The Kidney governs the waist and feet. In women, the Kidney is connected to the Bao Luo (uterine vessels). If labor or injury damages the Kidney Qi, causing weakness, and wind-cold pathogenic factors invade the Bao Luo, the pathogenic factors contend with the healthy Qi, resulting in pain in the waist and feet. Treatment should primarily focus on tonifying the Yuan Qi (Original Qi), assisted by formulas that dispel pathogens.
[Xue Ji's Commentary] For the aforementioned syndrome, if it is due to the failure of True Yang, with cold pathogens taking advantage of the deficiency, presenting with cold hands and feet, headache, aversion to cold, or vomiting and abdominal pain, this formula [referring to a warming formula mentioned previously] is appropriate. If it is due to deficiency of Qi and Blood, contracted cold pathogens, presenting with aversion to cold, fever, headache, thirst, or vomiting and abdominal pain, Wuji San (Five Accumulations Powder) is appropriate. (From Miscellaneous Diseases: Cold Damage.) If it is due to deficiency of Yuan Qi, injured by damp-heat, presenting with swelling and pain in both shins, alternating chills and fever, body pain, or vomiting and inability to eat, Bingsu Baidu San (Areca and Perilla Detoxification Powder) is appropriate. (From Miscellaneous Diseases: Dampness Injury.) If it is due to Spleen and Stomach weakness, with sinking of Yuan Qi, presenting with alternating chills and fever, vomiting, feverish sensation, headache, panting, thirst, and physical fatigue, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) is appropriate. (From Miscellaneous Diseases: Fatigue Injury.) If it is due to depletion of Essence and Blood in the Three Yin Meridians of the Foot, with Deficient Fire stirring internally, presenting with internal heat, afternoon tidal fever, thirst, profuse phlegm, and frequent urination, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) is appropriate. (From Miscellaneous Diseases: Consumption Syndrome.) If it is due to failure of Yang Qi in the Three Yin Meridians of the Foot, presenting with aversion to cold, fever, cold hands and feet, spitting phlegm, inability to eat, and slippery/frequent defecation and urination, Bawei Dihuang Wan (Eight-Ingredient Pill with Rehmannia) is appropriate. (From Miscellaneous Diseases: Consumption Syndrome.)
A woman first experienced pain in her waist and hips, followed by pain in both legs. I considered this to be deficiency-cold of the Three Yin Meridians of the Foot, injured by external pathogens. I administered Xiao Xuming Tang (Minor Xu Ming Decoction) and Duhuo Jisheng Tang (Angelica Pubescens and Porcinus Decoction). The symptoms came and went. She felt comfortable in her abdomen only when consuming very hot food and drink. I stated: "The pathogenic factor has departed, but the Yuan Qi is deficient and cold." Upon palpating her pulse, it was indeed deep and fine. I used Yangshen San (Kidney-Nourishing Powder) [see this section], and she gradually recovered. Subsequently, I used Shibu Wan (Ten Tonics Pill) [see Consumption Syndrome], and she was completely cured.
Another woman suffered from the same condition but presented with fever and thirst, preferring cold drinks and food. Her pulse was surging and rapid, but slow and choppy upon heavy pressure. I diagnosed this as Blood deficiency with Heat. I used Lingyangjiao San (Antelope Horn Powder) [see Body Pain], removing Areca Nut, and adding Atractylodis Rhizoma (Baizhu) and Poria (Fuling). After several doses, I switched to Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) [see Tubercle], and she recovered.
Another woman suffered from the aforementioned syndrome, occasionally experiencing pain in her legs and knees. Her pulse was floating and rapid, but slow upon heavy pressure. This indicated Yuan Qi deficiency with Wind-Dampness invading. Using Duhuo Jisheng Tang, she recovered immediately, and then An (stabilized/recovered) using Bazhen Tang (Eight Treasures Decoction).
A woman developed the aforementioned syndrome due to anger, presenting with alternating chills and fever, bitter taste in the mouth, inability to eat, afternoon tidal fever, and internal heat. I diagnosed Liver Fire with Blood deficiency. First, I used Xiaochaihu Tang (Minor Bupleurum Decoction) with Gardenia (Shanzhi), and she recovered immediately. Subsequently, I used Jiawei Xiaoyao San, and she was cured.
A woman suffered from the aforementioned syndrome, presenting with chills, fever, and headache, resembling Shanghan (Cold Damage). This was a Cold Pathogen syndrome. Using Bingsu Baidu San, she stabilized, and subsequently recovered after regulating with Buzhong Yiqi Tang.

Given "Ganqi" often means Licorice, but here might be a typo for another herb or specific preparation. However, standard translation for Ganqi is Glycyrrhizae Radix. But in pain formulas, sometimes refers to other roots. Let's stick to literal Glycyrrhizae Radix unless context implies otherwise. Wait, "Ganqi" can also mean "Dry Wood". In TCM herbs, Ganqi is Licorice. But let's look at the list. It lists "Ganqi (chao qu yan)" - stir-fried to remove smoke. This describes drying lacquer or resin, or perhaps a root. Actually, "Ganqi" (干漆) is Dried Lacquer. Yes, Dried Lacquer is stir-fried to remove smoke. So it is Dried Lacquer.), Niuxi (Achyranthis Bidentatae Radix, soaked in wine, each 1 liang).
