Jin Feng (Lockjaw), Cuo Kou (Contracted Mouth), and Qi Feng (Umbilical Wind) in newborns are essentially manifestations of the same disease. Jin Feng is characterized by closed eyes, lockjaw, absence of crying sound, accumulation of flesh-like granules on the tongue resembling millet grains, inability to suckle milk, foaming at the mouth, while urination and defecation remain normal. This is primarily due to heat pathogens contracted during gestation, which flow into the Heart and Spleen meridians, manifesting symptoms in the throat and tongue; alternatively, it may be caused by wind pathogens attacking the body after birth. This condition typically appears between one month and 120 days after birth and is termed "Fan Feng Jin" (Wind-Invading Lockjaw). If proper care is taken according to standard methods to prevent it beforehand, this affliction can be avoided.
Cuo Kou presents with yellowish-red complexion, rapid breathing, and absence of crying sound. It results from fetal qi carrying heat combined with wind pathogens entering the umbilicus and flowing into the Heart and Spleen meridians, causing a stiff tongue, blue lips, contraction of the mouth, and tightening of facial muscles, thereby hindering suckling. If white foam issues from the mouth accompanied by cold limbs, the condition is incurable. Abdominal distension, visible subcutaneous veins, intestinal hernia, or scrotal swelling, along with internal pain caused by qi traction, are all due to stagnation and obstruction of the gastrointestinal qi mechanism. The treatment principle emphasizes unblocking and promoting drainage. Cuo Kou is the most severe syndrome; its appearance within the first week (one lunar fortnight) after birth is particularly urgent.
Qi Feng occurs after umbilical cord cutting when water-dampness and wind-cold pathogens invade. The wind-damp qi enters the umbilicus and flows into the Heart and Spleen, leading to abdominal distension, umbilical swelling, heavy body sensation, rigid limbs, incessant day-and-night crying, and inability to suckle. In severe cases, it develops into wind convulsions. If the skin around the umbilicus turns dark blue-black, the mouth remains tightly shut, this indicates internal convulsion (Nei Chu), which is untreatable; black fingernails indicate imminent death. If heat accumulates in the chest, manifesting as stretching limbs and forceful exhalation, it also causes umbilical swelling; in such cases, Qianjin Longdan Tang (Thousand Gold Gentian Decoction) may be used.
The etiology of these three conditions does not develop overnight but stems from chronic internal qi stagnation and blockage. Treatment generally involves purgative methods to expel fetal toxins, using formulas such as Tianma Wan (Gastrodia Pill), Dingming Dan (Life-Determining Elixir), or Zhuyin Wan (Cinnabar-Silver Pill), dosed appropriately. *Qianjin Fang* states: Newborns have vigorous qi; if minor ailments arise, purgatives should be administered promptly. Delay leads to severe disease, which becomes difficult to treat once established; Zishuang Wan (Purple Frost Pill) may be given in appropriate doses. For Feng Jin, Kongtan San (Phlegm-Control Powder) induces vomiting of wind-phlegm, but Gancao Tang (Licorice Decoction) is preferred for stable phlegm expulsion, followed by He Wei Yi Pi San (Stomach-Harmonizing Spleen-Benefiting Powder) modifications and Chensha Gao (Cinnabar Paste) to calm fright, resulting in recovery. Hand clenching with locked jaw is an untreatable sign.
For Cuo Kou, Jiangcan Gao (Bombyx Paste) applied to lips and mouth, Gancao Tang, or Cuofeng San (Wind-Contracting Powder) may be used. Qi Feng is treated with Gudi San (Melon Pedicle Powder). Dr. Tang treats umbilical ulcers non-healing due to external wind invasion during cord cutting by applying powder of Bai Fan (Alum) and Long Gu (Dragon Bone) or burning old cotton to ash for dry application.
Regarding umbilical cord management: If cut short (less than one chi), the root naturally withers within one lunar fortnight. Those with strong constitution have deep roots, while weak ones have shallow roots, determined by innate endowment. Newborns with chest heat stretch frequently, make grunting sounds, and strain qi, causing heat to sink deeply, resulting in protruding, red, swollen, and alarming-looking umbilicus. Laypeople often blame improper cord cutting, which is incorrect. This arises from maternal fright/anxiety or consumption of hot-toxic foods during pregnancy. Dalianqiao Yinzi (Large Forsythia Decoction) disperses heat and restores the umbilicus without topical medication. Early treatment is crucial; delay makes cure difficult.
Another method: Infants with Qi Feng/Cuo Kou presenting within seven days of birth rarely survive. To avoid passive observation of death, a highly effective but rare method exists: inspect the infant's gums for millet-sized blisters. Wipe them open with a warm, wet cloth-wrapped finger; the infant will open their mouth and stabilize without medication.
Anqi San (Umbilicus-Anchoring Powder) treats umbilical discharge or redness/swelling using Baishi Zhi (White Halloysite) powder applied three times daily, or You Fa Hui (Hair Ash) or Dang Gui (Angelica Sinensis) powder.
