Book: Ningkun Miji (Secrets of Gynecology)
Chapter: Retained Placenta / Regulation and Nursing Care
Category: Comprehensive
[English Translation]
Because the mother lacks strength to expel the placenta after the fetus is delivered, and because it remains for a long time, allowing external cold qi to invade, the blood pathways become congealed and obstructed, resulting in the retained placenta. Furthermore, if the mother was constitutionally weak before delivery, with depleted Qi and Blood causing the placenta to stagnate, one should immediately decoct a large dose of Shenghua Tang (Postpartum Recovery Decoction), administering two or three bowls continuously, so that Blood becomes abundant and the abdomen harmonizes, allowing the placenta to fall naturally. Another method is to administer Yimu Wan (Motherwort Pill), followed by the use of Lujiao Hui (Deer Horn Ash); all formulas listed subsequently can be used.
Yimu Wan Formula
Harvest Motherwort (Leonuri Herba) on the day after the Dragon Boat Festival, hang it in a ventilated place to dry in the shade, pound it into powder using a stone mortar, and make pills the size of a bullet using honey. When taking, crush the pill, place it in a cup, steam it hot in a pot, and swallow with Shenghua Tang.
When the fetus is delivered but the placenta is not, the parturient may sit waiting; she must not lie down to sleep. First, cut the umbilical cord and press it with a straw shoe. If in cold months, support the parturient to sit leaning against the bed, cover her with quilts, use a fire basket for warmth, and keep clothes warm over the abdomen. After the placenta descends, to prevent deficiency collapse, quickly administer two cups of Shenghua Tang. Do not dislike frequent medication; there will be wonderful effects. Jikun Dan (Empowerment of the Womb Pellet) is extremely effective for expelling the placenta but must not be taken in excess; limit to two or three pellets. Zhu Danxi's "Zuanyao" records that using Poxiao (Mirabilite) for expelling the placenta is miraculous, but it harms those who are weak. The abdomen is empty after delivery; is this not weakness? This must not be a formula established by the master himself, but added later by his disciples. Jikun Dan is listed below. Rusheng Gao (Sage Paste) uses Bi Ma Zi (Ricini Semen, Castor Seed, 2 liang) and Xionghuang (Realgar, 2 qian). Grind these two ingredients into a paste and apply to the sole of the mother's foot; the placenta will then descend.
Jia Shen Shenghua Tang (Shenghua Tang with Added Ginseng) Formula
This is generally used for various critical postpartum conditions. One must take three or four doses within a day and night. If taken normally, one dose per day, how can it rescue the nearly exhausted Qi and save critical conditions?
Chuanxiong Rhizome (4 qian), Dang Gui Root (8 qian), Dried Ginger (4 fen, stir-fried until black), Licorice Root (5 fen, honey-fried), Peach Kernel (10 grains, processed, peeled), Ginseng Radix (2 qian).
For deficiency collapse, cold limbs, and excessive sweating, add Ginseng Radix (3-4 qian) and decoct with Jujube Decoction. Modifications: For pulse and form both depleted with Qi about to vanish, this formula must be taken, administered frequently to rescue; add Ginseng Radix (4-5 qian) to Shenghua Tang. For cold limbs and sweating after delivery, take Shenghua Tang with Ginseng Radix (3-4 qian); excessive sweating will also stop. Add Ophiopogon Tuber (1 qian); for left/right pulse depletion, also add Ginseng Radix.
For postpartum excessive sweating, add Ginseng Radix (3-4 qian); for excessive sweating with severe thirst, add Ginseng Radix and Ophiopogon Tuber (3 qian). For excessive sweating with phlegm-wheezing, add Bamboo Sap, Ginger Juice, and Apricot Kernel (10 grains). For excessive sweating with wheezing/cough and heavy voice, add Platycodon Root (5 fen) and Apricot Kernel (5 fen). For no sweating with wheezing/cough and shortness of breath, add Pinellia Tuber (>1 qian), Apricot Kernel (10 grains), and Platycodon Root (5 fen). For excessive sweating with body heat and shortness of breath, add Ginseng Radix. For unceasing sweating, after three or four doses, add Astragalus Root (1 qian). Within three days postpartum, if mass pain persists, use Ginseng Radix cautiously; add Ginseng Radix in emergencies to save life. If the disease trend shows signs of recovery, reduce Ginseng Radix and only take the original Shenghua Tang.
Jiawei Shenghua Tang (Modified Shenghua Tang) Formula
Treats postpartum shortness of breath resembling panting but not quite panting, where Qi does not connect. Some cases have concurrent Heat, some have concurrent Phlegm, some have one or two symptoms leading to critical shortness of breath. The main treatment should be greatly tonifying Qi and Blood. Although there are concurrent Wind-Cold pathogens with symptoms of headache, fever, and aversion to cold, one must prioritize postpartum regulation. Since Shenghua Tang contains Dried Ginger and Chuanxiong, adding exterior-releasing agents is very safe. However, those with Shanghan (Cold Damage) must be cautious with dispersing methods. Zhu Danxi said: Postpartum, one must absolutely not use exterior-releasing methods.
Chuanxiong Rhizome (1 qian), Dang Gui Root (2.5 qian), Honey-fried Licorice Root (5 fen), Dried Ginger (4 fen), Peach Kernel (10 grains), Ginseng Radix (2 qian), Ziziphus Seed (1 qian).

For much Phlegm, add Astragalus Root (1 qian); for previous symptoms of excessive sweating, add Astragalus Root and Ginseng Radix. If mass pain persists, pause and use Ginseng Radix and Astragalus Root to settle the mass.
Postpartum excessive sweating, slight wheezing, shortness of breath, and extreme laziness in speech indicate Qi deficiency and Blood collapse. Quickly take the previous formula; externally, use Vinegar Charcoal from time to time to prevent fainting.
Jiawei Buzhong Yiqi Tang (Modified Tonify the Middle and Augment the Qi Decoction) Formula
This formula can only be used after taking three doses of the original Shenghua Tang. If mass pain continues despite taking this...
Ginseng Radix (2 qian), Dang Gui Root (3 qian), Honey-fried Licorice Root (5 fen), Chuanxiong Rhizome (1 qian), Tangerine Peel (3 fen), Astragalus Root (8 fen), Chinese Yam (1 qian), Ophiopogon Tuber (1 qian).
For thirst, add Schisandra Berry (10 grains); for blood mass pain, add Peach Kernel (10 grains); for urinary obstruction, add Poria (8 fen). Absolutely do not use Polyporus, Alisma, or Akebia to cause loss of Body Fluids. Only Poria may be used.
Postpartum headache, fever,急促 breathing/panting, with sweating or severe panting, indicates sudden exhaustion of Qi and Blood, or caused by prolonged labor in the lying-in period. One must urgently decoct Shenghua Wannan Tang (Shenghua Ten-Thousand-Safe Decoction) and gradually add Ginseng Radix for treatment. Do not mistake it for excess Qi panting and mistakenly use descending or smoothing Qi medicines.
Another Jiawei Shenghua Tang Formula for Postpartum Headache, Fever,急促 Breathing, Panting, and Sweating
Chuanxiong Rhizome (2 qian), Dang Gui Root (3 qian), Ginseng Radix (3-4 qian), Honey-fried Licorice Root (5 fen), Tangerine Peel (3 fen), Apricot Kernel (7 grains, peeled and tip removed). Within seven days, use Astragalus Root (3 qian), Ziziphus Seed (1 qian), Ophiopogon Tuber (1 qian).
Panting and sweating postpartum are critical conditions. Many suspect Ginseng Radix promotes panting and dare not use it, leading to many untreatable cases. Now, added to the Xionggui Decoction, it is foolproof. Some ignorant people do not use Ginseng Radix; quack doctors hinder and mislead patients. Even if small amounts of Ginseng Radix are used, adding Tangerine Peel and Aurantium Fruit to control them causes dispersion; this must not be trusted. It is better to use Dushen Tang (Single Ginseng Decoction) which is foolproof. Postpartum Qi and Blood are extremely deficient; sweating occurs immediately after delivery. One must first take two or three doses of pre-prepared Shenghua Tang from before pregnancy. After finishing, when masses disappear and pain stops, then take Tingwei Zhihan Tang (Regulate Defense and Stop Sweat Decoction).
Tingwei Zhihan Tang Formula
Ephedra Root (Mahuang Gen) inside must not be mistaken. Using Mahuang Gen is good for stopping sweat; postpartum, inducing sweat is strictly forbidden.
Astragalus Root (1 qian), Dang Gui Root (2 qian), Ephedra Root (1 qian), Honey-fried Licorice Root (5 fen), Saposhnikovia Root (3 qian), Ginseng Radix (1.5 qian), for severe deficiency (2-3 qian), Cinnamon Twig (4 fen). After seven days, reduce Cinnamon Twig. Jujube (1 piece).
For excessive sweating with thirst, restore fluids and generate pulse. Add Ophiopogon Tuber (1 qian), Schisandra Berry (10 grains); for excessive sweating with difficult urination, due to insufficient Body Fluids, do not use diuretics. For Phlegm, do not use Pinellia Tuber or Fresh Ginger; only use Exocarpium Citri Reticulatae (4 fen).
Postpartum sudden exhaustion of Qi and Blood with profuse sweating, where complexion and form are depleted, is a critical condition. One cannot stick to routine methods. First, settle the painful mass, and pragmatically use Tingwei Zhihan Tang for two or three doses to save the crisis. Wait until the parturient has some spirit, then reduce Ginseng Radix and Astragalus Root to remove mass pain.
Tingwei Congquan Shenqi Formula (Pragmatic Ginseng-Astragalus Formula for Regulating Defense)
Astragalus Root (1.5 qian), Ginseng Radix (3-4 qian), Ephedra Root (1.5 qian), Dang Gui Root (1 qian), Honey-fried Licorice Root (5 fen), Saposhnikovia Root (2 fen), Cinnamon Twig (5 fen).
If sweating is little, remove [previous herbs], add Cinnamon and Ginseng; if mass is also not painful, use three Jujubes, decoct in water and take. Strictly forbid Pinellia Tuber and Ginger Juice; add Ophiopogon Tuber and Schisandra Berry.
For alternating chills and fever, do not use Bupleurum etc. For headache and fever, do not use Ephedra, Scutellaria, Coptis, or Phellodendron. Postpartum excessive sweating should be discussed as loss of Yin; if Yin is lost, Yang follows and is lost. Is this not dangerous! Urgently take the previous formula, and naturally peace will return.
Postpartum shortness of breath and spontaneous panting indicate Qi and Blood are not yet exhausted. Tonics can be slightly delayed; one must first take one or two doses of Shenghua Tang to move masses and settle pain, then add Ginseng Radix. If labor was severe, and there is massive bleeding, with depletion of form and color and急促 panting, this is indeed a critical condition. It is difficult to discuss mass pain; urgently add Ginseng Radix (3-4 qian) to Shenghua Tang to save the crisis. Take two doses continuously within a short time; delaying makes it hard to continue. If one dose per day, death will arrive immediately.

If for a long time eating little, vomiting upon smelling medicine, and mistakenly using cold substances, vomiting without accepting food, use Dushen Tang (Ginseng Radix 3-4 qian), Ginger (2 slices), and a pinch of rice, decoct in water. Decocting with scorched rice powder is also acceptable. Sweating, shortness of breath, and panting indicate severe deficiency without doubt; those who cannot accept tonification are difficult to treat.
Postpartum sweating, with stiff neck, clenched jaw, tight teeth, and muscle spasms, resembles Shanghan symptoms; strictly do not treat as Shanghan. The Nan Jing says: Excessive sweating leads to loss of Yin; if Yin is lost, Yang follows and is lost. Thus it is said: Excessive sweating leads to loss of Yang. Postpartum blood loss with excessive sweating is a critical condition of losing both Yin and Yang. Use Jiawei Shenghua Tang to treat postpartum excessive sweating.

Long-standing postpartum, continuous metrorrhagia, or bleeding like egg-sized clots, or shedding blood sheets. One should greatly tonify Spleen and Stomach, raise and lift Qi and Blood, and slightly add herbs for Heart Fire. Use Shengju Dabu Tang (Raising and Great Tonification Decoction).
