The eyes are neither painful nor itchy, yet vision gradually becomes blurred. The visual field is like a thin mist or light smoke that gradually thickens; sometimes one sees flying flowers or chaotic flies, and sometimes feels like silk floss floating in the void. How does this condition arise? It is due to heat and wind stagnating in the Liver [Liver]. Loud crying, fright, and fear cause "brain fat" [Brain Fat] to flow into the black pupil [Black Pupil]. Initially, only one eye becomes dim, which then sequentially affects the other until both are involved. Why must bitter medicine be endured as if it were unpleasant words? It is simply because the Qi of the Liver [Liver Qi] is not flowing smoothly. At that time, medication should resolve the condition; however, if nursing care is neglected or improper, efforts will be in vain. If the darkness persists for a long time and completely obscures the pupils, it is called "Internal Cataract" [Nei Zhang], blocking the double pupils. Based on their shapes, they are classified into sixteen types, which Master Longshi and the Sages can thoroughly analyze. A thousand miraculous medicines may fail to produce efficacy, but a single拨 (manipulation) with a golden needle can restore vision as bright as the sun in the sky. Exercise caution and follow the aforementioned methods for recuperation; otherwise, the previous disease will recur.
There are sixteen origins of internal cataracts; those who study medicine to benefit the world must examine them carefully. Clearly understanding each shape allows for safe acupuncture treatment. Applying the same needling method used for Round Opacity Cataract [Yuan Yi] would damage the Divine Light [Shen Guang] and make recovery difficult. One must differentiate between cold and heat, brightness, deficiency and excess, and harmonize the four limbs while waiting for full recovery. Otherwise, symptoms such as chest oppression, violation of dietary restrictions, vomiting, mental fatigue leading to opacity reversal, coughing, and shock may occur. For cases with significant fright, administer Zhenxin Wan [Calming the Heart Pill] first. If seeking cooling medicines such as Yin Gao [Silver Paste], observe the patient's constitution at the time. Old opacities require fine needles, while fresh recurrences differ; the needling technique cannot be uniform. For patients who are deficient, recently postpartum, or pregnant, one realizes after needling how difficult recuperation is. Needling should be performed on days without rain or wind, avoiding strong sunlight. Three days prior to needling, maintain a pure diet [Qing Qi]. Practice medicine with a calm and focused mind; avoid noise when chanting Buddha's name or interacting with relatives. The patient should sit cross-legged facing a bright area, tightening the belt to feel secure. The physician should walk the path of virtue and wisdom, guided by compassion. If bleeding occurs, do not panic; stop immediately, bandage as before, and inspect later. If sudden tremors or fright occur, the physician must reconsider. After thirty days of medication, clarity may return. If vision remains poor seven days after removing the bandage, and there is fluid movement resembling peanuts in the eye, do not speak further. Persistently take Huanjing Wan/San [Restore Vision Pill/Powder]; within one hundred days, clear vision will be restored.
After treating internal cataracts with golden needles, physicians must deeply consider their instructions. Use cotton-colored black beans, sized like small balls, placed on the eye and tied securely. When sleeping, ensure the head pillow is stable. Lie supine for three days without rushing. If slight pain occurs after bandaging, it is due to brain wind [Brain Wind] pulling; do not doubt this. Treatment may involve needling or cauterization according to classics. If pain is severe, apply hot compresses. To induce vomiting, hold and swallow juice from dried plums; lie supine afterward as instructed. Rising too soon risks injury from straining; though rare, this must be known. For seven days, consume warm soybean porridge. Avoid clenching teeth. Urinate and defecate slowly; do not stand up alone, but seek assistance. Loud shouting or excessive talking may disturb the eye wheel, causing snow-like floaters. Maintain this devotion for thirty days, gradually moving about and recognizing familiar people. Suppress frantic thoughts regarding yin-yang matters; sleep separately from spouses for one hundred days. Do not wash the face for one month; if the needle wound gets wet, slight pain may occur. Avoid five pungent meats [Wu Xing], alcohol, and wheat products for one year. Take medicine to eliminate the root of the disease.
