The *Shenghui Fang* discusses: For weaned infants, if the five zang organs are harmonious and the Ying (Nutritive) and Wei (Defensive) qi flow smoothly, body fluids circulate normally. Even if they drink large amounts of water, it will not cause disease. However, if care is improper, leading to obstruction in the Sanjiao (Triple Burner), the stomach and intestines fail to transport and transform. Drinking water then causes stagnation. If this is combined with cold pathogenic factors, they bind together to form Pi (masses). "Pi" refers to lesions located on one side, between the two hypochondria, causing intermittent pain.
Master Mao discusses: From five months after birth up to seven years of age, if a child has masses forming Pi in the abdomen, sized like a plum pit or resembling an egg, accompanied by constant crying and pain, these can be classified into several types. Pain under the right hypochondrium is diagnosed as Pi Qi. Taking Peng Ezhu San (see Men's Diseases section) combined with Jianpi San (see Stomach Qi Disharmony section) leads to recovery. If the complexion turns black, eyes gaze abnormally, black blood is passed in stool, nose and mouth feel cold, hands and feet are cold, and the child refuses food, this is a death sign.
Qian Yi discusses: Pediatric Pi arises from undigested milk and food lying latent in the abdomen. Symptoms include alternating chills and fever, drinking water, or wheezing and coughing, resembling tidal fever. If not treated early, it inevitably develops into Gan (malnutrition syndrome). Because Pi causes anorexia, the Spleen and Stomach become deficient and generate internal heat, leading to excessive water intake. This washes out the intestines and stomach, depleting body fluids, so the Stomach cannot transmit and digest food and water. The pulse is deep and fine. Continued anorexia further weakens the Spleen and Stomach, rendering the limbs immobile, allowing various pathogens to arise. Few escape becoming emaciated and developing Gan.
Qian Yi discusses Pi causing tidal fever: The son of Cao Xuande, aged three, had a yellow complexion, frequent chills and fever, anorexia, yet incessant drinking and nursing. Other physicians treated it as tidal fever using Niuhuang Wan (Calf Bezoar Pill) and Shexiang Wan (Musk Pill) without success. Using Zhike Gansan (Dry Scutellaria Decoction for Thirst) caused vomiting. Qian Yi stated: It should be purged with Baibingzi (White Cake Pill) (see Convulsions section), followed by Spleen tonification, and finally using Xiaoji Wan (Accumulation Elimination Pill) to grind down the accumulation (see Accumulations section). This was indeed Pi, and the child recovered. Why? Anorexia with only water intake indicates food stagnating in the gut unable to digest, causing chills. Vomiting after taking thirst-quenching medicine occurs because the medicine irritates the Spleen. Purging resolves it.
*Yingtong Baojian*: Pediatric Food Pi results from premature weaning, lack of maternal milk leading to sudden solid feeding, or irregular eating habits. Food remains in the Spleen, undigested by the child, forming masses. These masses move unpredictably within the abdomen, sometimes shaped like pears or chestnuts, or like inverted cups.
*Yingtong Baojian*: Cold stagnation failing to flow, combined with phlegm-fluids, forms Pi, naturally producing audible sounds.
*Yingtong Baojian*: Pediatric Pi refers to masses moving within the abdomen with changing locations.
