Pregnancy eclampsia (Zixian) is a malignant syndrome. If not treated early, it will inevitably lead to miscarriage. Its symptoms include lockjaw (koujin), neck stiffness (xiangqiang), spasms and contracture of the hands and feet, dysarthria (yanyu jianshe), excessive phlegm-saliva (tanyan yongsheng), and loss of consciousness (buxing renshi); or sudden dizziness and collapse, inability to speak, resembling stroke (Zhongfeng), but actually it is not a stroke syndrome, and must not be treated as such. Even if there is no phlegm and speech is normal, but wind-like symptoms are present, it is mostly due to blood dryness (xue zao) or blood deficiency (xue xu), and should not be generally treated as wind pathogen to avoid misdiagnosis. The main formula is Lingyangjiao San (Antelope Horn Decoction). Feng Shi stated: When a pregnant woman suddenly collapses stiffly, with excessive phlegm-saliva and loss of consciousness, this is due to blood deficiency causing Yin fire to flare upward, stirring up phlegm. The left pulse is slightly rapid (wei shu), and the right pulse is slippery and large (hua da), named Pregnancy Eclampsia (Zixian). It is suitable to use Siwu Tang (Four Substances Decoction) to nourish blood, wine-processed Huangqin (Scutellariae Radix) to clear heat, Erchen Tang (Two Aged Ingredients Decoction) to resolve phlegm and regulate Qi. The treatment method should still focus on calming the fetus (antifetal agitation), and one should not overuse medicines for treating stroke. This arises from blood deficiency generating heat, and excessive heat generating wind; all are internally generated wind-fire. Nourishing blood extinguishes the wind naturally. If it is Heart-Liver wind-heat, use Gouteng Tang (Uncaria Decoction); if Liver-Spleen blood deficiency, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder); if Liver-Spleen depression and anger, use Jiawei Guipi Tang (Modified Restore the Spleen Decoction); if Qi stagnation with phlegm obstruction, use Zisu Yin (Perilla Infusion); if Spleen depression with phlegm obstruction, use Erchen Tang plus Zhuli (Bamboo Sap) and Jiangzhi (Ginger Juice). Mizhai stated: For pregnant women with Qi deficiency complicated by phlegm-fire syndrome, resembling stroke, sudden collapse followed by immediate awakening, and recurrence after awakening, the main formula is Qingshen Tang (Spirit-Clearing Decoction). I believe: In pregnancy eclampsia, cases progressing to blurred vision, blackouts, and cold extremities (muhun hei er jue) are extremely rare in the prenatal period. However, whenever such symptoms occur, it is "Fetal Dizziness" (Eryun), belonging to Qi and Phlegm issues. Therefore, for blurred vision and fainting, only Zisu Yin (Perilla Infusion) should be taken. One must strictly avoid taking Suhewan (Amber Pill) and Wuyao Shunqi San (Tso-Yao Regulating Qi Powder) and similar medicines. As for trauma with blood loss, or hematemesis (tu xue) and epistaxis (nu xue), suddenly developing lockjaw, neck stiffness, and rigid back, resembling stroke symptoms, these are all caused by blood loss and must be known. Use Jingfang Antai San (Schizonepeta and Saposhnikovia Fetus-Calming Powder) to treat them.
