According to *Shenghui Fang* (Prescriptions from the Holy Imperial Library): The condition known as "Tiandiao" (Heavenly Suspension) in infants is caused by wind-heat in the zang-fu organs and the production of phlegm-saliva by the spleen and stomach. Once phlegm-saliva is generated, it causes stagnation and obstruction in the heart and diaphragm, with pathogenic heat accumulating and failing to be dispersed and unblocked. This typically results from the wet nurse having irregular eating habits, consuming excessive alcohol and meat, allowing noxious and toxic qi to enter the breast milk. When the infant ingests this milk, old food stagnation fails to digest, generating heat in the heart and lungs. As the heat-toxin becomes abundant, it is invaded by wind-pathogens. Wind and heat combine, disturbing the heart, leading to mental fright and palpitations, upward deviation of the eyes, high fever that does not cease, and convulsions of the four limbs; hence it is called Tiandiao.
Mr. Mao states: Infants born with Tiandiao exhibit restlessness of the heart spirit, generalized high fever, convulsions of the hands and feet, fright and palpitations, and upward deviation of the eyes. This syndrome arises because the mother's diet is irregular, and she consumes excessive alcohol and meat, allowing toxic vapors to enter the milk. Old qi stagnates and fails to dissipate, causing the heart and lungs to clash, with wind-heat disturbing the heart, thus producing this syndrome. Treatment should urgently administer Duoming San (Life-Seizing Powder) to induce vomiting or purging of wind-phlegm (formula see section on Acute and Chronic Convulsive Disorders), followed by Yunqi San (Qi-Balancing Powder) (formula see section on Disharmony of Stomach Qi) to awaken the spleen. After the qi mechanism is balanced, one should frequently administer Xionzhu San (Male Cinnabar Powder) (formula see section on Fright and Epilepsy), combined with Zhenxin Wan (Heart-Calming Pills) (formula see section on All Fright Conditions) and Jianpi San (Spleen-Strengthening Powder) (formula see section on Disharmony of Stomach Qi) to promote health. If signs such as fixed gaze, fish-mouth opening, crow-like sounds, black and lusterless eyes, or black fingernails appear, these are death omens and are incurable.
Zhang Huan discusses heart-diaphragm stagnation with pathogenic heat, where phlegm-saliva accumulates and cannot be dispersed; or when the wet nurse drinks alcohol and eats meat without restraint, noxious toxic qi enters the milk, causing infant food stagnation. Pathogenic heat and toxic vapors attack the heart spirit, resulting in fright, upward eye deviation, unremitting high fever, and limb spasms. This disease is named Tiandiao. In severe cases, the nail beds turn entirely blue, resembling possession by spirits.
