Blood stasis in the Spleen with Fire. Heat sores arise on the upper eyelid. Excessive tearing, redness, and swelling. The sensation of sand rubbing is hardest to bear. Sometimes pain accompanies itching. Severe cases make it inconvenient to open the eyes. Disgusting are the ignorant and stubborn ones who rely entirely on bloodletting for efficacy.
The eyeball relies solely on Blood. If Blood is damaged, the eye loses luster. In mild cases, careful evacuation is required; if treatment is excessive, it causes injury. Red nodules between the eyelids indicate Jiaochuang (Phacochroma/Sand Eye) caused by Wind-Heat.
This condition arises inside the eyelid; those that are red and firm are Jiaochuang. With this condition, there is a gritty feeling making opening difficult, accompanied by excessive tearing and pain. People all call it Suchuang (Granular Eyelid), which is mistaken. Indeed, Suchuang also grows in the eyelid, but its color is yellow and soft, and it disperses easily. This one [Jiaochuang] is firm and hard to disperse. Common people use Dragon Whisker Grass or Rush Pith to pierce and bleed for effect, not knowing that the eye takes Blood as its nourishment. Damaged Blood leads to weakness in the visual brilliance. Mild cases should only be treated carefully. As for nodules connecting in patches, lumps uneven in height, and severe blood stasis, one must reluctantly employ drainage methods. Stop when the disease is addressed; do not go too far. Going too far damages Blood, fearing injury to True Water (Kidney Yin), making it difficult to nourish the Shen Gao (Shen Gao/Glassy Body). Generally, using a soothing compress method, if it recurs after subsiding, it is due to internal stasis. Only then can one gradually guide based on the illness. If at initial treatment, one does not lightly perform drainage, excessive treatment may cause harm. It is better to be cautious from the start. Suitable to take: Guizhi Honghua San (Cinnamon Twig and Red Flower Powder) (Treats eyelid swelling and hardness, internal lumps.) Danggui (Angelicae Sinensis Radix), Dahuang (Rhei Radix et Rhizoma), Zhizi Ren (Gardeniae Semen), Huangqin (Scutellariae Radix), Honghua (Carthami Flos) (All above washed with wine and slightly stir-fried), Chishaoyao (Paeoniae Radix Rubra), Gancao (Glycyrrhizae Radix et Rhizoma), Baizhi (Angelicae Dahuricae Radix), Fangfeng (Saposhnikoviae Radix), Shengdihuang (Rehmanniae Radix), Lianqiao (Forsythiae Fructus) (Equal parts). Grind into powder. Take three qian each time, far from meals, decocted in white water and taken orally.
The Spleen Meridian often has Damp-Heat. Qi stagnation causes slow Blood circulation. Suchuang arises inside the eyelid, granular like golden pearls. Looks like pus but pus does not come out. Gritty pain without cease. Eyelid tension,涩 (astringency/stiffness) when opening. Must prevent pathological changes. When the disease comes, it feels urgent, but disperses not for long.
This condition arises inside both eyelids; fine, yellow, and soft granules are Suchuang. Nowadays, people mistakenly call Jiaochuang "Suchuang." Indeed, Jiaochuang is red and firm; when present, the eyeball becomes stiff, gritty and inconvenient, but not yet urgent. If Suchuang appears, and there is eye pain and headache, there must be pathological changes. Suchuang is Damp-Heat郁 (stagnating) in the Earth component (Spleen), extremely heavy. But Jiaochuang emphasizes Wind-Heat. Although both conditions arise inside the eyelid and belong to the Blood aspect, Jiaochuang is red, firm, and easy to disperse; Suchuang is yellow, soft, and hard to disperse. Thus, treatments differ. How can they be generalized? Suitable to take: Chufeng Qingpi Yin (Wind-Expelling and Spleen-Clearing Decoction) Guangchenpi (Citri Reticulatae Pericarpium), Lianqiao (Forsythiae Fructus), Fangfeng (Saposhnikoviae Radix), Zhimu (Anemarrhenae Rhizoma), Yuanmingfen (Natrii Sulfas), Huangqin (Scutellariae Radix), Xuanshen (Scrophulariae Radix), Huanglian (Coptidis Rhizoma), Jingjiesui (Schizonepetae Spica), Dahuang (Rhei Radix et Rhizoma), Jiegeng (Platycodonis Radix), Shengdi (Rehmanniae Radix) (Equal parts). Cut into pieces. Add two zhong of white water, decoct until eight fen remains. Remove dregs. Take far from meals.
