1. The eyes receive the essence of the five zang organs and six fu organs, shining brightly like the sun and moon in the sky, unobscurable. The right side corresponds to Yin, and the left to Yang. It contains light and nurtures radiance, akin to mulberry leaves bathing in the abyss. Alternating in operation. If Kan (Kidney/Water) and Li (Heart/Fire) lose their balance, causing misty vision, disordered meridians, and irregular changes, the people will suffer from diseases. Upon opening the book, one can see clearly; regarding the diseases governed by the five zang organs, although somewhat redundant, this manual was inherited from my great-grandfather, studied through generations for over thirty years, and further refined after consulting renowned masters, verifying theories, purchasing rare books and secret prescriptions, removing verbosity, and compiling concise content. Any valuable theoretical points are included.
2. The Five Wheels and Eight Palaces each have their specific functions, defined by form and meaning, with organ distributions arranged like chess pieces and stars, tightly interwoven. Laypeople only grasp the superficial aspects; here, the profound mysteries are fully explored. Symptoms are differentiated according to meridians, with appropriate tonification or sedation. First, observe changes in the contours, then investigate the source of the disease. The difference between main and guest, reversal and順 (shun), is minute yet critical.
3. Historical medical cases, starting with Hua Yuanhua of the Han Dynasty who stood out, Ni Zhongxian who collected great achievements, and the four major masters, along with works like the *Longmu Lun*, harmonize like celestial music. However, some effective treatments hide their flaws, while others offer unique insights distinct from common views, making truth and falsehood hard to distinguish; these are not recorded.
4. Treatment methods are categorized distinctly, balancing Yin and Yang transformations. Without examining the ruling Qi of the year (Si Tian), one cannot discern the extremes of the Six Qi and Five Movements; without observing pulse conditions, one cannot understand the Extraordinary Vessels or verify the effects of Seven Emotions and Six Evils. Although the "Tiger's Key" exists, distinguishing subtle signs is difficult. This manual assists classics in disseminating knowledge, following laws to examine causes, using *Yuanji Qiwei* as a standard, supplemented by various scholarly discussions, validating seasonal influences, and regulating climate-related issues. Its principles and items are interconnected like pearls and embroidery, aiming for precise solutions, and are detailed herein.
5. Using cold or hot medicines is like employing empty or full tactics in war; it requires firm judgment, unlike empty guessing or scratching an itch through a boot. One must identify the affected meridian, primary symptom location, cause origin, and timing. Whether warming or cooling, internal or external, consistency is key. Treating symptoms without addressing the root, applying cold when heat is needed, or heat when cold is needed, misusing dosages, and reversing acupuncture techniques rarely avoid immediate disaster. Caution is essential!
6. Decoctions, pills, powders, and other forms differ in usage and treatment approaches. Some suit pills, some powders, some water infusions, some pastes, and some fit multiple forms. Some cannot be taken in decoctions or wines, depending on herb properties. Decoctions cleanse and treat severe diseases; powders disperse and treat acute diseases; pills act slowly for gradual treatment. Dosages vary: if ground into powder, 7-8 qian is standard. For frail patients, 5-6 qian suffices; for robust ones, over 1 liang is needed. Too little lacks efficacy; too much overwhelms the body.
7. Preparation methods must be meticulous, and herbs must be sourced authentically. Using fake herbs wastes effort and fails to cure. Roasting involves heating whole herbs in charcoal, wrapped in dough or wet paper, until cracked (e.g., Fuzi, Nanxing). Honey-frying coats herbs in honey, ginger juice, butter, child's urine, or wine, then heats them until fragrant (e.g., Huangqi, Houpo). Calcination burns minerals like Muli, Shijueming, Shigao in fire, sometimes quenching with vinegar or urine. Baking uses paper barriers to dry herbs like Tandong, Maidong. Stir-frying uses silver or clay pots until crisp without burning. Refining honey fixes 1 jin to 12.5 liang. Fire control is crucial.
8. Enlightenment is like wiping dust from a mirror or removing firewood from under a pot. While everyone knows the value of mirrors, true experts who clear clouds to reveal the sun are rare. This manual, passed down through generations, is precious. It offers deep insights, accumulated over years, not mere boasting. It is worthy of recording.
9. Ancient eye treatments used acupuncture when herbs acted too slowly. Physicians must know meridians and acupoints precisely for instant results. Modern people fear acupuncture, leading to lax practice. However, skilled use yields rapid cures for chronic ailments, proving its significant value beyond minor assistance.
10. Many books exist, but few offer genuine insight into anatomy and pathology. This manual stands alone, clarifying essence through classical research, covering complex symptoms. It categorizes sections, analyzes deeply, determines wheels and palaces, observes colors and shapes. It provides clear guidance, more detailed than previous works, aiding those in crisis.
11. Formulating prescriptions based on symptoms follows established theories, not guesswork. Previous lists of 160 symptoms were excessive; 72 were too simple. This manual selects 108 key symptoms, simplifying complexity. It clarifies Yin-Yang, root-branch, internal-external, and deficiency-excess patterns. Diagnosis relies on shape, color, and pulse. Flexibility in application ensures effectiveness. Prescriptions match symptoms perfectly. True mastery lies in the practitioner. Scholars should reflect on this.
