（寒热疟瘴）凡二十二门
《巢氏病源》小儿痰候∶痰者，水饮停积胸膈之间，结聚痰也。小儿饮乳，因冷热不调，停积胸膈之间，结
聚成痰。痰多，则令儿饮乳不下，吐涎沫，变结而微壮热也。痰实壮热不止，则发惊痫。
《千金》治小儿痰实结聚，宿癖羸露，不能饮食。真珠丸方
真珠（半两） 麦门冬（去心，一两） 蕤仁（去皮，二百枚） 巴豆（去皮膜，四十粒）
上四味末之，蜜丸。期岁儿，服二丸，如小豆大；二百日儿，服如麻子二丸，渐增，以知为度。当下病赤、
黄、白、黑葵汁，纵下勿绝药，病尽下自止。久服使小儿肥白，以试验。
《千金》治八岁以上儿，热结痰实，不能食，自下方。
芍药 栀子 知母 大黄（各二两） 柴胡（二两六铢） 升麻 黄连 黄芩（各二两半） 竹
叶（切，一升半） 桔梗（一两半） 细辛（十五铢）
上十一味 咀，以水六升，煮取一升八合，去滓，分四服。十岁儿为三服。《外台》有枳实、杏仁各一两半，
而无桔梗、黄连。
《千金》治十五以下儿，热结多痰，食欲减，自下方。

两半） 生姜（十八铢） 杏仁（汤浸去皮尖、二两）
竹叶（切，一升半）
上十一味， 咀，以水六升半，煮取二升，十岁至十五岁者，分三服。
《经验后方》治大人、小儿痰实，久患风痫，缠喉风，咳嗽，遍身风疹，急中涎潮等。此药不大吐逆，只出涎水方。
瓜蒂（不限多少）
上细碾为末，壮年一字；十五以下气怯小儿半字。早晨井华水下一食，须含沙糖一块；良久，涎如水出，
年深涎尽，有一块如涎布水上如鉴矣。涎尽食粥一、二日，如吐多困甚，即咽麝香汤一盏即止矣。麝细研，
温水调下。昔大平尚书觉昏眩，即服之，取涎有效。
《圣惠》治小儿痰实壮热，心胸壅闷，不欲乳食。前胡散方
前胡（半两，去芦头） 枳壳（麸炒微黄，去瓤） 赤茯苓 川大黄（锉碎，微炒） 甘草（炙微赤，锉。各一分）

上件药捣，粗罗为散。每服一钱，以水一小盏，入生姜少许，煎至五分，去滓。温服，日三、四服。量儿
大小以意加减。

《圣惠》治小儿五、六岁痰实不散，宜服此方。

上件药捣，粗罗为散。每服二钱，以水一小盏，入生姜如莲子大，煎五分，去滓。量儿大小分减温服。

川芒硝 川大黄（锉碎，微炒） 代赭石（各半两） 半夏（汤浸七遍去滑） 甘遂（微炒。
各一分） 杏仁（十粒，汤浸，去皮尖、双仁，麸炒微黄）
上件药捣，罗为末，炼蜜和丸如绿豆大。空心以温水下两丸。量大小加减与服之。
《灵苑》治小儿痰实结滞，时发寒热，胸中涎壅及哮呷喘急，烦躁不得睡眠。犀角丸方
犀角（一钱，醋末） 白术 桔梗 陈橘皮（各一钱） 金银箔（各用三片，以水银一
钱，结成砂子） 巴豆（三粒，去皮，以枣子一个裹之烧令香熟，只取巴豆细研）
上件七味，同研令匀，以炼蜜为丸如小豆大。每服一丸至二丸，用薄荷水研下。量儿大小，临时加减丸数。
《婴孺》治小儿膈上有痰饮。疟候方
常山（五分） 知母（四分） 松萝 甘草（炙。各三分）
上水一升，浸一宿，煮五沸，去滓。空心服三合，当出痰为度。
张涣香橘皮丹 消小儿宿食痰滞。
陈橘皮（去白，焙干） 木香（各一两） 白术（炮） 草豆蔻（面裹微炮） 牵牛子 姜黄（各半两）
上件捣，罗为细末，滴水和丸如黍米大。每服十粒，煎葱白汤下。大小便涩或不通，即乳食前服之。
张涣枳壳汤 治小儿痰实，壮热不除。
枳壳（麸炒，去瓤） 半夏（汤洗七遍，焙干） 木香 前胡（各一两） 干姜 甘草（炙。各半两）
上件捣，罗为细末。每服一钱，水一小盏，入生姜三片、陈橘皮少许，同煎至六分去滓，放温服。
张涣前胡半夏丹 治小儿痰实壮热。

上件捣，罗为细末，取生姜汁和丸如黍米大。每服十粒，煎人参汤下。
张涣神曲汤 治小儿痰实。
神曲（微炒） 姜黄 木香 半夏（用生姜八两，切作片子，同捣成膏，慢火焙，炙令黄。各
一两） 甘草（炙） 青橘皮（炒黄） 白茯苓（各半两）
上件捣，罗为细末。每服半钱，入盐少许，沸汤点，放温热服。
张涣白金丹 消小儿痰实，利胸膈。
前胡 桑白皮（锉） 半夏（汤泡七遍） 白术（炮。各一两） 人参（去芦头） 陈橘皮（各半
两） 甘遂（一分，微炒）
上件捣，罗为细末，炼蜜和丸如黍米同大。温水下五粒至七粒；周 小儿并二、三岁只可服三粒。以上临
时加减。
《保生信效方》逍遥丸 治膈实气痞，痰盛喘促。
半夏（汤浸洗七次，焙，二两） 枳实（去瓤，麸炒） 槟榔（锉） 赤茯苓（去粗皮。各一两）
上同为细末，生姜自然汁煮面糊和丸如绿豆大。每服二、三十丸，荆芥汤送下。别作小丸与小儿。
《吴氏家传》治大人、小儿风壅，咽喉不利，痰实烦渴，困倦头昏；或发潮热，及一切风痰疮疥，并宜服之。
龙脑 薄荷叶（去沙土，用十两） 栝蒌根（生用一两） 荆芥穗（生用四两） 甘草（生用五两
一分） 缩砂仁（生用三两）
上件为细末，每四两药末入炒盐末一两，研匀，以瓷器盛贮。每服一钱，如茶点吃。
《吉氏家传》治小儿生百日以来痰实。

上水四合，煎取一合，去滓。分两日与服。

天南星 半夏 猪牙皂角 巴豆 白矾
上件等分，入瓦罐子，用火 ，存性，每用半钱。齑汁调下，或麦门冬、桑白皮汤入蜜下。
长沙医者郑愈传治 小儿咽喉涎鸣如锯，兼伤寒身热面赤，一切涎等。坏涎丸方
半夏（二钱，研，以生姜自然汁搜作饼子，用慢火炙黄干） 粉霜 铅白霜 巴豆霜 雄黄 蝎梢（各半钱）
上各为末，再研令匀，稀曲糊为丸如此○大。取涎每服五丸，用灯心汤化破。如涎未下，再用灯心汤投下
即吐。如取涎连三服即泻，补之。
（寒热疟瘴）共二十二门
《诸病源候论》中关于小儿痰证的论述：所谓痰，是水饮停滞积聚在胸膈之间，凝结而成的病理产物。小儿在喝奶时，如果冷热调节不当，导致水饮停积在胸膈之间，凝结成痰。痰多时，会导致婴儿无法顺利吃奶，吐出涎沫，身体逐渐结聚并出现轻微发热。如果痰实导致壮热持续不退，就会引发惊痫。
《千金方》治疗小儿痰实结聚、宿食癖块、身体消瘦虚弱、不能进食的方剂：真珠丸方
真珠（半两） 麦门冬（去心，一两） 蕤仁（去皮，二百枚） 巴豆（去皮膜，四十粒）
以上四味药研成末，用蜂蜜制成丸药。一岁的幼儿，服用两丸，大小如小豆；二百天大的婴儿，服用两丸，大小如麻子，逐渐增加剂量，以见效为限度。服药后应排出赤、黄、白、黑等颜色的脓血样物质，即使腹泻也不要立即停药，直到病邪排尽自然停止。长期服用可使小儿身体肥白，可作为验证疗效的依据。
《千金方》治疗八岁以上儿童，因热结导致的痰实、不能进食，自下方（此处指作者推荐的方剂或特定版本方剂）：
芍药 栀子 知母 大黄（各二两） 柴胡（二两六铢） 升麻 黄连 黄芩（各二两半） 竹叶（切碎，一升半） 桔梗（一两半） 细辛（十五铢）
以上十一味药切碎，用水六升，煮取一升八合，滤去药渣，分四次服用。十岁儿童分三次服用。《外台秘要》中有枳实、杏仁各一两半，但没有桔梗、黄连。
《千金方》治疗十五岁以下儿童，因热结导致痰多、食欲减退，自下方：

瓜蒂（不限多少）
以上细碾为末，成年男子服“一字”量；十五岁以下气弱的儿童服“半字”量。早晨用井华水送服一次，必须含一块沙糖；过一会儿，涎水像水一样流出，年深日久的涎水排尽后，会有一块状如涎布的东西浮在水面上，如同镜子一般清晰。涎水排尽后，吃一两天粥。如果吐得太多感到极度困倦，就喝一盏麝香汤即可止住。麝香细研，温水调下。从前太平尚书觉昏眩时，服用此药，取涎效果显著。
《圣惠方》治疗小儿痰实壮热，心胸壅闷，不想吃奶吃饭的前胡散方：
前胡（半两，去芦头） 枳壳（麸炒微黄，去瓤） 赤茯苓 川大黄（锉碎，微炒） 甘草（炙微赤，锉。各一分）
以上药物捣碎，粗筛为散。每次服用一钱，用水一小盏，煎至五分。滤去药渣温服，每日三到四次。再根据孩子的大小加减剂量服用。
《圣惠方》治疗小儿痰气结实，烦闷壅滞的半夏散方：
半夏（热水洗七遍去滑腻） 川大黄（锉碎、微炒） 甘草（炙微赤，锉。各一分） 前胡（半两，去芦头） 川朴硝（一两）

《婴孺书》治疗小儿膈上有痰饮的疟疾候方：

以上用水一升，浸泡一夜，煮五沸，滤去药渣。空腹服用三合，以吐出痰液为度。
张涣香橘皮丹 消除小儿宿食痰滞。
陈橘皮（去白，焙干） 木香（各一两） 白术（炮制） 草豆蔻（面裹微炮） 牵牛子 姜黄（各半两）
以上药物捣碎，筛为细末，滴水泛丸如黍米大。每次服用十粒，用葱白汤送下。大小便涩滞或不通时，就在乳食前服用。
张涣枳壳汤 治疗小儿痰实，壮热不退。
枳壳（麸炒，去瓤） 半夏（热水洗七遍，焙干） 木香 前胡（各一两） 干姜 甘草（炙。各半两）
以上药物捣碎，筛为细末。每次服用一钱，用水一小盏，加入生姜三片、陈橘皮少许，同煎至六分，滤去药渣，放温服。
张涣前胡半夏丹 治疗小儿痰实壮热。
前胡 半夏（各一两，热水洗七遍，焙干） 川朴硝 麦门冬（去心） 大黄（炮制。各半两）
以上药物捣碎，筛为细末，取生姜汁和丸如黍米大。每次服用十粒，用人参汤送下。

以上药物捣碎，筛为细末。每次服用半钱，加入少许盐，用沸水冲点，放温热服。
张涣白金丹 消除小儿痰实，利胸膈。
前胡 桑白皮（锉碎） 半夏（热水泡七遍） 白术（炮制。各一两） 人参（去芦头） 陈橘皮（各半两） 甘遂（一分，微炒）
以上药物捣碎，筛为细末，炼蜜和丸如黍米同大。温水送服五粒至七粒；周岁以内的婴儿及二、三岁幼儿只可服三粒。以上根据情况临时加减。
《保生信效方》逍遥丸 治疗膈实气痞，痰盛喘促。
半夏（热水浸泡洗七次，焙干，二两） 枳实（去瓤，麸炒） 槟榔（锉碎） 赤茯苓（去粗皮。各一两）
以上同为细末，用生姜自然汁煮面糊和丸如绿豆大。每次服用二三十丸，用荆芥汤送下。另做小丸给小儿服用。
《吴氏家传》治疗成人、小儿风壅，咽喉不利，痰实烦渴，困倦头昏；或发潮热，及一切风痰疮疥，都适宜服用。
龙脑 薄荷叶（去沙土，用十两） 栝蒌根（生用一两） 荆芥穗（生用四两） 甘草（生用五两一分） 缩砂仁（生用三两）
以上为细末，每四两药末加入炒盐末一两，研匀，用瓷器盛贮。每次服用一钱，像喝茶一样点服。
《吉氏家传》治疗小儿出生百日以来患痰实。

以上用水四合，煎取一合，滤去药渣。分两天服用。

以上等分，放入瓦罐子中，用火煅烧，存性，每次用半钱。用腌菜汁调下，或用麦门冬、桑白皮汤加蜜送下。

*Zhu Bing Yuan Hou Lun* (Source Book of All Diseases) on Pediatric Phlegm Syndrome: The term "phlegm" refers to pathological products formed by the stagnation and accumulation of fluid retention in the chest and diaphragm, which condense into phlegm. When infants drink milk, if there is improper regulation of cold and heat, fluid retention stagnates and accumulates between the chest and diaphragm, condensing into phlegm. When there is excessive phlegm, it causes the infant to be unable to nurse properly, spit out saliva and foam, and gradually develop a slight fever due to the accumulation of phlegm. If phlegm-solidification leads to persistent high fever, it will trigger convulsions and epilepsy (Jing Xian).

Zhenzhu (Pearl, 0.5 liang) Maidong (Ophiopogonis Radix, heart removed, 1 liang) Ruren (Pruni Semen Persicae/Peach Kernel? Note: Text says Rui Ren, likely Prunus avium or similar, peeled, 200 pieces) Badou (Crotonis Fructus/Croton Seed, skin and membrane removed, 40 grains)
The above four ingredients are ground into powder and made into pills with honey. For one-year-old children, administer two pills, sized like small beans; for infants of two hundred days old, administer two pills, sized like sesame seeds, gradually increasing the dose until efficacy is observed. After administration, red, yellow, white, black, or pus-like discharges should be expelled. Even if diarrhea occurs, do not stop the medication immediately; it will naturally cease when the pathogenic factors are fully expelled. Long-term use can make children plump and fair-skinned, serving as empirical evidence.

*Jingyan Houfang* (Later Experience Prescriptions) for treating adult and pediatric phlegm-solidification, long-standing wind-epilepsy, stranglewind (Chantoufeng), cough, generalized urticaria, and acute salivary tide. This prescription does not cause severe vomiting but only expels salivary fluid:

The above is finely ground into powder. For young adults, take an amount equivalent to "one character" (a very small measure); for children under fifteen with weak qi, take "half a character". Take once in the morning with Jinghua Water (well water drawn at dawn). Must hold a piece of rock sugar in the mouth for a while. Soon, salivary fluid will flow out like water. For chronic cases where phlegm-saliva has accumulated for years, when all phlegm-saliva is expelled, a piece resembling a cloth soaked in saliva will float on the water surface, clear as a mirror. After the phlegm-saliva is expelled, consume congee for one or two days. If vomiting is excessive and causes extreme fatigue, drink one cup of Shexiang Tang (Musk Decoction) to stop it. Musk is finely ground and taken with warm water. Previously, Taiping Shangshue Jue suffered from dizziness and took this medicine, finding it effective for expelling phlegm-saliva.
*Shenghui Fang* (Prescriptions of Holy Grace) Qianhu San (Peucedani Radix Powder) for treating pediatric phlegm-solidification with high fever, chest and heart oppression, and reluctance to nurse or eat:

Xijiao (Rhinocerotis Cornu/Rhino Horn, 1 qian, vinegar-ground powder); Baizhu (Atractylodis Macrocephalae Rhizoma/Atractylodes Rhizome); Jiegeng (Platycodonis Radix/Platycodon Root); Chenju Pi (Citri Reticulatae Pericarpium/Aged Tangerine Peel. Each 1 qian); Jinyinbo (Gold Foil) and Yinbo (Silver Foil) (three pieces each, reacted with 1 qian mercury to form cinnabar-like grains); Badou (Crotonis Fructus/Croton Seed, three grains, skin removed, wrapped in one date, roasted until fragrant and cooked, only the Croton Seed is finely ground).
The above seven ingredients are ground together uniformly and made into pills the size of small beans with refined honey. Take one to two pills per dose, dissolved in mint water. Adjust the number of pills according to the child's size.
*Yingru Shu* Changshan-Zhimu Decoction for treating phlegm-fluid retention in the diaphragm in children, specifically for malaria symptoms:
Changshan ( Dichroae Radix/Dichroa Root, 5 fen); Zhimu (Anemarrhenae Rhizoma/Zhimu Root, 4 fen); Songluo (Usnea/Usnea Fibra); Gancao (Glycyrrhizae Radix et Rhizoma/Licorice Root, honey-fried. Each 3 fen).
Soak the above in one sheng of water overnight, boil for five boils, filter. Take 3 he on an empty stomach, stopping when phlegm is expelled.

Each 0.5 liang).
Crush and sift into fine powder. Take half a qian per dose, add a little salt, brew with boiling water, and take warm.

Grind into fine powder, mix with fresh ginger juice and wheat paste to form pills the size of mung beans. Take 20-30 pills per dose, with Schizonepeta decoction. Make smaller pills for children.
*Wu Shi Jia Chuan* treats wind congestion in adults and children, throat obstruction, phlegm-solidification with thirst, fatigue, dizziness, tidal fever, and all types of wind-phlegm sores and scabies. Suitable for all these conditions.
Longnao (Borneolum Syntheticum/Synthetic Borneol); Bohe Ye (Menthae Haplocalycis Herba/Mint Leaf, cleaned of sand and dirt, 10 liang); Gualou Gen (Trichosanthis Radix/Trichosanthes Root, raw, 1 liang); Jingjie Sui (Schizonepetae Spica/Schizonepeta Spike, raw, 4 liang); Gancao (Glycyrrhizae Radix et Rhizoma/Licorice Root, raw, 5 liang 1 fen); Suosharen (Amomi Fructus Rotundus/Cardamom Fruit, raw, 3 liang).

Boil in five large he of water until reduced to 2 he. Filter. Add Poxiao and take throughout the day.

Grind into powder, boil in 6 he of water until reduced to 3 he. Filter. Add Poxiao. Take on an empty stomach, twice daily.

Huangqin (Scutellariae Radix/Scutellaria Root); Qianhu (Peucedani Radix/Peucedanum Root. Each 1 fen); Huanglian (Coptidis Rhizoma/Coptis Rhizome, 3 fen); Shanzhizi (Gardeniae Fructus/Gardenia Fruit, seven pieces).
Grind into powder, boil in 9 he of water until reduced to 5 he. Take.
Heisanzi (Black Powder) passed down by Ding Shifa, a physician in Changsha, treats pediatric salivary congestion, cough, and vomiting.
Tiannanxing (Arisaematis Rhizoma Preparatum/Prepared Arisaema Rhizome); Banxia (Pinelliae Rhizoma/Pinellia Tuber); Zhuya Zaojiao (Gleditsiae Sinensis Fructus/Chinese Honeylocust Pod); Badou (Crotonis Fructus/Croton Seed); Baifan (Alumen/Alum).
Equal parts, place in a clay pot, roast over fire until charred but retaining properties (cun xing). Use half a qian per dose. Mix with pickling juice or take with Maidong and Sangbaipi decoction plus honey.
Huai Xian Wan (Broken Saliva Pill) passed down by Zheng Yu, a physician in Changsha, treats children with rattling saliva in the throat like a saw, combined with cold damage, body heat, red face, and all types of salivary issues.
Banxia (Pinelliae Rhizoma/Pinellia Tuber, 2 qian, ground, kneaded into cakes with fresh ginger juice, roasted slowly until yellow and dry); Fenshuang (Hydrargyri Oxydum Rubum/Red Mercury Oxide? Note: Text says Fen Shuang, often Tin Oxide or similar); Qian Baishuang (Hydrargyri Chloridum Corrosivum/Sublimated Calomel? Note: Lead White Frost); Badoushuang (Crotonis Fructus Preparatum/Prepared Croton Seed); Xionghuang (Realgar/Realgar); Xie Shao (Scorpio/Scorpion tail). Each 0.5 qian.
Grind each into powder, then grind together uniformly. Use thin malt paste to form pills of size ○. To expel phlegm, take five pills per dose, dissolved in Jiedexin Tang (Centella Decoction). If phlegm is not expelled, give more Jiedexin Tang to induce vomiting. If taking phlegm-expelling medicine causes diarrhea after three consecutive doses, provide supplementation/nourishment.
