Liu Zhu (Flowing Pearl) pulse presents as only a single red spot. Huan Zhu (Ringed Pearl) pulse is slightly larger in shape. Chang Zhu (Long Pearl) pulse is round and elongated. The above do not refer to circular patterns; generally, they describe how the red pulse traverses Qi in these specific forms. Lai She (Incoming Snake) pulse is essentially the Chang Zhu pulse spreading out and lengthening, with one end thick and the other sharp; Qu She (Outgoing Snake) pulse is similar. This distinguishes their upper and lower orientations, hence termed "Incoming and Outgoing." Jiao Gong Fan Zhang (Opisthotonos-like) pulse resembles an arched bow; curvature inward indicates a favorable prognosis, while curvature outward indicates an unfavorable one. Qiang Xing (Spear-shaped) pulse rises straight upward; Yu Gu (Fish Bone) pulse branches like fish bones; Shui Zi (Water Character) pulse consists of three parallel lines; Zhen Xing (Needle-shaped) pulse crosses the Guan position by approximately one or two grains of rice. She Jia Ming Mai (Shooting Nail Life Pulse) shoots outward, while Tou Zhi Ming Mai (Piercing Finger Life Pulse) curves inward. Among the forty-nine positions, there are distinctions of severity, ranging from subtle to pronounced, requiring careful deliberation. There are five colors: yellow, red, purple, cyan, and black. When the disease intensifies, the color changes. Excessive yellow becomes Yue Huang (Exceeding Yellow) and reddish-yellow; excessive red turns purple, presenting a red-purple hue; excessive purple turns cyan, showing a cyan-purple shade; excessive cyan turns black, displaying a cyan-black tint. Pure black color indicates an incurable condition.
Purple indicates Shang Han (Cold Damage); cyan indicates Jing Feng (Wind-Stroke/Convulsion); white indicates Gan (Infantile Malnutrition); black indicates Zhong E (Acute Poisoning/Malignant Qi); yellow indicates Pi Xu (Spleen Deficiency/Fatigue).
One must first distinguish the infant's nature; male left and female right must be clearly recognized. If the five fingers are cold as ice, this indicates severe onset of Jing Feng (Convulsion). If the five fingers and the heart region are hot as fire, this indicates Ji Shi Shang Feng (Food Retention with Wind-Cold) or Wind-Cold pathogenic factors. Heat in the index and middle fingers indicates Wind-Cold; coldness in these fingers indicates Tu Xie (Vomiting and Diarrhea). Heat in the middle finger indicates Shang Han (Cold Damage); coldness requires differentiation between Ma Dou (Smallpox/Measles). Heat in the index finger indicates upper body fever; coldness indicates upper jiao stagnation. Heat in the middle and ring fingers indicates combined Jing Feng (Convulsion); coldness indicates Shang Shi (Food Injury).
An octagonal pattern at the Fontanelle indicates a critical illness. If Jing Feng (Convulsion) penetrates the Three Passes, life expectancy is short. Initial invasion into the First Pass indicates mild convulsion, which can be prevented; invasion into the Second Pass can also be guarded against. Red and hot veins indicate food stagnation; distinct cyan veins indicate injury by Water-Wind; black veins indicate Cold-Wind; purple veins indicate Yin-Yang imbalance. Equal amounts of cold and heat with red and white colors; red veins definitely indicate Heat, suitable for cooling methods. White veins should not appear in severe diseases; white veins indicate deep cold, yet rescue is possible. Veins connecting to the thumb indicate Yin-Cold syndrome; vein patterns resembling flowers indicate misfortune. A Xuan Zhen (Hanging Needle) mark on the vein indicates vomiting and diarrhea; veins extending beyond the Guan position indicate imminent death. Paralysis and cold limbs with abdominal distension, vomiting and diarrhea often due to milk and food injury. Yu Kou (Fish Mouth) and Ya Sheng (Crow Cry) result from急促 breathing; Quan Fei (Dog Bark) and Ren He (Human Roar) result from fright-induced mania. Lan Bing (Bladder Strangury) is difficult to identify; a change at Tian Xin (Heavenly Heart) point relates to bladder issues. Kou Yi (Mouth Twitching), Xin Yue (Heart Regurgitation) accompanied by Qi Hou (Gasping), cold fingers, and coma indicate fatal prognosis. Panting and急促 breathing in the mouth, eye rolling, and hand spasms can be alleviated by tuina. Dry nose, black mouth, visible vein shadows, yellow teeth, pale lips, and bright eyes. Changes in voice and breath without facial changes, dancing hands and feet, and delirious speech. Frantic grasping like chicken claws, fixed gaze like a sheep's eyes. Gan (Malnutrition) treatment involves moxibustion on upper and lower areas; generally, transverse wrinkles indicate recovery. Distinguishing high and low at Tian Xin (Heavenly Heart) point requires careful attention to Yin and Yang. If such infants have poor vein conditions, life will pass by the road to Nan Ke (Dream/Illusion). Early tuina is essential for children's illnesses; delay leads to certain death. Severe cases may require Deng Xin Cao (Juncus Effusus/Stem of Rush) for diagnosis/treatment; mild cases still require skilled manual techniques. These true methods left by immortals, if followed by later learners, will bring fame and renown.
