Xue Lizhai stated: If the aforementioned syndrome is caused by external pathogens, use Duhuo Jisheng Tang (Notopterygium and Dangshen Decoction for Expelling Wind-Dampness and Nourishing Qi and Blood). If it is due to labor injury damaging Qi and Blood, use Bazhen Tang (Eight Treasure Decoction) with added Du Zhong (Eucommia Bark), Sha Ren (Amomum Fruit), E Jiao (Donkey-Hide Gelatin), and Ai Ye (Mugwort Leaf). If there is Spleen and Kidney deficiency, add Bai Zhu (Atractylodes Rhizome) and Gu Zhi (Bone Fragment) to the previous prescription. If there is Qi and Blood stagnation, use Zisu Yin (Perilla Infusion) with added Zhi Shi (Immature Bitter Orange) and Jie Geng (Platycodon Root). If Liver Fire is agitated, use Xiao Chai Hu Tang (Minor Bupleurum Decoction) with added Bai Zhu, Zhi Ke (Bitter Orange Peel), and Shan Zhi (Gardenia Fruit). If there is Liver and Spleen constraint, use Guipi Tang (Restore the Spleen Decoction) with added Chai Hu (Bupleurum Root) and Zhi Ke.
Shen Zhai commented: The above four points describe syndromes of lower back pain occurring before pregnancy. Lower back pain during pregnancy can be caused by external contraction or internal injury. *Da Quan* attributes it to wind-cold, while Shi Shan attributes it to blood stasis; these are conditions of excess pathogenic factors.
Zheng Zong discusses lower back pain during pregnancy in relation to overexertion and sexual intercourse, thereby grasping the essence of the pathogenesis.
*Da Quan* states: Lower abdominal pain during pregnancy is caused by deficiency of the uterine collaterals, where wind and cold contend with each other; severe pain may also disturb the fetus.
Shen Zhai commented: The above point describes syndromes of lower abdominal pain occurring before pregnancy. The lower abdomen is the area governed by the Foot-Jueyin Liver Meridian. If this meridian suffers from insufficient Yin-Blood or Qi stagnation due to suppressed anger, it can lead to lower abdominal pain. Furthermore, the fetus is anchored by the Kidneys, and the Kidneys and Liver often suffer together. *Da Quan*’s discussion on wind and cold contending only addresses the aspect of external pathogens.
Wang Shuhe said: When a woman’s menstruation flows but is scanty, and the physician palpates the pulse, stating contrary that she is pregnant, which proves true later, what kind of pulse is this? The physician said: At the Cun Kou (radial artery) position, both Yin and Yang pulses are balanced, Ying and Wei (Nutritive and Defensive Qi) are harmonious. Pressing reveals a slippery quality, while light palpation feels floating. The Yangming and Shaoyin meridians follow their respective normal methods. The body feels chills, there is loss of appetite, headache, mental restlessness, vomiting, rapid shallow breathing upon exhalation, and no alarm upon inhalation. Yang is abundant with Qi overflowing, Yin is slippery with Qi flourishing. Slippery indicates excess. The six meridians nurture life, hence menstruation occurs. Yin encounters Yang essence, fluids coagulate and the uterus disperses. Dispersal leads to damage and abortion. If Yang becomes excessive again, twins may result. Now, since Yang is insufficient, it causes "Ji Jing" (intermenstrual bleeding or continued spotting during early pregnancy).
Lou Quanshan said: During pregnancy, if menstruation comes but is not heavy, and diet and spirit remain normal, with the six pulses being harmonious, slippery, and large without pathological signs, this indicates abundant Blood and excess. Once the fetus grows large enough to consume nutrients, menstruation will cease naturally.
Shen Zhai commented: The above two points discuss menstruation before/during pregnancy, distinguishing between excess and deficiency. Bleeding during pregnancy is named "Lou Tai" (Leaking Fetus). Menstruation during pregnancy is named "Ji Jing". Lou Tai involves continuous bleeding, whereas Ji Jing occurs at regular intervals. Wang Shuhe attributes it to insufficient Yang, while Lou Quanshan attributes it to abundant Blood and excess. One must consider the individual’s constitutional strength when judging.
