阴囊肿大邪气凝，风痒湿坠热多疼，疏风五苓导赤散偏坠守效丸

【注】阴器者，乃诸筋之总会也。因邪客于少阴、厥阴之经，湿热之气与风冷之气相
搏，气不得通，故结聚而阴囊肿大。总之，风盛多痒，湿盛多坠，热盛多疼。如外肾肤囊
肿大，痒痛坠下，此风湿袭于下也，宜疏风五苓散主之。如外肾肤囊肿痛光亮，此因心
火移热于小肠故也，宜加味五苓散或导赤散主之。更有偏坠一证，或左或右，睾丸作肿
者，此因食积不消，湿气下行故也，宜加味守效丸主之。
疏风五苓散
防风 苍术（米泔水浸） 肉桂 羌活 猪苓 泽泻 赤茯苓 白术（土炒）
引用生姜，水煎服。
【方歌】阴肿疏风五苓散，防风苍术肉桂羌，猪苓泽泻赤苓术，煎服之时入生姜。
加味五苓散
金铃子 白术（土炒） 泽泻 木通 茴香（炒） 赤茯苓 橘核仁 肉桂 槟榔 猪苓
引用生姜、灯心，水煎服。
【方歌】五苓散内用金铃，白术泽泻与木通，茴香赤苓橘核配，肉桂槟榔合猪苓。
导赤散
加味守效丸（方见不小便）
南星 山楂肉（酒炒） 苍术（炒，各二两） 白芷 半夏（姜制） 橘核仁 神曲（炒，各一
两） 海藻 昆布（各五钱） 吴茱萸 青皮（醋炒） 元胡索（醋炒） 荔枝核（炒，各一两）
共为末，神曲糊丸，如梧桐子大，每服三十丸，空心酒下。
阴囊肿大是由于邪气凝聚所致，风邪盛则多瘙痒，湿邪盛则多有下坠感，热邪盛则多疼痛。治疗偏坠证应使用守效丸，效果最为灵验。
【注】外生殖器是全身筋脉汇聚之处。因为邪气客居于足少阴肾经和足厥阴肝经，湿热之气与风寒之气相互搏结，导致气机运行不畅，从而凝结聚积，造成阴囊肿大。总之，风邪盛则瘙痒明显，湿邪盛则下坠感重，热邪盛则疼痛剧烈。如果外肾皮肤囊袋肿大，伴有痒、痛、下坠感，这是风湿之邪侵袭下部所致，适宜用疏风五苓散主治。如果外肾皮肤囊肿且疼痛、光亮，这是因为心火将热邪转移至小肠所致，适宜用加味五苓散或导赤散主治。另外还有偏坠这一病症，睾丸左侧或右侧肿胀，这是由于食积不消化，湿气下行所致，适宜用加味守效丸主治。
疏风五苓散
组成：防风、苍术（用米泔水浸泡）、肉桂、羌活、猪苓、泽泻、赤茯苓、白术（用土炒）。
用法：引用生姜为引，水煎服。
【方歌】阴肿疏风五苓散，防风苍术肉桂羌，猪苓泽泻赤苓术，煎服之时入生姜。
加味五苓散
组成：金铃子、白术（用土炒）、泽泻、木通、茴香（炒）、赤茯苓、橘核仁、肉桂、槟榔、猪苓。
用法：引用生姜、灯心草为引，水煎服。
【方歌】五苓散内用金铃，白术泽泻与木通，茴香赤苓橘核配，肉桂槟榔合猪苓。
导赤散
加味守效丸（方剂见“不小便”章节）
组成：南星、山楂肉（酒炒）、苍术（炒，各二两）、白芷、半夏（姜制）、橘核仁、神曲（炒，各一两）、海藻、昆布（各五钱）、吴茱萸、青皮（醋炒）、元胡索（醋炒）、荔枝核（炒，各一两）。
用法：共研为细末，用神曲糊制作成丸药，大小如梧桐子，每次服用三十丸，空腹时以酒送服。
Swelling of the scrotum is caused by the condensation of pathogenic factors. When wind predominates, itching is prominent; when dampness predominates, a sensation of heaviness or prolapse is felt; when heat predominates, pain is severe. For treating hernia (pian zhui), Shou Xiao Wan should be used, which is most effective.
[Note] The external genitalia are the convergence point of all sinews. Pathogenic factors invade the Foot-Shaoyin Kidney Meridian and Foot-Jueyin Liver Meridian. Damp-heat and cold-dwindle interact, obstructing the flow of Qi, leading to condensation and swelling of the scrotum. In summary, excessive wind causes itching, excessive dampness causes heaviness/prolapse, and excessive heat causes pain. If the skin of the external genitalia swells with itching, pain, and a sensation of dropping, this is due to wind-damp attacking the lower part; Shu Feng Wu Ling San is indicated. If the swollen skin of the external genitalia is painful and shiny, this is due to Heart Fire transferring heat to the Small Intestine; Jia Wei Wu Ling San or Dao Chi San is indicated. There is also a condition called hernia (pian zhui), where the testis swells on either the left or right side. This is due to food stagnation not digesting and dampness descending; Jia Wei Shou Xiao Wan is indicated.
Shu Feng Wu Ling San
Ingredients: Fangfeng (Saposhnikovia Root), Cangzhu (Atractylodis Rhizoma, soaked in rice water), Rougui (Cinnamon Bark), Qianghuo (Notopterygii Rhizoma et Radix), Zhuling (Polyporus), Zexie (Alismatis Rhizoma), Chifuling (Poria with Red Hostwood), Baizhu (Atractylodis Macrocephalae Rhizoma, stir-fried with soil).
Administration: Use fresh ginger as a guide, decoct in water and take orally.
[Prescription Song] For scrotal swelling, Shu Feng Wu Ling San is used;
Fangfeng, Cangzhu, Rougui, and Qianghuo constitute its base.
Zhuling, Zexie, Chifuling, and Baizhu are added too,
Decoct with fresh ginger for the best effect to pursue.
Jia Wei Wu Ling San
Ingredients: Jinlingzi (Meliae Semen), Baizhu (Atractylodis Macrocephalae Rhizoma, stir-fried with soil), Zexie (Alismatis Rhizoma), Mutong (Akebiae Caulis), Huixiang (Foeniculi Fructus, stir-fried), Chifuling (Poria with Red Hostwood), Juhe Ren (Citri Reticulatae Semen), Rougui (Cinnamon Bark), Binglang (Arecae Semen), Zhuling (Polyporus).
Administration: Use fresh ginger and Dengxincao (Junci Medulla) as guides, decoct in water and take orally.
[Prescription Song] In Wu Ling San, add Jinlingzi,
Baizhu, Zexie, and Mutong together.
Huixiang, Chifuling, and Juhe Pei,
Rougui, Binglang, and Zhuling combine.
Dao Chi San
Jia Wei Shou Xiao Wan (See formula under "Urination Difficulty")
Ingredients: Nanxing (Arisaematis Rhizoma), Shan Zha Rou (Crataegi Fructus flesh, stir-fried with wine), Cangzhu (Atractylodis Rhizoma, stir-fried, 2 liang each), Baizhi (Angelicae Dahuricae Radix), Banxia (Pinelliae Rhizoma, processed with ginger), Juhe Ren (Citri Reticulatae Semen), Shen Qu (Massa Medicata Fermentata, stir-fried, 1 liang each), Haizao (Sargassum), Kunbu (Eckloniae Thallus, 5 qian each), Wuzhuyu (Evodiae Fructus), Qingpi (Citri Reticulatae Pericarpium Viride, processed with vinegar), Yuanhusuo (Corydalis Rhizoma, processed with vinegar), Lizhi He (Litchi Seed, stir-fried, 1 liang each).
Administration: Grind all into fine powder, make pills using Shen Qu paste, size like wutong seeds. Take 30 pills per dose, orally with wine on an empty stomach.
