Tianxing Liqi Plague Disease, affecting households and regions alike, all stem from pathogens entering via the mouth and nose, thus spreading rapidly like the wind.
This plague disease caused by the violent Qi of Heaven and Earth often affects entire households and regions simultaneously; it is because pathogens enter the body through the mouth and nose that transmission is as swift as the wind.
One must distinguish between exterior and interior, Yin and Yang poisons, treating according to the seasons and assessing severity. Ancient methods prioritized attacking the pathogen, using Jingfang Baidu San and Puji Xiaodu Yin for rescue.
Treatment requires distinguishing whether the pathology is in the exterior or interior, and whether it constitutes a Yin or Yang poison. Therapies should be selected based on seasonal variations, carefully assessing the severity. Ancient practitioners prioritized attacking the pathogen, commonly employing Jingfang Baidu San (Aster and Saposhnikovia Decoction) and Puji Xiaodu Yin (Universal Relief Detoxification Decoction) for treatment.
[Note] The plague syndrome is the prevalence of violent Qi from Heaven and Earth, spreading door-to-door and household-to-household. Regardless of age or strength, anyone who contacts it falls ill immediately. Since pathogens enter via the mouth and nose, transmission is as rapid as wind and fire. However, toxins vary by location (exterior/interior) and nature (Yin/Yang). Treatments may involve dispersing, purging, or clearing heat, adapted to the distinct climates of spring warmth, summer heat, autumn coolness, and winter cold, as well as the patient’s deficiency or excess and the severity of the condition. Ancient methods prioritized purging toxins because pathogens entering via the mouth and nose often result in interior diseases. Dispersal uses Jingfang Baidu San, clearing uses Puji Xiaodu Yin, and purging uses Ersheng Jiuku Dan. By weighing these appropriately and treating prudently, one may achieve mastery in clinical practice.
[Note] The plague syndrome results from the prevalence of violent Qi from Heaven and Earth, spreading throughout communities. Both young and old, strong and weak, fall ill upon contact. Because pathogens enter through the mouth and nose, transmission is as rapid as wind and fire. Toxins can be located in the exterior, interior, Yin, or Yang. Treatments may involve dispersing, purging, or clearing heat, depending on the specific climatic conditions of spring, summer, autumn, and winter, the patient’s constitutional status (deficiency or excess), and the severity of the illness. Ancient methods prioritized purging toxins because pathogens entering via the mouth and nose frequently cause interior diseases. For dispersing pathogens, use Jingfang Baidu San (Aster and Saposhnikovia Decoction); for clearing heat and detoxifying, use Puji Xiaodu Yin (Universal Relief Detoxification Decoction); for purging toxins, use Ersheng Jiuku Dan (Two Sages’ Rescue Bitter Pill). By judiciously selecting appropriate methods and treating with prudence, one can achieve adeptness in clinical application.

Shanghan Zhen [Ban/Shasha], due to improper sweating or purging, or initial contraction of seasonal Qi. Exterior pathogens re-constrain Ying and Wei, manifesting externally as Shasha and Zhenban.
Shasha and Zhenban arising from Cold Damage are often due to improper use of diaphoresis or purgation, or initial infection with seasonal pathogens. Exterior pathogens re-constrain the Ying (Nutrient) and Wei (Defensive) levels, manifesting externally on the skin and vessels as Shasha (petechiae-like spots) and Zhenban (rashes/macules).
Shasha white like skin millet, Zhen red like beans or patches. Red is mild, crimson severe, black mostly fatal. Pale red sparse dark indicates Yin Shasha/Zhenban.
Shasha appears white like millet grains on the skin; Zhen appears red like beans or connected patches. A red color indicates mildness, crimson indicates severity, and black indicates a high mortality rate. Pale red, sparse, and dull colors indicate Yin Shasha or Yin Zhenban.
If not fully erupted, treat with Shengma Xiaodu; if heat is excessive, use Sanhuang Shigao Tang; if erupted, use Xiaoqingdai Yin. For simultaneous exterior-interior involvement, use Shuangjie Tongsheng Tang first.
If the rash has not fully erupted, treat with Shengma Xiaodu method (Shengma Gegen Tang combined with Xiaodu Xijiao Yin). If heat is excessive, use Sanhuang Shigao Tang (Three Yellows and Gypsum Decoction). If already erupted, use Xiaoqingdai Yin (Xiaoban Qingdai Yin). For simultaneous exterior-interior involvement at onset, use Shuangjie Tongsheng Tang (Dual-Resolving Holy Guardian Decoction) to resolve both levels first.
[Note] Shanghan-induced Shasha, Zhen, and Ban result from improper sweating or purging, causing exterior pathogens to constrain and interior heat to erupt. Seasonal epidemics transmit and erupt immediately upon infection, similar to plague in its intensity and speed. Eruption at the Wei level causes Shasha; since Wei governs Qi, the color is white like millet grains. Eruption at the Ying level causes Zhen; since Ying governs Blood, the color is red. Superficial lesions are Zhen, deep/severe are Ban. Ban resembles beans, sometimes merging into patches. Red Zhen is mild, crimson is severe, black is fatal; color distinguishes heat depth and prognosis. Pale red, sparse, and dull colors indicate pathogens in the Three Yang channels, transforming into Yin due to interior transmission or overuse of cold medicines, constituting Yin Ban, Yin Shasha, or Yin Zhen, requiring treatment for Yin-Cold. If not fully erupted with mild exterior heat, use Shengma Gegen Tang combined with Xiaodu Xijiao Yin. If exterior heat is severe, use Sanhuang Shigao Tang. If erupted, use modified Xiaoqingdai Yin. For early-stage simultaneous exterior-interior ambiguity, use Shuangjie Tongsheng Tang to open both levels; other methods follow above.
[Note] Shasha, Zhen, and Ban arising from Cold Damage are caused by inappropriate use of diaphoresis or purgation, leading to the constraint of exterior pathogens and the eruption of interior heat. Seasonal epidemic diseases are contagious and erupt immediately upon infection, resembling plague in their intensity and speed. When pathogens affect the Wei (Defensive) level, Shasha occurs; since Wei governs Qi, the color is white like millet grains. When pathogens affect the Ying (Nutrient) level, Zhen occurs; since Ying governs Blood, the color is red. Lesions superficial to the skin are Zhen, while deeper or more severe ones are Ban. Ban resembles beans and may merge into large patches. Red Zhen indicates mildness, crimson indicates severity, and black indicates fatality; color is used to determine the depth of heat and prognosis. Pale red, sparse, and dull colors indicate that pathogens are in the Three Yang channels, have transformed into Yin due to transmission from exterior to interior or overuse of cold-natured medicines, constituting Yin Ban, Yin Shasha, or Yin Zhen, which should be treated according to Yin-Cold patterns. If the eruption is incomplete with mild exterior heat, use Shengma Gegen Tang (Pueraria and Cimicifuga Decoction) combined with Xiaodu Xijiao Yin (Detoxification and Rhinoceros Horn Decoction). If exterior heat is severe, use Sanhuang Shigao Tang (Three Yellows and Gypsum Decoction) to disperse. If already erupted, use modified Xiaoqingdai Yin (Xiaoban Qingdai Yin) to clear heat. For early-stage cases with unclear exterior-interior presentation, use Shuangjie Tongsheng Tang (Dual-Resolving Holy Guardian Decoction) to resolve both levels first; other methods remain the same.
