1. Shangyang (Jing-Well Point)
Location: On the radial side of the index finger, approximately the width of a leek leaf from the corner of the nail.
Indications: Fullness of qi in the chest, wheezing and coughing, swelling of the limbs, heat diseases with no sweating, tinnitus and deafness, alternating chills and fever, malaria, dry mouth, swelling of the mandible, toothache, aversion to cold, stiffness of the shoulder and back, pain radiating to the clavicular fossa (Quepen), blindness.
Acupuncture and Moxibustion: Insert needle 1 fen deep, retain for one breath. Apply cross-needling (left disease treated on right, right disease treated on left). Recovery occurs within the time it takes to eat a meal. Moxibustion: 3 moxa cones.
Commentary: Shangyang is the starting point of the Yang Metal Jing-Well point of the Large Intestine channel. "Water above wood" is called a well; water is generated by metal (Metal generates Water). Yang is often in excess. The musical note 'Shang' corresponds to the Lung, and the Large Intestine is internally-externally related to the Lung. Hence, it is named Shangyang. The Large Intestine meridian ascends through the neck, passes through the cheek, crosses at Renzhong (人中), going from left to right and right to left, and ascends flanking the nostrils. Therefore, for treating dental and ocular diseases, one should treat the left side using the right point and vice versa.
2. Erjian (Ying-Spring Point)
Location: In the depression on the ulnar side, proximal to the proximal interphalangeal joint of the index finger.
Indications: Throat obstruction (Bi syndrome), swelling of the jaw, shoulder and back pain, shivering chills, nasal congestion or epistaxis, frequent fright, deviation of the mouth or speech difficulties.
Acupuncture and Moxibustion: Insert needle 3 fen deep. Moxibustion: 3 moxa cones.
Commentary: Erjian is the Yang Water Ying-Spring point of the Large Intestine channel. Metal and Water generate each other; it can be needled and moxibusted. The second digit of the hand is also called the index finger, which has three segments. This point lies between the second and third segments, hence the name Erjian (Second Interval).
3. Sanjian (Shu-Stream Point)
Location: In the depression on the ulnar side, distal to the proximal interphalangeal joint of the index finger.
Indications: Throat obstruction, sensation of a foreign body in the throat like a fishbone, carious toothache, lethargy, chest fullness, borborygmus, watery diarrhea, cold malaria, scorched lips and dry mouth, asthma, urgent eye pain.
Acupuncture and Moxibustion: Insert needle 3 fen deep, retain for three breaths. Moxibustion: 3 moxa cones.
Commentary: Sanjian is the Yang Wood Shu-Stream point of the Large Intestine channel, where the Qi pours in. It is located posterior to the third bone of the proximal phalanx of the index finger and anterior to the bifurcation bone of the thumb and index finger. The point resides in this interval, thus named Sanjian (Third Interval).
4. Hegu (Yuan-Source Point)
Location: In the depression between the metacarpal bones of the thumb and index finger.
Indications: Treatment of alternating chills and fever, malaria, nasal congestion or epistaxis, heat diseases with no sweating, blurred vision, headache, carious toothache, throat obstruction, flaccid paralysis of the arm, facial swelling, inability to close the lips, aphonia, trismus (lockjaw).
Acupuncture and Moxibustion: Insert needle 3 fen deep, retain for six breaths. Moxibustion: 3 moxa cones. Contraindicated in pregnant women, as needling may damage fetal Qi.
Commentary: Hegu is the Yuan-Source point of the Hand-Yangming Large Intestine channel. It is located in the slight depression slightly偏向 the index finger side between the bifurcation of the thumb and index finger. Directly below, it aligns with Laogong (PC8) and Houxi (SI3) in a straight line. Where the thumb and index finger meet, the shape resembles a deep valley, hence the name Hegu (Combining Valley). For severe conditions, it can be inserted deeply 8 fen or 1 cun.
5. Yangxi (Jing-River Point)
Location: In the depression between the tendons of the thumb and index finger on the radial aspect of the wrist crease.
Indications: Delirious speech, excessive laughter, hallucinations of seeing ghosts, irritability due to heat diseases, wind-pathogen causing red, rotting eyes with nebula,厥逆 (Jue-ni) headache, chest fullness with dyspnea, alternating chills and fever malaria, throat obstruction, tinnitus, toothache,惊掣 (frightful convulsions) preventing elevation of the elbow and arm, scabies.
Acupuncture and Moxibustion: Insert needle 3 fen deep, retain for seven breaths. Moxibustion: 3 moxa cones. Precautions are the same as for Hegu.
Commentary: Yangxi is the Jing-River point of the Hand-Yangming Large Intestine channel, where it intersects with Lieque (LU7), the Luo-Connecting vessel of the Hand-Taiyin Lung. The Lung is located highest, while the Large Intestine is lower. When the palm is extended dorsally, the tendons of the thumb and index finger rise high. The intersection of the Taiyin and Yangming channels resembles a mountain stream, hence the name Yangxi (Yang Stream).
6. Quchi (He-Sea Point)
Location: Lateral to the olecranon process of the humerus, in the depression formed when the elbow is flexed. To locate, flex the elbow against the chest.
Indications: Pain in the elbow, hemiplegia due to wind-stroke, urticaria (wind-rash), throat obstruction with inability to speak, chest烦满 (fullness and irritability), flaccidity of sinews, inability to grasp objects, inability to draw a bow, difficulty in flexion and extension, thin and weak arms/elbows, residual heat from cold damage diseases, dry skin.
Acupuncture and Moxibustion: Insert needle 7 fen deep. Upon obtaining Deqi, apply reducing method first, then reinforcing method. Moxibustion: 3 moxa cones.
Commentary: Quchi is the He-Sea point of the Hand-Yangming Large Intestine channel. Yang He-Sea points belong to Earth. The Yangming channel is abundant in Qi and Blood; excess should be reduced. However, the Large Intestine belongs to Geng Metal, and the Earth point is the mother of Metal (Earth generates Metal). Therefore, after reducing, one should reinforce. When pathogenic Qi arrives, the pulse is tight and rapid; when Gu Qi (healthy Qi) arrives, it is slow and harmonious. The distinction between reducing and reinforcing is felt under the needle. The point is located in the slight depression anterior to the lateral epicondyle of the humerus, found by flexing the arm, hence the name Quchi (Curved Pool).
