Cao Yigang frequently experienced nocturnal emissions. I said: This is due to insufficiency of Shen and Zhi (Spirit and Will). He also suffered from hernia, and recently developed epigastric discomfort (Zaocha), with a sensation of obstruction and pain in the throat when eating hard foods. I reasoned that diseases have urgency and non-urgency, so treatment must follow a sequence. Throat conditions are certainly the most urgent. Initially, I cleared and purified the Upper Jiao (Upper Burner), then tonified Shen and Zhi, ensuring the Spirit was robust and Essence had a master, preventing reckless emission. Subsequently, I used formulas for the Lower Jiao to treat the hernia. To clear and purify the Upper Jiao, I used Liu Jun Zi Tang (Six Gentlemen Decoction) added with Hua Shi (Talcum), Jiu Lian Lian (Wine-processed Coptidis Rhizoma), Pi Pa Ye (Loquat Leaf), Lu Chai Gen (Reed Rhizome), Xiang Fu (Cyperus Rhizome), and Wu Zhu Yu (Evodiae Fructus). After four doses, the epigastric discomfort ceased, and the throat felt relaxed. Then, I used Zhu Gan Wan (Pork Liver Pill) to tonify Shen and Zhi, consisting of Yuan Zhi (Polygalae Radix), Shi Chang Pu (Acori Tatarinowii Rhizoma), Shi Lian Zi (Nelumbinis Plumula), Jiu Cai Zi (Allii Tuberosi Semen), Huang Lian (Coptidis Rhizoma), and Bei Mu (Fritillariae Bulbus), each 2 liang; Bai Zhu (Atractylodis Macrocephalae Rhizoma) 5 liang; Gou Qi Zi (Lycii Fructus) and Bai Fu Ling (Poria) each 1 liang, ground into powder, placed inside a male pig's stomach, steamed until cooked over rice, mashed into pills the size of Sophora seeds, coated with Zhu Sha (Cinnabar), taken at night with Deng Xin Cao Tang (Junci Medullae Decoction), 2 qian per dose. For the hernia pill, Ju He (Aurantii Fructus Immaturus) and Kun Bu (Laminariae Thallus) each 4 liang; Chuan Jiao (Sichuan Pepper), Chao Hei Shan Zhi Zi (Stir-fried Black Gardenia Fruit), and Shan Zha He (Crataegi Fructus Seed) each 2 liang; Chai Hu (Bupleuri Radix) and Xiao Hui Xiang (Foeniculi Fructus) each 1 liang; Bu Ji Zi Qiao (Shell of Poultry Egg) 3 liang, made into pills with Qu Hu (Malt Paste), taken on an empty stomach with plain water or wine, 3 qian per dose. The disease was cured before the course was finished.
Wu Jingqiao, a scholar and a gentleman of amiable disposition. His Left Guan and Chi pulses were very wiry. He had suffered from hernia for half a year, with hard, painful nodules in both groins. I treated him with Ping Shen Wan (Hernia-Calming Pill) to dissipate them: Hai Zao (Sargassum Thallus), Kun Bu (Laminariae Thallus), and Ju He (Aurantii Fructus Immaturus) each stir-fried with wine and vinegar, 4 liang; Yan Hu Suo (Corydalis Rhizoma), Shan Zhi Zi Ren (Gardenia Seed), Shan Zha He (Crataegi Fructus Seed), Xiao Hui Xiang (Foeniculi Fructus), and Chai Hu (Bupleuri Radix) each 1 liang; Long Dan Cao (Gentianae Radix) wine-processed, 5 qian. Made into pills with vinegar-dough paste, size of Sophora seeds, taken on an empty stomach with wine, 3 qian per dose. After one month of administration, the nodules dissipated.
Mr. Wu Hezhou had phlegm in the Middle Jiao (Middle Burner) and insufficiency of Lung Qi. He suffered from malaria occurring once daily, with more heat than chills, thirst, difficult urination, fatigue, and headache. His pulse was wiry and large on the left, short and weak at the Right Cun, and slippery and large at the Right Guan and Chi. I prescribed Shi Gao (Gypsum Fibrosum), Zhi Mu (Anemarrhenae Rhizoma), and Huang Qin (Scutellariae Radix) decocted together with Chai Ling Tang (Bupleurum and Pinellia Decoction). After taking it, he experienced abdominal diarrhea. I removed Shi Gao and Zhi Mu from the previous formula, and the pathogenic heat decreased by more than half. However, tidal fever and severe thirst remained. I changed the prescription to Ren Shen (Ginseng Radix), Ge Gen (Puerariae Lobatae Radix), Zhi Mu (Anemarrhenae Rhizoma), Mai Men Dong (Ophiopogonis Tuber), Chai Hu (Bupleuri Radix), Chen Pi (Citri Reticulatae Pericarpium), Gan Cao (Glycyrrhizae Radix et Rhizoma), Bai Zhu (Atractylodis Macrocephalae Rhizoma), and Bie Jia (Testudinis Carapax et Plastrum), administered at Wugeng (the third watch of the night, around 1-3 AM), resulting in recovery.
