Gout (Tongfeng) is a condition caused by the deficiency of healthy Qi, improper regulation, and invasion by pathogenic factors such as Wind, Cold, Summer-heat, and Dampness. The pathology lies within the four limbs, while the color of the flesh remains unchanged. The symptoms are mild during the day and severe at night. Pain described as cutting or stabbing indicates an excess of Cold; swelling and fullness feeling like being gouged out indicates an excess of Dampness. This disease may also arise from bathing in water immediately after sweating. If left untreated for a long time, it can lead to dislocation of bone joints and emaciation of the thighs and calves, which becomes difficult to treat. In my study of Gout, the pulse patterns vary greatly. Cases where symptoms stabilize in the morning but worsen at night, with a Wiry (Xian) and Tight (Jin) pulse, are classified as Gout. Cases with a Deep (Chen) and Hidden (Fu) pulse indicate Middle Qi deficiency (deficiency of Spleen/Stomach Qi or stagnation of Qi mechanism). Treatment cannot be uniform; clinical differentiation is essential. The Classics state: Bi Syndrome (Bi Zheng) results from the combination of Wind, Cold, and Dampness pathogens. Thus, it is called Painful Bi (Tong Bi), characterized by pulling pain in sinews and bones; Adherent Bi (Zhuo Bi), characterized by fixed pain; Wandering Bi (Xing Bi), characterized by migrating pain; and Generalized Bi (Zhou Bi), characterized by whole-body pain. These are all manifestations of excess pathogenic factors.
Regarding Gout: Pain in the waist, back, hands, feet, and joints is due to Blood deficiency and Qi weakness, leading to dry and obstructed meridians and stagnation of Cold. Worsening in the afternoon and night indicates weak Yin Blood. Canji Qinjiao Tang (Five-Ingredient Decoction with Qinjiao): Danggui (Radix Angelicae Sinensis, 3 qian), Chishao (Radix Paeoniae Rubra, stir-fried with wine, 0.7 qian), Cangzhu (Rhizoma Atractylodis Macrocephalae, soaked in children's urine, 1 qian), Shengdihuang (Radix Rehmanniae Preparata, soaked in wine, 1 qian), Bibi (Rhizoma Dioscoreae Hypoglaucae, 1 qian), Heigouji (Os Spinae Canis Nigri, de-haired root, 2 qian), Chuanxiong (Rhizoma Chuanxiong, 0.7 qian), Qianghuo (Rhizoma et Radix Notopterygii, 1.5 qian), Qinjiao (Radix Gentianae Macrophyllae, de-radicle, 1.5 qian), Chuan Duhuo (Radix Angelicae Pubescentis, 1 qian), Wujia Pi (Cortex Acanthopanacis, 2 qian), Huanglian (Rhizoma Coptidis, stir-fried with ginger juice, 1 qian), Huangbai (Cortex Phellodendri, stir-fried with wine, 1 qian), Honghua (Flos Carthami, washed with wine, 0.8 fen), Huangqin (Radix Scutellariae, stir-fried with wine, 1.5 qian), Huangqi (Radix Astragali, stir-fried with wine, 2 qian), Renshen (Radix Ginseng, 2 qian), Niuxi (Radix Achyranthis Bidentatae, de-radicle and soaked in wine, 1.5 qian), Duzhong (Cortex Eucommiae, processed with Fennel and salt, 2 qian), Sheng Gancao (Radix Glycyrrhizae Preparata, 0.2 fen). Cut the herbs, add seven branches of Peach Wood (each 1.5 cun long) and seven roots of Jiedixin (Herba Centellae Asiaticae), decoct with water. Before taking, add one cup of children's urine and one cup of good wine. Take warm on an empty stomach. Decoct the dregs again for a second dose. Avoid alcohol, wheat products, carp, and damp-heat generating meats like mutton and goose. If pain exacerbates predictably before rain or on cloudy days, add Fangfeng (Radix Saposhnikoviae), Tianma (Rhizoma Gastrodiae), and Shengma (Rhizoma Cimicifugae). For worsening in the afternoon/night due to Yin Blood deficiency, add Shengma (0.5 fen) and Mudanpi (Cortex Moutan, 1 qian). For worsening in the morning/midday due to Qi stagnation and Yang weakness, add Lianqiao (Fructus Forsythiae), Chenxiang (Lignum Aquilariae Resinatum), Zhuli (Succus Bambusae), and Ru Zhi (Lactis Humanus). For severe pain, double the dosage of Qianghuo, Honghua, and Huangqin (stir-fried with wine). Cooling the blood stops the pain. This condition involves sinews and bones; patients often suffer from long-term external pathogen invasion or age-related decline, resulting from poor self-care. Sinews belong to Liver Blood, and bones belong to Kidney Water; this is internal damage.
Regarding pain caused by Damp-Heat: Cangzhu (Rhizoma Atractylodis Macrocephalae) excels at drying Dampness, and Huangbai (Cortex Phellodendri) excels at clearing Heat. Both possess potent properties and constitute a simple formula. Adding Niuxi treats Damp-Heat flowing downward, causing numbness in both legs or burning heat sensations. Ermiao San (Two Marvels Powder): Cangzhu (soaked in rice wash water), Huangbai (soaked thoroughly in human milk). Grind into powder. Take 3 qian per dose, mixed with wine. For severe pain, add fresh Ginger Juice and take hot.
Regarding stagnation of Blood vessels, contraction of sinews, joint pain, and difficulty walking: Invigorating Blood and regulating Qi is the primary method. Shujin San (Sinew-Relaxing Powder): Yanhusuo (Rhizoma Corydalis, stir-fried), Danggui (Radix Angelicae Sinensis), Lagui (Cortex Cinnamomi, equal parts). Grind into pills. Take 2 qian per dose, mixed with wine. Shiquan Dabu Tang (Ten Complete Tonifying Decoction): Treats whole-body pain due to fatigue and overexertion; adding Banxia (Rhizoma Pinelliae, prepared with ginger) improves efficacy.
A woman over seventy suffered from whole-body pain, especially in sinews and bones, unable to flex or extend. She experienced itching all over like insects crawling, dry mouth, red eyes, dizziness, phlegm surging, chest oppression, short dark urine, worsening at night. She was treated with Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) modified with Zhizi (Fructus Gardeniae) and Chaihu (Radix Bupleuri), decocted in water.
Regarding pain caused by Stasis of Blood and Phlegm-Damp accumulating between joints, at the convergence of sinews and bones, and in cavities: Heavy joints indicate Phlegm-Damp; worsening at night indicates Blood Stasis. Gantong Tang (Pain-Chasing Decoction): Ruxiang (Resina Draconis), Moyao (Resina Olibani), Dilong (Pheretima, stir-fried with wine), Xiangfu (Rhizoma Cyperi), Taoren (Semen Persicae), Honghua (Flos Carthami), Gancao Jie (Node of Radix Glycyrrhizae Preparata), Niuxi (Radix Achyranthis Bidentatae, soaked in wine), Danggui (Radix Angelicae Sinensis), Qianghuo (Rhizoma et Radix Notopterygii), Wulingzhi (Faeces Trogopterori, washed with wine to remove soil). Cut the herbs, decoct with water, take warm.
Regarding Cold-Damp obstructing joints, causing numbness and pain: Xuduan Wan (Drynaria Pill): Huangqi (Radix Astragali, 1 liang), Renshen (Radix Ginseng, 7 qian), Baifuling (Poria, 1 liang), Shanzhuyu (Fructus Corni Officinalis, 1 liang), Yiyiren (Semen Coicis, 1 liang), Xuduan (Radix Dipsaci, 1 liang), Fangfeng (Radix Saposhnikoviae, 7 qian), Guixin (Cortex Cinnamomi Cassiae, 1 liang), Shanyao (Rhizoma Dioscoreae Oppositae, 1 liang), Baizhu (Rhizoma Atractylodis Macrocephalae, 7 qian), Shu Dihuang (Radix Rehmanniae Preparata, 2 liang), Mudanpi (Cortex Moutan, 1 liang), Maimendong (Radix Ophiopogonis, 1 liang), Shihu (Caulis Dendrobii, 1 liang), Lu Jiao Jiao (Colla Cornus Cervi). Grind into fine powder, make pills with refined honey, size of Sophora seeds. Take 50 pills per dose, warm wine on an empty stomach.
Regarding inability to flex/extend hands and feet, whole-body pain: Xiaofeng Yin (Wind-Dispersing Decoction): Tested successfully by Xu Peihong of Linchuan. Chenpi (Pericarpium Citri Reticulatae), Baizhu (Rhizoma Atractylodis Macrocephalae, de-radicle), Danggui (Radix Angelicae Sinensis, washed with wine), Baifuling (Poria, peeled, 1 qian each), Fangji (Radix Stephaniae Tetrandrae), Duhuo (Radix Angelicae Pubescentis), Mugua (Fructus Chaenomelis, 0.4 fen), Qinjiao (Radix Gentianae Macrophyllae, 0.3 fen each), Fangfeng (Radix Saposhnikoviae, 0.5 fen). Cut one dose, decoct with fresh ginger, take anytime. For Qi deficiency, add Renshen (0.8 fen).
Regarding all types of whole-body joint pain or migratory unbearable pain: Ren Shen (Radix Ginseng), Baizhu (Rhizoma Atractylodis Macrocephalae, de-radicle), Baifuling (Poria), Danggui (Radix Angelicae Sinensis), Chuanxiong (Rhizoma Chuanxiong), Tianma (Rhizoma Gastrodiae), Chenpi (Pericarpium Citri Reticulatae), Gancao (Radix Glycyrrhizae Preparata), Chishao (Radix Paeoniae Rubra), Fangfeng (Radix Saposhnikoviae), Sheng Dihuang (Radix Rehmanniae, 1 qian), Qianghuo (Rhizoma et Radix Notopterygii), Duhuo (Radix Angelicae Pubescentis), Nanxing (Rhizoma Arisaematis Cum Bile), Huangqin (Radix Scutellariae). Cut, decoct with fresh ginger, take.
Regarding Wind-Damp battling, causing whole-body pain: Treat with Yiqi Tang (Qi-Supplementing Decoction) plus Qianghuo, Fangfeng, Gaoben (Rhizoma Ligustici Chuanxiong), and Cangzhu. Once the disease resolves, do not continue using Wind-dispersing herbs, as they damage Healthy Qi and worsen the condition.
A man suffered unbearable pain in both hips, tested successfully by Xu Kuowu of Linchuan. Huangqi (Radix Astragali, honey-fried, 1 liang), Renshen (Radix Ginseng, 2 qian), Cangzhu (Rhizoma Atractylodis Macrocephalae, rice-wash processed, 1.5 qian), Danggui Shen (Radix Angelicae Sinensis, washed with wine, 1.5 liang), Qinjiao (Radix Gentianae Macrophyllae, 1 liang), Niuxi (Radix Achyranthis Bidentatae, de-radicle and washed with wine, 1 liang), Duhuo (Radix Angelicae Pubescentis, 1 liang), Duzhong (Cortex Eucommiae, stir-fried with wine, 1.5 liang), Shu Dihuang (Radix Rehmanniae Preparata, 1 liang), Sangjisheng (Ramulus Taxilli, 1.5 liang), Guanzi Gui (Cortex Cinnamomi Cassiae, 2 qian), Mugua (Fructus Chaenomelis, 5 qian), Xiaohuixiang (Fructus Foeniculi, stir-fried with salt and wine, 5 qian). Grind into powder, roll in paper to finger thickness. Place seven layers of straw paper on the affected area, ignite the medicine, and quench it. Stop when pain is felt.
Regarding Ironing Therapy (Yun Fa): Treats various Wind toxins, Cold-Bi numbness/swelling/pain, or whole-body bone pain. Effective immediately upon ironing. Cangzhu (Rhizoma Atractylodis Macrocephalae, 2 liang), Qianghuo (Rhizoma et Radix Notopterygii, 1 liang), Duhuo (Radix Angelicae Pubescentis, 5 qian), Shechuangzi (Fructus Cnidii, 5 qian), Manjingzi (Fructus Viticis Negundo, 5 qian), Chuanshanjia (Squama Manis, earth-stir-fried, 5 qian), Xionghuang (Realgar, 3 qian), Liuhuang (Sulfur, 3 qian), Shexiang (Moschus, 0.3 fen). One patient experienced limb pain upon exertion. Using Phlegm-clearing and Qi-regulating formulas made the pain constant even without exertion. Adding Damp-draining herbs led to arm swelling and pain, fatigue, internal heat, night sweats, floating pulse that is fine upon pressure. This indicates Yang Qi deficiency and Cold. Treated with Buzhong Yiqi Tang (Middle-Supplementing Qi-Boosting Decoction) plus Fuzi (Radix Aconiti Lateralis Preparata, 1 qian) and Renshen (Radix Ginseng, 5 qian); swelling and pain resolved. Subsequently cured with over a hundred doses of Shiquan Dabu Tang (Ten Complete Tonifying Decoction).
One patient had a robust physique, soft/sore sinews and bones, excessive phlegm, thirst preferring cold drinks. Using Yufeng Tang (Wind-Curing Decoction) and Tianma Wan (Gastrodia Pill) increased Phlegm-Heat. Taking Niuhuang Qingxin Wan (Calves' Gallbladder Heart-Clearing Pill) worsened limb paralysis. I diagnosed Spleen and Kidney deficiency. Treated with Buzhong Yiqi Tang (Middle-Supplementing Qi-Boosting Decoction) and Jiaban Bawei Wan (Modified Eight-Ingredient Pill). Cured after three months. Lifespan reached seventy.
Treating Baihu Lijie Feng (White Tiger Joint-Wandering Wind): Migratory pain, hot swollen knees.
