Stomach, clear Fire, and resolve Phlegm. If vomiting is severe, anti-emetics may paradoxically increase vomiting; Zhang Zhongjing stated that stopping medication allows self-recovery after about a month.
If vomiting results from improper diet causing food stagnation, mild digestive promotion is appropriate; do not treat as E Zu. If the Spleen and Stomach are weak, add Ren Shen and Bai Zhu. Treating E Zu requires Da Ban Xia Tang (Large Pinellia Decoction), modified as follows: for dizziness and abundant phlegm, add Xuan Fu Hua (Inulae Flos); for Fire, add Jiang Zhi, Chao Huang Lian (Stir-fried Coptidis Rhizoma), and Zhu Ru.
