[Da] The syndrome of diarrhea arises from deficiency-cold in the intestines and stomach, allowing pathogenic factors to take advantage. The Classics state: "Wind injured in spring leads to undigested stool diarrhea (Sun Xie) in summer." This is because wind injures the Liver, causing Liver Wood to flourish and overact on Spleen Earth, which belongs to external causes. If the Seven Emotions (Joy, Anger, Worry, Thought, Sadness, Fear, Fright) are unbalanced, injuring the Qi of the Zang organs, it belongs to internal causes. If raw and cold food injures the Spleen, it belongs to neither-internal-nor-external causes. The general treatment principles are: warm what is cold, cool what is hot, astringe what is slipping, and dry what is damp.
[Xue] For the aforementioned syndromes, if caused by raw and cold food, use Liu Jun Zi Tang (Six Gentlemen Decoction), Mu Xiang (Aucklandiae Radix), and Sha Ren (Amomi Fructus). If injured by pungent-hot food, use Er Chen Tang (Two Aged Decoction), Chao Lian Lian (Stir-fried Coptidis Rhizoma), and Shan Zhi (Gardeniae Fructus). If injured by flour foods, use Liu Jun Zi Tang (Six Gentlemen Decoction) and Shen Qu (Massa Medicata Fermentata). If injured by rice foods, use Liu Jun Zi Tang (Six Gentlemen Decoction) and Gu Ya (Oryzae Fructus Germinatus). If injured by irregular eating habits, use Si Jun Zi Tang (Four Gentlemen Decoction). If injured by overeating, use Liu Jun Zi Tang (Six Gentlemen Decoction). If food stagnation occurs, use Ren Shen Yang Wei Tang (Ginseng Stomach-Nourishing Decoction). For Spleen Qi deficiency, use Liu Jun Zi Tang (Six Gentlemen Decoction) with Sheng Ma (Cimicifugae Rhizoma) and Chai Hu (Bupleuri Radix). For Spleen Qi deficiency-cold, use Liu Jun Zi Tang (Six Gentlemen Decoction) with Mu Xiang (Aucklandiae Radix) and Pao Jiang (Prepared Zingiberis Rhizoma). For Liver Wood encroaching on Spleen, use Liu Jun Zi Tang (Six Gentlemen Decoction) with Chai Hu (Bupleuri Radix) and Shao Yao (Paeoniae Radix Alba). For Liver Fire overacting on Spleen, use Liu Jun Zi Tang (Six Gentlemen Decoction) with Shao Yao (Paeoniae Radix Alba) and Shan Zhi (Gardeniae Fructus). For Middle Qi deficiency with sinking, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). For depression damaging the Spleen, use Ji Sheng Gui Pi Tang (Ji Sheng Heart-Spleen Decoction). For Kidney Qi deficiency, use Wu Wei Zi San (Schisandra Powder). For Spleen Qi deficiency, use Er Shen Wan (Two Spirits Pill). For Spleen-Kidney dual deficiency, use Si Shen Wan (Four Spirits Pill). for Ming Men Fire decline, use Ba Wei Wan (Eight Flavor Pill). For True Yang deficiency and collapse, use Gu Zhen Wan (Secure the True Pill). These should be used concurrently with formulas that resolve stagnation. The wife of Imperial Attendant Shen Dongjiang suffered from abdominal pain and diarrhea due to food retention; she was cured using Liu Jun Zi Tang (Six Gentlemen Decoction) added with Mu Xiang (Aucklandiae Radix) and Pao Jiang (Prepared Zingiberis Rhizoma). It later recurred and transformed into Kidney Diarrhea (Ren Xie), which was stabilized using Si Shen Wan (Four Spirits Pill). The daughter-in-law of Imperial Attendant Xu Nanhu suffered from diarrhea belonging to the Kidney channel; not believing my advice, she exclusively used draining and leeching methods, leading to her demise. A woman over fifty years old had not eaten dinner and suffered from early morning diarrhea for twenty years. Later she developed dysentery; she took Xiang Lian Wan (Cinnamon and Coptis Pill) in the morning and Er Shen Wan (Two Spirits Pill) in the afternoon, two doses each, stopping the dysentery. After several more doses of Er Shen Wan (Two Spirits Pill), she could eat dinner again, and within a month her physique returned to normal. The mother of Shi Xuannian from Wujiang suffered from blood diseases for nearly twenty years, accompanied by chronic diarrhea, poor appetite, sallow complexion, and abdominal fullness, worsening in summer. She took hundreds of doses of blood-treating medicines without any relief. I considered this Spleen-Kidney deficiency, treating her with Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) delivered with Er Shen Wan (Two Spirits Pill), followed by Shi Quan Da Bu Tang (Ten Complete Great Tonifying Decoction) decocted to deliver the previous pill. Her appetite improved, stools became formed, and the disease receded significantly. Adhering strictly to the dogmas that "abdominal fullness contraindicates Ren Shen (Ginseng) and Bai Zhu (Atractylodis Macrocephalae Rhizoma)," "phlegm obstruction contraindicates Shu Di Huang (Rehmanniae Radix Preparata)," and "diarrhea contraindicates Dang Gui (Angelicae Sinensis Radix)" would all lead to errors.
*Supplement* treats diarrhea: for Wind patterns, use Wei Feng Tang (Wind-Dispersing Decoction) or Bu Huan Jin Zheng Qi San (Unchanging Golden Rectifying Qi Powder). For Cold patterns, use Li Zhong Tang (Regulate the Middle Decoction), Si Zhu San (Four Pillars Powder), or Da Yi Han Wan (Greatly Warm Cold Pill). For Summerheat, use Wu Ling San (Five-Ingredient Powder with Poria), Xiang Ru San (Elsholtzia Powder), or Liu He Tang (Six Harmonies Decoction). For Dampness, use Huo Xiang Zheng Qi San (Agastache Rectifying Qi Powder) or Wu Ji Wan (Wuji Pill). This is a general discussion; for comprehensive treatments, various formulas can be consulted in texts, treating according to the pattern. However, for chronic diarrhea in men and women that does not cease, there is no formula more effective than Gu Chang Wan (Intestine-Securing Pill).
Xiang Po Wan (Aucklandia and Magnolia Pill) treats intestinal-stomach deficiency-cold diarrhea, profuse watery stools, Spleen deficiency with Qi obstruction, and inability to eat.
Da Nong Bo (Magnoliae Officinalis Cortex, 5 liang), Bei Xin (Asari Radix et Rhizoma), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Chen Pi (Citri Reticulatae Pericarpium, 3 liang each), He Zi (Terminaliae Chebulae Fructus), Chi Shi Zhi (Halloysit Rubrum, 1.5 liang each).
Grind into fine powder, make pills with wheat paste the size of paulownia seeds. Take 50 pills with rice water on an empty stomach. Regular use warms the Spleen and Stomach.
Gui Xiang Wan (Cinnamon and Aucklandia Pill) treats deficiency of the Zang-Fu organs combined with Wind-Damp-Cold binding, uncontrollable cold slippery diarrhea, critical conditions in the elderly and debilitated; highly effective.
Fu Zi (Aconiti Lateralis Radix Praeparata), Rou Dou Kou (Myristicae Semen, both prepared), Bai Fu Ling (Poria, 1 liang each), Gui Xin (Cinnamomi Ramulus), Mu Xiang (Aucklandiae Radix, prepared), Bai Jiang (Zingiberis Rhizoma Recens, prepared, 0.5 liang each), Ding Xiang (Syzygii Flos, 2.5 qian).
Grind into fine powder, make pills with rice paste the size of paulownia seeds. Take 50 pills with rice water on an empty stomach.
Dou Kou Fen Qi Yin (Cardamom Distributing Qi Drink) treats Zang-Fu deficiency-cold, profuse diarrhea, extreme emaciation, and postpartum diarrhea in women; severe cases of hole-like diarrhea.
Huo Xiang Ye (Agastaches Herba), Cao Dou Kou Ren (Cardamomi Fructus, prepared), Qing Ju Pi (Citri Reticulatae Pericarpium Viride, 4 liang each), Gan Cao (Glycyrrhizae Radix et Rhizoma), Ding Xiang (Syzygii Flos, 0.5 liang each), Wu Mei (Mume Fructus, 50 pieces), Rou Dou Kou (Myristicae Semen, 10 pieces, prepared).
Cut into pieces, take 4 qian per dose, add 2 cups of water and a pinch of glutinous rice, decoct to 0.7 portion, strain and take warm.
Dou Kou Wan (Cardamom Pill) treats organ-cold diarrhea persisting despite other medications. (Also treats men).
Rou Dou Kou (Myristicae Semen, wrapped in dough and baked until fragrant), quantity as needed, grind fine, mix with aged white rice porridge pounded to appropriate consistency, make pills the size of mung beans. Swallow 100 pills with cooked millet drink on an empty stomach. My family has repeatedly saved lives with this medicine effectively.
Ren Shen Dou Kou San (Ginseng and Cardamom Powder) treats chronic persistent diarrhea ineffective to other medicines; this medicine is frequently effective.
Ren Shen (Ginseng Radix et Rhizoma), Rou Dou Kou (Myristicae Semen), Gan Jiang (Zingiberis Rhizoma), Nong Bo (Magnoliae Officinalis Cortex), Gan Cao (Glycyrrhizae Radix et Rhizoma), Chen Ju Pi (Citri Reticulatae Pericarpium, 1 liang each), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Gui Xin (Cinnamomi Ramulus), He Zi (Terminaliae Chebulae Fructus), Bei Xin (Asari Radix et Rhizoma, 0.5 liang each).

Grind into powder, take 3 qian per dose. Use approximately 4 liang of pig liver that has not been exposed to menstrual blood, remove fascia, slice thinly, layer with medicine powder, place in a pot with one bowl of fermented rice water and a little vinegar, cover and cook the liver thoroughly. Add 1 qian salt, three stalks of finely chopped scallion white, and one palm-sized piece of smashed ginger. Simmer until liquid is almost gone. Take as one dose on an empty stomach, eat cold. Mild diarrhea upon first intake is acceptable, as it expels cold qi; it will stop shortly. Chronic cold dysentery and slippery diarrhea lasting years require only one dose. Drink porridge soup when thirsty. Avoid raw, cold, and greasy foods. If unable to eat cold food, add some fermented rice water to warm it. Physician Xie from Jiaxing obtained this formula but found it cumbersome; he simply boiled the pig liver in fermented rice water, made pills the size of paulownia seeds, and took 50 pills with porridge drink, which was also effective. For acute diarrhea-dysentery, only one dose is needed. It is unsuitable for heat dysentery and heat diarrhea.
The wife of Bo Yiren in the Zhao mansion suffered from persistent diarrhea. She took Fu Zi (Aconiti Lateralis Radix Praeparata), Mu Xiang (Aucklandiae Radix), He Zi (Terminaliae Chebulae Fructus), Rou Dou Kou (Myristicae Semen), Long Gu (Fossilia Ossis Mastodi et Ossa Cruris), and various elixirs, all ineffective. I was summoned to diagnose; the Liver and Kidney pulses were weak, indicating Liver-Kidney deficiency. The Marquis asked, "Which meridian does this theory refer to?" I said, "Most treatises on diarrhea and dysentery attribute them solely to Spleen-Stomach disorders, discussing invasion by Wind-Cold-Damp-Toxins or food stagnation leading to diarrhea when the intestines are deficient. They fail to recognize that Liver-Kidney Qi deficiency can also cause diarrhea and dysentery. Ancient texts clarify this clearly; it must be distinguished. The Classics state: 'Diarrhea and dysentery unceasing anteriorly and posteriorly indicate Kidney deficiency.' Also: 'All厥逆, obstruction, and diarrhea belong to the Lower Jiao.' Lower refers to the Qi of the Lower Jiao, Liver and Kidneys. The gate's control relies on Liver Qi; guarding below relies on Kidney Qi. When Liver Qi rebels and ascends, the Lower Jiao cannot secure, leading to diarrhea and dysentery. The Kidney is the gate of the Stomach; if the gate is not secured, the granaries cannot store. If the source of diarrhea lies here, treating solely with Spleen-Stomach medicines would be a grave error." Thus, using Mu Xiang San (Aucklandia Powder) for several doses resulted in recovery.
