Zhang Huan discusses: When an infant is in the womb, the zang-fu organs are not yet fully developed, and the spirit (Shen) and vital energy (Qi) are weak. The infant's movements, breathing, and respiration all follow the mother. Whatever the mother does, the fetus will be affected. If the mother fails to regulate her lifestyle properly—such as having irregular diet and drinking, experiencing frequent anger or joy, sitting or lying in drafts, consuming alcohol, or engaging in excessive sexual activity—and violates all taboos, the born child may appear physically frail with open cranial sutures. Alternatively, if breastfeeding management is improper or bathing exposes the child to drafts, it can cause high fever, vomiting, restlessness of the heart-mind, convulsions of the hands and feet, rigidity of the body, upward deviation of the eyes, and teeth grinding. This is known as fetal fright wind disease (Tai Jing Feng).
The song from *Shibi Jing* regarding the thirty-six types of fetal fright conditions states:
Within the first month of gestation, if the mother suffers from fright-induced illness, the child will manifest symptoms upon birth. The back remains straight; when crying, the mouth puckers first; the face turns blue, fists clench, and cheeks retract. Eyes close, teeth grind, and tendons become tense. (*Fengsui Jing* notes: Closed eyes with discharge indicate the time of conception. Commentary explains: Discharge indicates early qi reception. If Yin Qi is weak, the fetus is prone to miscarriage due to fright; if Yang Qi is weak, the fetus is less prone. Weak Yang leads to thin limbs and emaciated muscles; weak Yin leads to severe flesh wasting, though neither allows one to reach a full lifespan. If premature birth occurs due to fright near term, symptoms include puckered lips, clenched fists, nasal congestion, and locked jaws. In such cases, do not treat as fright wind initially. First, induce mild sweating, then treat the fright and regulate Qi. The nursing mother should take Qi-regulating herbs, and the infant may have external patches applied to the anterior fontanelle to expel pathogens.) No matter how much one calls, the eyes remain closed. Physicians must inquire about the mother upon seeing these signs to understand the origin of the ailment. The treatment involves resolving wind-phlegm; once phlegm is cleared, the fright pathogen naturally dissipates. (If treated out of sequence, eyes may remain shut; treating fright first may cause vomiting or diarrhea, potentially transforming into Spleen Wind in autumn or summer. Initially, do not diagnose as Spleen Wind, lest failure to sweat lead to other complications. *Fengsui Jing* confirms the song content and adds: Administer Wuxi Gao, followed by Shengyin Wan. These formulas are identical to those in *Yujue*, see sections on acute and chronic fright wind.)
The song for forty-eight fetal fright conditions in *Xiao'er Xingzheng Lun* is similar, with the following explanation: This condition arises from maternal fright during gestation and manifests twenty-five days after birth. If it does not occur at this fixed time, or appears on the thirty-fifth day or another day, it is not fetal fright. Physicians unfamiliar with this often misdiagnose it as general fright wind. Symptoms include closed eyes, rigid back, retracted cheeks, and clenched fists. Treat only with [specific] pills to descend phlegm (see section on all epilepsy types), or half a pill of Daqing Dan (see section on acute/chronic fright wind).
The *Feixian Lun* describes fetal fright as follows: If the mother experiences fright during pregnancy, the fright qi transmits to the fetus. After birth, the child may have a bluish-red complexion, twitching lips, frequent nightmares, heat in the five centers (palms, soles, and chest), unclosed fontanelles, frequent startled cries, and high fever. This is the fetal fright syndrome.
*Culun Jing* prescribes Niuhuang Wan for pediatric fetal fright, epilepsy, or heart heat:
