Chuanxiong Rhizoma (Washed) Angelicae Sinensis Radix (Danggui, 当归) Cinnamomi Cortex (Guixin, 桂心) Paeoniae Radix Alba (Baishao Yao, 白芍药)
The above four herbs are each ground into fine powder. Take one tael of the pure powder for each dose, adjusting according to the specific symptoms.
When fetal movement is first felt during pregnancy, use half a qian of Chuanxiong Rhizoma and Paeoniae Radix Alba, taken with rice water. When labor pain is severe at delivery, use half a qian each of Chuanxiong Rhizoma, Paeoniae Radix Alba, and Angelicae Sinensis Radix, plus one qian of Cinnamomi Cortex powder, taken with warm wine (to support the mother's Qi and blood circulation). If lochia is not discharging smoothly postpartum, use half a qian each of Chuanxiong Rhizoma and Angelicae Sinensis Radix, mixed with young boy's urine. Simultaneously, vinegar-soaked charcoal should be fumigated frequently so that the fragrance is continuous; this will naturally consolidate the blood vessels, bringing peace and health without disease. One must absolutely avoid administering random medications, lest excessive blood removal lead to Heart deficiency and weak Qi.
Within three days postpartum, only take one qian each of Angelicae Sinensis Radix and Chuanxiong Rhizoma powder, mixed with young boy's urine, three times daily. After three days, only take Sishun Lizhong Wan (Four-Harmony Regulating Middle Pill), swallowed with young boy's urine on an empty stomach. For tonifying deficiency within seven days, take one qian each of Angelicae Sinensis Radix and Chuanxiong Rhizoma powder, mixed with young boy's urine or rice water; after seven days, tonic medicines may be taken. Tonifying too early will retain evil blood. Once the condition stabilizes, take half a qian each of the four herbs, mixed with young boy's urine. For postpartum dizziness and vertigo, take half a qian each of Chuanxiong Rhizoma, Paeoniae Radix Alba, and Angelicae Sinensis Radix powder, mixed with young boy's urine. For those less than or more than one month postpartum, take half a qian each of the four herbs, decocted in one cup of water until reduced to eight-tenths, taken warm. For abdominal pain, take one qian of Angelicae Sinensis Radix powder and half a qian of Cinnamomi Cortex powder, mixed with young boy's urine. For chest and heart irritability, take one qian each of Angelicae Sinensis Radix and Paeoniae Radix Alba, mixed with young boy's urine. For excessive lochia, take one qian each of Cinnamomi Cortex, Paeoniae Radix Alba, and Angelicae Sinensis Radix, mixed with young boy's urine. For cold constipation, use Lizhong Wan (Regulate Middle Pill), decocted in one cup of water until reduced to eight-tenths, taken warm.
Treats pre- and postpartum abdominal pain, body heat, headache, and various ailments. Immediately after delivery, before consuming any other food, administer this medicine first. It can eliminate various diseases, expel stale blood, and generate new blood.
Chuanxiong Rhizoma (Two taels) Angelicae Sinensis Radix (Three taels)
Grind the above into fine powder. Each dose is two qian. Decoct in one cup of water and two parts wine until reduced to seven-tenths, taken warm.
Another method: grind into coarse powder. Each dose is four qian. Decoct in seven parts water and three parts wine until reduced to seven-tenths, taken hot. Prepare this medicine before delivery; decoct both doses immediately after birth and administer quickly. Within three days, twice daily; after three days, once daily.
Another prescription named Xiongui Tang (Chuanxiong and Danggui Decoction) consists of these two herbs in equal proportions, chopped and decocted in water. It specifically treats hemorrhage due to injury to the fetus, postpartum hemorrhage, metrorrhagia, trauma wounds, tooth extraction bleeding that does not stop, and any excessive blood loss causing mental restlessness, vertigo, fainting and unconsciousness, heavy head, dim vision, and feeling as if falling over when lifting the head; it can treat all these conditions. If postpartum vertigo occurs, add Paeoniae Radix Alba.
Also known as Guixiang San (Cinnamon Fragrance Powder), it treats unbearable postpartum abdominal pain. Add Cinnamomi Cortex in equal proportion, decoct with wine and young boy's urine; administration brings immediate relief.
Also known as Danggui Tang (Angelica Decoction), it treats pregnancy where the fetus is dead or alive but unstable, with fetal agitation. Decoct each dose with wine and water, taking several doses consecutively. If the fetus is already dead, it will be expelled upon ingestion; if alive, the fetus will stabilize. This classic has been effective repeatedly, never failing.
Also known as Hupo San (Amber Powder), taken near term to facilitate easy delivery by contracting the fetus. It also treats various postpartum ailments.
Also known as Yangrou Tang (Mutton Decoction), it treats deficiency and emaciation, abdominal pain, alternating chills and fever, shortness of breath, inability to sustain oneself, dizziness with spontaneous sweating, and abdominal tension. Each dose uses one tael of lean mutton, ten slices of ginger, and two cups of water, decocted until reduced to six-tenths, taken warm.
Also known as Junchen San (Ruler-Minister Powder), it treats women and unmarried girls with heart and abdominal pain, irregular menstruation, decocted in water. For pregnant women with unstable fetal Qi or various postpartum ailments, add wine and decoct.
Also known as Xiong Dang San (Chuanxiong and Danggui Powder), it treats women's Qi and Blood disorders with upper asthma and lower edema. The two herbs are ground equally into fine powder; each dose is two qian, taken on an empty stomach mixed with mugwort water.
It also treats postpartum physical damage, blood rushing to the Heart, and abdominal distension with Qi stagnation, proving miraculous.
For difficult labor, transverse presentation, or fetus dead in the womb, first stir-fry one large measure of black beans until cooked. Add one cup of water and one cup of young boy's urine, along with four qian of herb powder, decoct until reduced to one cup. Divide into two doses; if ineffective, prepare again. For postpartum evil blood attacking the Heart, causing confusion, panting, urgency, and abdominal pain, follow the previous method, adding half a measure of fresh ginger juice to the black bean decoction.
Furthermore, this medicine also treats intestinal abscesses (Zangdu, 脏毒). Each dose is one and a half qian, added with half a qian of fried Sophorae Flos powder, decocted in one cup of water until reduced to six-tenths, taken at any time. After three days, bloody clots will be discharged, and recovery follows. For postpartum headache, add Schizonepetae Herba (Jingjie, 荆芥) for decoction. It is also suitable for hematemesis.
Discussion on the main indications of Danshen San (Salvia Decoction) (from "Mingli Fang" treatise): Siwu Tang (Four Substances Decoction) is widely used by women because it is universally applicable regardless of pre- or postpartum status or menstrual flow volume. Only Danshen San shares similar main indications. Why? Because it can break up old blood, tonify new blood, secure a living fetus, expel a dead fetus, stop metrorrhagia and leukorrhea, and regulate menstruation. It is largely similar to Angelicae Sinensis Radix, Rehmanniae Radix, Paeoniae Radix Alba, and Chuanxiong Rhizoma.
