General Overview
Pediatric heatstroke diseases have four types. Heatstroke (Zhongshu) is caused by Yang pathogenic factors, involving only the contraction of summer-heat, manifesting as fever with sweating. Summer-heat injury (Shangshu) is caused by Yin pathogenic factors, occurring when summer-heat is compounded by cold, manifesting as fever without sweating. If extreme summer-heat attacks the Liver, convulsions occur. If it attacks the Heart, it leads to cold extremities, fainting, and loss of consciousness.
Heatstroke
The syndrome of heatstroke manifests as sweating and high fever due to the steaming effect of summer-heat. Consequently, headache, extreme thirst, restlessness, and irritability occur. In severe cases, there is Qi deficiency with mental fatigue, and coldness in both legs. Jiawei Renshen Baihu Tang (Modified Ginseng and White Tiger Decoction) must be used for excellent efficacy.
Summer-heat Injury
Pediatric summer-heat injury refers to contracting summer-heat followed by exposure to wind-cold. The symptoms include fever without sweating, thirst, reddish complexion, dry retching, nausea, abdominal colic, lethargy, anorexia, and heavy, painful limbs. The treatment method should be internal clearing and external dispersing; Erxiang Yin (Two Fragrances Infusion) is highly effective. If the healthy Qi is deficient, tonifying the healthy Qi to eliminate pathogens using Liujunzi Tang (Six Gentlemen Decoction) yields results. If summer-heat injury is complicated by food stagnation, leading to aversion to food and frequent vomiting and diarrhea, Jiawei Xiangru Yin (Modified Mosla Infusion) is the most efficacious treatment.
Summer-wind Convulsion
Summer-wind convulsion presents with twitching of hands and feet, resembling wind-stroke or convulsive disorders. It arises from summer-heat attacking the Liver and injuring the interior with wind pathogens. Symptoms include irritability, thirst, sweating, fever, and yellow-red urine and stool. First, use Jiawei Xiangru Yin to disperse wind; subsequently, use Yulu San (Jade Dew Powder) to clear heat. Once the summer-heat is resolved, the convulsions cease naturally. It must not be treated as epilepsy or wind-stroke.
Summer-jue (Syncope)
The syndrome of summer-jue manifests as dizziness, fainting, and loss of consciousness. This is because the patient's primordial Qi has always been deficient. Summer-heat combined with phlegm rushes upward to the Heart, causing mental confusion. For those with deficiency, Qingshu Yiqi Tang (Clear Summer-heat and Tonify Qi Decoction) is used. For those with phlegm accumulation, Chensha Yi Yuan San (Cinnabar and Yi Yuan Powder) or Baolong Wan (Embracing Dragon Pill) can be used.
General Overview
Huoluan (Vomiting and Diarrhea Syndrome) is caused by mixed pathogens such as wind, cold, summer-heat, and dampness. It occurs suddenly with chaotic vomiting and diarrhea accompanied by heart and abdominal pain. Cases where vomiting and diarrhea occur are due to abundant summer-dampness and are called Damp Huoluan. Although summer-dampness is abundant, if vomiting and diarrhea have occurred, the pathogen is expelled, making it easy to treat. Cases where one desires to vomit but cannot, and desires to defecate but cannot, are due to cold excess causing stagnation, known as Dry Huoluan. Since there is no outlet for vomiting or defecation, the pathogen remains trapped, making the condition serious.
Damp Huoluan
When summer-heat and dampness combine, symptoms include incessant vomiting and diarrhea, frequent abdominal pain, thirst with desire to drink, and chest/epigastric stuffiness and distension. For abundant summer-dampness, Erxiang Yin is the primary treatment; for abundant summer-heat, Chensha Yi Yuan San is used. This clears the summer-dampness pathogen and cures the Huoluan syndrome with excellent efficacy.
Dry Huoluan
Desiring to vomit but unable, and desiring to defecate but unable, this is caused by the coagulation of cold and summer-heat, resulting in unbearable abdominal colic, commonly known as "Intestinal Twisting Miasma." Treatment requires distinguishing between cold and heat. If there is irritability, thirst, and excessive drinking, it is a heat pattern, primarily treated with Guiling Ganlu Yin (Cinnamon Twig, Poria, and Sweet Dew Infusion). If there are cold extremities and no thirst, Lizhong Tang (Regulate the Middle Decoction) is decocted and administered.
