Nest's *Pathogenesis of Diseases* (Chao Shi Bing Yuan) on the condition of infantile constipation: When infants suffer from constipation, it is due to heat in the zang-fu organs affecting the large intestine. The spleen and stomach are the sea of water and grain; the essence of water and grain transforms into qi and blood, while its dregs travel through the large intestine. If the triple burner and five zang organs are disharmonious, heat gathers in the large intestine, causing excess heat and solidification, resulting in dry, rough, and obstructed bowel movements.
*Wai Tai Mi Yao* records: Zi Shuang Wan (Purple Double Pill) from *Qian Jin Fang* treats infants with fever, headache, indigestion, abdominal distension, or colic in the lower abdomen, difficulty in urination and defecation, or frequent dysentery-like episodes. If an infant has no other ailments but suffers only from overeating without self-restraint, irregular breastfeeding, or fright palpitations alternating with chills and fever, this pill is indicated. If not cured, it may be administered again. Infants desiring to defecate indicates steaming heat; reduced feeding, labored breathing, night crying, and insomnia indicate internal disharmony. All these conditions are suitable for this pill, avoiding other medicines. Frequent use yields miraculous results. This prescription is not found in *Qian Jin Fang*.
Herbs: Crotonis Fructus Preparatus (Badou, 巴豆) [peeled, cored, roasted], Prunus mume Kernel (Ruihe Ren, 蕤核仁) [18 zhu each, pounded separately], Ophiopogonis Radix (Maidong, 麦门冬) [10 zhu, cored], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [5 zhu, honey-fried], Kansui Radix (Gansui, 甘遂), Cinnabaris (Zhenzhu, 真朱) [2 zhu each], Ostreae Concha (Muli, 牡蛎) [roasted], Animal Fat (La, 腊) [8 zhu each].
The above eight herbs: Wash Crotonis Fructus Preparatus in hot water, grind, and squeeze out the oil using a new cloth. Pound Gansui, Gancao, Muli, and Maidong separately, sieve finely; pound Badou and Ruihe Ren until very fine, then add the powdered herbs and pound for three thousand strokes. If the medicine is too dry, add a little honey. For half-year-old infants, administer one pair of seeds size (like a apricot seed); for one- to two-year-olds, half a sesame seed size per pair; for three-year-olds, one sesame seed size per pair; for four-year-olds, two sesame seed-sized pills; for five- to six-year-olds, two hemp seed-sized pills; for seven- to eight-year-olds, two small bean-sized pills; for nine- to ten-year-olds, slightly larger than two small bean-sized pills. Administer at cockcrow time. If defecation does not occur by sunrise, drink a few he of hot rice gruel to induce passage; the pills usually pass in pairs. If diarrhea is severe, drink cold rice gruel to stop it.
