(Attached: Cough after Shanghan)
*Chao Shi Bing Yuan* (Source of Diseases by Chao Yuanfang) on the syndrome of pediatric cough after Shanghan: Shanghan is caused by cold pathogenic factors invading the skin, struggling with blood and qi, causing the couli (skin pores and texture) to become closed and dense, preventing the free discharge of qi. The stagnation generates heat. Consequently, headache, body pain, and high fever occur. The reason for coughing is that the pathogen is in the Lung. The Lung governs the skin and hair externally and controls Qi. The Shanghan pathogen first invades the skin and follows the Qi into the Lung, thus severe coughing may be accompanied by pus and blood.
*Chao Shi Bing Yuan*: If a child continues to cough after recovering from Shanghan, it is because the pathogenic factor remains lodged in the Lung and has not been fully cleared. Cold pathogens injure the body, first invading the skin and hair. Skin and hair are the external manifestations of the Lung; the Lung governs Qi. Cold struggles with Qi and enters the five Zang and six Fu organs, resulting in heat in both the exterior and interior. After the heat subsides, the Lung Qi has not yet harmonized, and the pathogen remains. The pathogen follows the Qi into the Lung and struggles with the Lung Qi, hence the persistent cough after Shanghan.
*Huo Ren Shu* (Book of Saving Lives) discusses the treatment methods for pediatric and adult Shanghan cough: Generally, only a smaller dosage is needed, using herbs with slightly cooler properties. Question: Why does coughing occur? There are two types of Shanghan cough. One is Taiyang syndrome cough, treated with Xiao Qing Long Tang (Minor Blue Dragon Decoction) and Xiao Chai Hu Tang (Minor Bupleurum Decoction). The other is Shaoyin syndrome cough, treated with Zhen Wu Tang (True Warrior Decoction), Si Ni San (Frigid Extremities Powder), and Zhu Ling Tang (Polyporus Decoction). Generally, if there is heat in the Upper Jiao, the person will drink water. If water stagnates under the heart (epigastrium), the Lung floats upward. The Lung governs coughing; when water qi attacks it, coughing and slight wheezing occur. Zhang Zhongjing stated: "If the exterior syndrome of Shanghan is not resolved and there is water retention under the heart, causing dry heaving, fever, and cough, treat with Xiao Qing Long Tang." (If urination is difficult and the lower abdomen is full, remove Ma Huang and add Fu Ling.) For alternating chills and fever, chest and hypochondriac fullness/pain or cough, treat with Xiao Chai Hu Tang. (Remove Ren Shen and Da Zao from Xiao Chai Hu Tang, add Wu Wei Zi and Gan Jiang.) If it is Shaoyin syndrome cough, with heavy and painful limbs, difficult urination, spontaneous diarrhea, and cough, treat with Zhen Yu Tang. (Add Wu Wei Zi and Gan Jiang to Zhen Wu Tang. Generally, water qi in Shanghan is due to excessive drinking. Ancients treated water qi-induced cough: if the disease is in Yang channels, use Xiao Qing Long Tang; if in Yin channels, use Zhen Wu Tang.) For cold extremities, abdominal pain, or dysentery with cough, treat with Si Ni San. (Add Wu Wei Zi and Gan Jiang to Si Ni San.) *Shi Bi Jing* (Stone Wall Classic) describes thirty-six types of internal Shanghan cough syndromes: Cyan lips (the 48th syndrome mentions white lips), purple nose, continuous coughing sound, toxic phlegm nourished in the Lung and Stomach. Night cough with high chest and abdominal distension, sweat collection with marrow qi contraction making heat hard to dissipate. Food regurgitation, heavy speech, frequent coughing—how can one recover? Conducting wind-releasing sweating is the best method, ensuring wind removal and cutting off the root source.
Internal heat accumulation, transmitted by thieving wind, causes cold and heat to interact, generating cough. Over time, the chest becomes high like an inverted cup. Abdominal distension and heavy voice require sweating and wind-dispelling first, followed by regulating Qi transformation, resolving phlegm, and benefiting the diaphragm. If abdominal organs are knotted, purging will lead to recovery.
*Feng Sui Jing* (Phoenix Marrow Classic) uses the same verse for this syndrome. It notes: (Use Pi Jian San, see the section on Shanghan with convulsions. Next, use Xing Ren Gao, see the section on chronic cough.)
*Xiao Er Xing Zheng* (Pediatric Manifestations) discusses the song for the 48th syndrome of Shanghan cough, identical to above. It adds: This is Shanghan transmitting to cause vomiting and cough.
