Heart and abdominal pain that is severe and unrelenting. A pulse that is fine, small, and slow indicates survival (favorable prognosis). A pulse that is large and rapid indicates death (unfavorable prognosis).
Abdominal pain is often caused by cold pathogenic qi invading the middle jiao (middle burner), disturbing the spleen and stomach, leading to pain. It may also be caused by accumulated food stagnation remaining in the intestines and stomach; by qi binding and failing to disperse; by pain accompanied by vomiting; by pain accompanied by diarrhea; by pain accompanied by constipation; by heat-type pain; by deficiency-type pain; by excess-type pain; by damp-phlegm type pain; by blood stasis type pain; or by parasite type pain. The conditions vary greatly. Treatment must distinguish between cold, heat, deficiency, and excess, and therapy should be administered according to the specific syndrome obtained. If caused by external pathogens, use dispersing methods; if caused by internal accumulation, use purging methods. For cold, use warming methods; for heat, use clearing methods; for deficiency, use tonifying methods; for excess, use draining methods. For diarrhea, regulate it; for obstruction/constipation, unblock it. For blood stasis, resolve it; for qi stagnation, soothe it. For parasites, expel them; for accumulation, dissipate it. Additionally, strengthen and regulate the spleen and stomach, and nourish and regulate qi and blood. This is the essence of treatment.
Section 1 discusses abdominal pain arising from external contraction of wind-cold, internal injury from diet, and excessive emotional anger/frustration at the initial stage. Use Xingqi Xiangsu San (Qi-Regulating Fragrant Perilla Powder). (Formula see "Dietary Injury" section)
Section 2 discusses continuous, lingering abdominal pain with cold extremities due to external contraction of cold pathogenic qi and internal injury from cold substances. Use Wuji San (Five Accumulations Powder). (Formula see "Middle Cold" section)
Section 3 discusses abdominal pain regardless of cold or heat, food stagnation or qi stagnation; this is the general formula for all types of abdominal pain.

Grind these ingredients into powder. Form pills the size of millet seeds using rice paste. Take 0.5 qian per dose, well after meals, with plain water. "Dry pain" refers to pain without vomiting or diarrhea. "Intermittent pain" refers to pain occurring when hungry after eating bland food, but not occurring when full after eating rich food. The classics state: Dry pain inside the abdomen is due to parasites within the abdomen. This is what is meant.
A child suffered from abdominal pain, appearing near death. Palpation of the abdomen revealed masses of varying sizes. Various physicians failed to treat him. I instructed someone to slowly rub and knead the painful area with their hands. After half a day, the parasites were expelled via stool, and he recovered.
A woman suffered from abdominal pain like needle pricks, dying with each episode, afraid to let anyone touch her. Her six pulses were surging and rapid. This was intestinal abscess toxicity. Use Shan Jianjia (Pangolin Scales), Chao Baizhi, Beimu, Jiangcan, and Dahuang. Combine into one large dose, decoct in water and take. Pus and blood discharged from the urine indicate recovery.
Official Li suffered frequently from abdominal pain. He was treated with Buzhong Yiqi Tang plus stir-fried Shanzhi, and recovered.
Prefect Zhu suffered from abdominal pain and diarrhea due to anger. Sometimes his hypochondrium felt distended, sometimes his chest and breasts hurt, sometimes he had alternating chills and fever, sometimes difficult urination, loss of appetite, inability to lie down, vomiting of phlegm foam, and unclear mental thoughts. This was Liver Wood overacting on Spleen Earth. He was treated with Chaihu plus Shanzhi, Paojiang, Fuling, Chenpi, and Zhi Huanglian. He recovered after one dose. (Zhi Huanglian is prepared by mixing equal parts Huanglian and Wuzhuyu with salt water, letting them sit for two to three days, stir-frying together until scorched, and then using only the Huanglian.)
A person had internal food stagnation and external contraction of wind-cold, presenting with headache, fever, nausea, and abdominal pain. I prescribed Huoxiang Zhengqi San plus Xiangfu and Chuanxiong. Symptoms ceased after one dose. The next day, previous symptoms resolved completely, except for persistent abdominal pain, which subsided slightly upon heavy manual pressure. This was guest cold taking advantage of deficiency. He was treated with Xiangsha Liujun Zi Tang plus Muxiang and Paojiang. Upon waking from sleep, pain reduced by sixty to seventy percent. Muxiang and Sharen were removed, and he took the remainder, recovering fully.
A woman suffered from umbilical and abdominal pain, losing consciousness. One single dose stopped it immediately. Those unaware claimed it was Heart Qi pain, which was incorrect. I used Baizhi, Wulingzhi, and Mutong (peeled), in equal parts. Take 5 qian per dose, decoct in half vinegar and half water until reduced to 0.7 portion, and take.
Section 12 discusses Baishao (White Peony Root), which is sour and slightly cold, a herb that tonifies the middle jiao. Assisted by Zhi Gancao (Honey-fried Licorice), it is the supreme medicine for treating abdominal pain. In summer heat-induced abdominal pain, add a small amount of Huangqin. For cold-induced abdominal pain with aversion to cold, add only a small amount of Rougui (1 qian), Baishao (3 qian), and Gancao (1.5 qian). These three herbs are the divine formula for treating cold abdominal pain.
This is Zhang Zhongjing's divine formula. For autumn abdominal pain, add Guizhi (3 qian). For winter extreme cold with cold abdominal pain, add Guizhi (1.5 qian), decoct in two portions of water, and take.
