The condition of canthal fistula (Le Jing) at the large canthus originates from true fire-toxin. It frequently discharges blood and fluid, accompanied by swelling and pain. Although initially undamaged, it will inevitably lead to tissue destruction eventually. The therapeutic key lies in replenishing the Kidney Qi and clearing Heart Fire.
This condition manifests as a fistula between the large canthus (medial canthus) and the small canthus (lateral canthus), frequently discharging blood with a purplish hue and halo. The pathology resides in the Heart region, caused by excess fire-toxin. Therefore, treatment requires tonifying the Kidney to drain the Heart. The appropriate prescription is:
Zaoshi Tang (Dampness-Drying Decoction)
Coptidis Rhizoma (Chuan Lianlian, stir-fried, 1 qian) Atractylodis Rhizoma (Cangzhu, processed with rice water) Atractylodis Macrocephalae Rhizoma (Baizhu, earth-fried) Citri Reticulatae Pericarpium (Chenpi, each 8 fen) Poria (Baifuling) Pinelliae Tuber (Banxia) Aurantii Fructus (Zhike) Gardeniae Semen (Zhiren, stir-fried until black, each 7 fen) Glycyrrhizae Radica et Rhizoma (Xigancao, 3 fen)
Crush these ingredients into pieces. Add two zhong of clear water, decoct until reduced to eight-tenths. Strain the dregs and take warm.
Wuhua Wan (Five-Flower Pill; treats Le Jing with pus discharge, wind-heat stagnation in the eyelids, accumulation of pus mixed with tears, constant salivation-like discharge. If untreated for a long time, it may lead to the prolapse or loss of the cornea/pupil.)
Antennariae Herba (Jinfecao, 2 liang) Amomi Fructus Rotundus (Sharen, stir-fried) Zanthoxyli Pericarpium (Chuanjiaopi, each 7 qian) Glycyrrhizae Radica et Rhizoma (Gancao, honey-fried, 4 qian) Chrysanthemi Flos (Baijuhua) Phellodendri Cortex (Huangbai, wine-processed) Lycii Fructus (Gouqizi, each 1.5 liang) Morinda Officinalis Radix (Bajitian, 8 qian)
Grind these into fine powder. Form pills using refined honey, sized like wu-tong seeds. Take 20 pills per dose, administered on an empty stomach with salted water or warm wine.
When ministerial fire circulates and injures the small canthus, bleeding occurs intermittently, accompanied by unbearable swelling and pain. One must prevent blood loss from damaging the Shen Gao (crystalline lens/intraocular structures), which would result in lifelong blindness.
This condition presents as a fistula between the small canthus and the large canthus, frequently discharging bright red blood and fluid. This pathology stems from the Heart collaterals and is a disorder of lower jiao fire pathogen moving horizontally. Treatment should involve tonifying the Kidney to suppress the fire. The appropriate prescriptions are:
Xieshi Tang (Dampness-Draining Decoction)
Plantaginis Semen (Cheqianzi) Scutellariae Radix (Huangqin) Akebiae Caulis (Mutong) Citri Reticulatae Pericarpium (Chenpi, each 1 qian) Lophatheri Herba (Dan Zhuye, 20 leaves) Poria (Fuling) Aurantii Fructus (Zhike) Gardeniae Semen (Zhiren, stir-fried until black) Schizonepetae Spica (Jingjisui) Atractylodis Rhizoma (Cangzhu, each 8 fen) Glycyrrhizae Radica et Rhizoma (Gancao, 3 fen)
Crush these ingredients into pieces. Add two zhong of clear water, decoct until reduced to eight-tenths. Strain the dregs and take warm.
Baiwei Wan
Anemarrhenae Rhizoma (Baiwei, 5 qian) Punicae Pericarpium (Shiliupi) Saposhnikoviae Radix (Fangfeng) Tribuli Fructus (Bailiji, pounded and thorns removed) Notopterygii Rhizoma et Radix (Qianghuo, each 3 qian)
Grind these into fine powder. Form pills using rice flour paste, sized like wu-tong seeds. Take 20 pills per dose, administered with plain boiled water.
Yiyin Shenqi Wan (See Volume II; add Qianghuo and Fangfeng to tonify Liver and Kidney deficiency.)
