At the end of summer and beginning of autumn, infants suffering from diarrhea and dysentery are usually caused by sucking milk with internal heat from their mothers, or by consuming raw, cold drinks and foods leading to accumulation, combined with improper care. Diarrhea and dysentery occurring in summer are invariably heat syndromes. Treatment should employ medications to resolve food stagnation and guide it out, separating turbidity and promoting urination to harmonize Yin and Yang, after which the diarrhea and dysentery will naturally cease. However, some foolish people give infants distilled liquor (Huojiu) mixed with brown sugar on an empty stomach. Occasionally, for robust infants who can drink, if the condition is indeed due to cold-raw accumulation, there may be some effect. But they do not know that after distilled liquor sits overnight, its toxicity floats and gathers, forming a scum on the surface. When pouring from the flask, this toxin goes directly into the vessel. Infants drinking this substance present with body stiffness and collapse, and convulsions of the limbs, resembling Jing-Feng (Convulsion) or epilepsy. The difference is that the infant's four limbs remain soft as cotton, the complexion is cyanotic, and there is a smell of alcohol in the mouth and nose; this is the syndrome of "Poisoning by Distilled Liquor" (Zhong Huo Jiu Du). One must carefully inquire if the infant consumed distilled liquor. Place the infant in a wind-shielded, warm area. First, administer Yi Yuan San (Yuan Powder, a formula for clearing heat and resolving summerheat) mixed with cool water. Subsequently, use Si Ling San (Four-Ling Decoction, a formula for draining dampness) added with Ge Gen (Kudzu Root) and Huang Lian (Coptis Rhizome) for gradual regulation. Some infants may take two or three days to regain Yang energy and speak; this can be considered a revival. If treated as Convulsion, survival is rare. I have witnessed such cases repeatedly, hence this record.
Yi Yuan San (See Section on Diarrhea)
Hu Po Bao Long Wan (Amber Embracing Dragon Pill, See previous section on Convulsion, refer to Huang Lian Gan Cao Tang usage)
Si Ling Jie Du Tang: Treats poisoning by distilled liquor, and also treats heat-type diarrhea and dysentery.
Prescription: Bai Zhu (Atractylodis Rhizoma, 1 qian), Fu Ling (Poria, 1 qian), Zhu Ling (Polyporus, 1 qian), Ze Xie (Alisma Rhizome, 1 qian), Gan Ge (Puerariae Radix, 2 qian), Huang Lian (Coptidis Rhizoma, 1.5 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma, 1 qian).
Preparation: Deng Xin Cao (Junci Medulla, 20 cun), decoct in water. Wait for the liquid to cool, then administer frequently like water. If heat is severe, add Xiang Ru (Moslae Herba), Chai Hu (Bupleuri Radix), Huang Qin (Scutellariae Radix), Mu Tong (Akebiae Caulis); if phlegm is excessive, add Bei Mu (Fritillariae Bulbus), Mai Dong (Ophiopogonis Radix), Hua Fen (Trichosanthis Radix), Huang Qin.
Treatment of Shang Han (Cold Damage) should follow the methods of Zhang Zhongjing, ensuring precise correspondence between symptoms and prescriptions, a level rarely achieved by other physicians. Tao Jie'an proposed 397 methods and 113 formulas, amidst which various complex diseases arise; without true transmission of knowledge, one cannot discuss this matter. Pediatric Shang Han is no different from adult Shang Han, except for the concurrent presence of Jing-Feng (Convulsion) and Ji-Zhi (Food Accumulation). The dosage of decoctions should be reduced according to the child's age compared to adults. In adults, pulse diagnosis combined with obvious symptoms makes the diagnosis clear; however, pediatric symptoms are hidden and difficult to observe. Moreover, the mixture of Jing-Feng, Ji-Zhi, and Bian Zheng (physiological changes during infant growth) makes differentiation even harder. If Shang Han is confirmed, one must not use drastic sweating or purging methods. Why? Because children have fragile intestines and stomachs, and delicate skin. Once subjected to sweating or purging, complications such as Dou Sha (smallpox/rash diseases) may arise, making treatment difficult. Be cautious! Be very cautious! One must also examine the Three Passages at the Tiger’s Mouth (veins on the inner aspect of the infant's index finger); if the vein color is red-purple and vibrant, it indicates this syndrome. For older children, palpate the Ren Ying pulse (carotid artery); if it is twice as strong as the Qi Kou pulse (radial artery), it confirms a true exterior cold contraction. One must consider the climate, identify interior/exterior locations, differentiate Yin/Yang, and prudently decide on sweating or purging based on the specific situation. Medication requires extreme precision; a difference of a hair leads to a thousand-mile error. Physicians hold the fate of humans; how can they not be careful? Additionally, during epidemic plagues, both adults and children are contagious, and initial symptoms are similar. However, the treatment differs from Shang Han and cannot be treated uniformly. One must understand the climate, recognize the annual运气 (Luck/Climate cycle), adjust for cold/warmth, determine which meridian is affected, and treat accordingly. Medications should primarily focus on cooling and clearing heat-toxins.
Ma Huang Tang: Treats Cold injuring the Ying (Nutrient) level.
Prescription: Ma Huang (Ephedrae Herba, 3 qian), Xing Ren (Armeniacae Semen Amarum, 1.5 qian), Gui Zhi (Cinnamomi Ramulus, 1 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma, 0.5 qian).
