Pediatric Luoli (scrofula) is caused by wind-heat and blood dryness in the Liver and Gallbladder meridians. These two meridians typically have abundant Qi but deficient Blood. Anger causes Liver fire to move and Blood to become hot; Kidney Yin deficiency fails to nourish Liver Wood, leading to Blood dryness. Dryness affects the sinews, causing nodules to accumulate like beads on a string. Symptoms often appear before and behind the ears, extending to the jaw and neck, down to the supraclavicular fossa and lateral chest/axilla, also known as Machdao (horse knife). Initially like bean grains, they gradually enlarge like plums or peaches, appearing singly or in clusters. They are movable upon pressure with slight pain, not very hot. Over time, they worsen, possibly accompanied by neck stiffness/pain, mild afternoon fever, night-time dry mouth, loss of appetite, limb fatigue, hardness without ulceration, or ulceration without healing. All stem from Qi and Blood deficiency, often transforming into Gancalcination and consumption (Gan Lao). This condition does not originally belong to surgery; knives, needles, and caustic medicines to remove the core must be strictly avoided. Ignorant practitioners, failing to recognize the disease origin, misuse caustic medicines to extract the core, unaware that the Liver and Gallbladder meridians contain Ministerial Fire. When depressed and unable to extend, Luoli forms. At such times, one should tonify Qi and nourish Ying (Nutritive) level, soothe sinews and dissipate depression; even then, time may be insufficient. How dare one use knives, needles, and caustic medicines, causing open wounds that fail to close and mixing pus with blood? I have witnessed countless deaths caused by such mis-treatment. Whenever children suffer from Luoli, do not treat recklessly; only internal single-formula prescriptions are suitable. Absolutely avoid removing the core. Be cautious!
For any pediatric cervical lymphadenopathy, whether 3-5 or dozens of nodes, painful or painless, hot or non-hot, use Fengweicao (Pteridaceae plant growing at wall bases, stems iron-wire black, leaves like phoenix tails, named Shichangsheng in Materia Medica, i.e., small ferns in wall cracks). Use only the roots, wash clean with water. Take 1 liang per dose, decoct strongly in a clay pot with one bowl of thick glutinous rice wine. Strain and drink the wine, once daily. Do not seek quick results; it takes one month at most, twenty days at least, for the nodes to completely disappear and never recur. This medicine has a plain taste and is not bitter/cold. It is truly a divine gift, cheap yet efficacious, a true immortal prescription.
Whenever suddenly sores form nodes like apricot pits of varying sizes before and behind the ears in children, named Machdao Chuang (Horse Knife Sores), these are the root of Luoli. Use Taoshu Baipi (Cortex Pruni Persicae Albus), cut a piece three fingers wide, scrape off the outer bark, keep the inner layer, apply to the sore. Moxibustion with Ai Zhu (moxa cones) on the peach bark. Stop when heat and pain are felt, do not injure the skin. Moxibustion again the next day; after a few sessions, the nodes will vanish.
To treat pediatric Luoli that has not ulcerated, promoting internal resolution; and for those already broken, promoting wound closure. Take this for one month for recovery. Use Zhijiangcan (Bombyx Batryticatus) 8 liang. Wash three times with clear water to remove calcareous impurities, sun-dry and stir-fry until crisp. Separately, stir-fry Banbanmi (Oryza Sativa Var. Glutinosa) 8 liang until cooked. Grind both into fine powder together. Bind with rice paste to form pills weighing 1 qian each. Take on an empty stomach daily: older children take two pills, younger ones one pill, crushed and mixed with XiaKucao (Prunellae Spica) decoction. Usually, nourish the body with sweet, fatty, meat-based, and moistening foods.
