Wu Ying (Five Stiffnesses) manifests as the head tilting backward to seek breath, difficulty in moving or shaking the body, qi stagnation causing pain radiating to the chest and diaphragm, cold and stiff soles of hands and feet. This is due to yang qi failing to circulate to the four extremities. The Classics state: "The Spleen governs the four limbs." It also states: "The Spleen governs all yin meridians." Now, with cold and stiff hands and feet, it indicates the presence of only yin without yang, making it difficult to treat. If the abdominal tendons are cyanotic and tense, this is Wood overacting on Earth (Liver overacting on Spleen); immediately use Liujunzi Tang (Six Gentlemen Decoction) added with Paojiang (Prepared Zingiberis Rhizoma), Rougui (Cinnamomi Cortex), Chaihu (Bupleuri Radix), and Shengma (Cimicifugae Rhizoma) to restore true qi. If caused by wind pathogen, it should be treated referring to Jing Feng (Wind Tremors). This condition originates from the Liver and Spleen organs; one should tonify the Spleen and soothe the Liver, and refer to the sections on Ji Jing Feng (Acute Wind Tremors) and Man Jing Feng (Chronic Wind Tremors). "Baiwen" states: if confirmed as a wind syndrome, treat according to Zhong Feng (Wind Stroke), which will surely have life-saving effects; use Xiao Xuming Tang (Minor Rescue Life Decoction) with modifications.
Xiao Xuming Tang treats Zhong Feng (Wind Stroke) with unconsciousness, phlegm rattling in the throat, opisthotonos, aphonia, and cold limbs.
Herbs include: Mahuang (Ephedrae Herba, de-jointed), Renshen (Ginseng Radix et Rhizoma), Huangqin (Scutellariae Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Shaoyao (Paeoniae Radix), Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried), Xingren (Armeniacae Semen Amarum, skin and tip removed, stir-fried), Hanfangji (Stephaniae Tetrandrae Radix), Guangui (Cinnamomi Cortex, bark removed, each 0.5 liang), Fangfeng (Saposhnikoviae Radix, 7.5 qian), Fuzi (Aconiti Lateralis Radix Praeparata, baked, skin and navel removed, 7.5 qian).
Above herbs, except Fuzi and Xingren, are crushed into coarse powder, then the two are mixed in between. Take 1 qian per dose, decoct with Jiang (Zingiberis Rhizoma) and Zao (Jujubae Fructus), take before meals. If there is heat, remove Fuzi and reduce Guanggui by half.
The Classics say: "All sudden stiffness belong to wind," hence this formula is included; do not use unless it is a wind syndrome.
Whenever chills and fever alternate without regularity, if they occur at regular intervals, it is Nue (Malaria). Chao Yuanfang says: "Wind pathogen externally resides in the skin, while phlegm-fluid internally infiltrates the fu-organs, causing disharmony of blood and qi, and mutual overacting of yin and yang. Yang excess leads to heat, Yin excess leads to cold. When yin and yang qi are attacked by pathogen, and pathogen interferes with healthy qi, yin and yang contend, appearing and disappearing intermittently, resulting in alternating chills and fever." "Quansheng Zhimi" discusses: "If a person feels hot like 'xi xi' (shivering warmth) and cold like 'xi xi' (chilly), without fixed timing, joints feeling loose and sore, limbs painful, dizziness and blurred vision, this is due to deficiency of Ying (Nutritive) and Wei (Defensive) qi, allowing external wind pathogen to invade. Pathogen combats Yang leading to fever, combats Yin leading to cold; if untreated for long, it becomes Lao (Consumption). Use Jingjie San (Schizonepeta Powder)."
It also says: "If chills and fever resemble Malaria without fixed timing, abdominal fullness and distension, dizziness upon rising, constipation, occasional abdominal pain, chest and diaphragm stuffiness, this is due to retained food failing to transform, knotting in the intestines, obstructing qi mechanism, and chaotic yin and yang. Use Beijij Wan (Urgent Rescue Pill)." The Classics say: "Yang deficiency leads to external cold, Yin deficiency leads to internal heat; Yang excess leads to external heat, Yin excess leads to internal cold. Alternating chills and fever result from mutual victory of yin and yang. Thus, when cold qi merges with Yin, cold occurs; when Yang qi merges with Yang, heat occurs. Cunkou pulse being faint indicates insufficient Yang, Yin qi entering Yang causes aversion to cold; Chi pulse being weak indicates insufficient Yin, Yang qi entering Yin causes fever. Insufficient Yang leads to cold first then heat; insufficient Yin leads to heat first then cold. If yin and yang do not return to their respective positions, chills and fever contend. Furthermore, excess above leads to heat, excess below leads to cold; Yang excess leads to sudden heat, Yin excess leads to sudden cold. Mutual victory of yin and yang, imbalance of deficiency and excess, cause pathogens to alternate, resulting in alternating chills and fever, or sudden cold and heat. Shaoyang Gallbladder is the residence of the Liver, situated between Taiyang and Yangming, in the half-exterior half-interior region, where yin and yang qi easily overact. Thus, chills and fever mostly belong to Shaoyang/Gallbladder-Liver meridian syndromes, regulated by Xiaochaihu Tang (Minor Bupleurum Decoction) with modifications. If only chills and fever are present, daily activities remain normal, persisting for a long time, or after severe illness when Yuan Qi (Original Qi) has not recovered, all belong to Yin deficiency generating heat and Yang deficiency generating cold. Use Bazhen Tang (Eight Treasures Decoction) to tonify (for emaciation); for severe cases, use Shiquan Dabu Tang (Ten Significant Tonifying Decoction) (for sweating). Food retention causing disease also leads to chills and fever; use Baohe Wan (Preserve Harmony Pill) to resolve (for retained food). If accompanied by vomiting and diarrhea, use Liujunzi Tang (for vomiting/diarrhea); for cold limbs desiring hot drinks, use Ren Shen Lizhong Wan (Ginseng Center-Regulating Pill) (for diarrhea); for unrelenting thirst, use Qiwei Baizhu San (Seven-Ingredient Atractylodes Powder) (for thirst); if chills and fever persist after food retention is resolved, it is Liver pathogen overacting on Spleen, where the conquered insults the conqueror; use Yigong San (Extraordinary Merit Powder) added with Chaihu (Bupleuri Radix) and Shanzhi (Gardeniae Fructus). For details on Malaria chills and fever, see the Malaria section.
[Tang] Food retention causing chills and fever resembling Malaria, with thirst, diarrhea, and rapid breathing, requires lying flat with Di Gu Pi (Phellodendri Cortex). This condition should first be purged of retention, using only Pingwei San (Level the Stomach Powder), followed by frequent administration of Jinshi Wan (Appetite-Stimulating Pill). (Pingwei San see "Refusal to Nurse"; Jinshi Wan see "Retention".)
[Qian Yi] Cao Yide's son, aged three, had yellowish complexion, intermittent chills and fever, anorexia but excessive drinking of water and milk. Other physicians considered it Chao Re (Tidal Fever), used Niuhuang Wan (Calculus Bovis Pill) without success, then used Zhike Gansan (Thirst-Relieving Kudzu Powder), which caused vomiting after ingestion. Qian said one should purge with Baibing Zi (White Cake Pills), then tonify the Spleen, using Xiaoji Wan (Retention-Resolving Pill) to grind away retention. This was the correct approach, and he indeed recovered. Why? Not eating but only drinking indicates food hidden in the Spleen unable to digest, causing chills and fever. Vomiting from thirst-relieving medicine is due to the medicine irritating the Spleen; thus, purging resolves it.
[Zhang Congzheng] The son of Patrol Officer Gao, aged eight, suffered from heat disease. Physicians all diagnosed it as Cold Injury and treated with hot medicines. The child desired ice water, but was denied, leading to depletion of internal fluids, increased irritability, closure of both upper and lower orifices, dry mouth and nose, alternating chills and fever, intermittent cough, and generalized anhidrosis. They wished to apply moxibustion. Fortunately, Dairen (Zhang Congzheng's pseudonym) met them. Dairen reproached the mother: "Wrapped in heavy quilts, on a red-hot heated kang, the child cannot bear the heat; how can moxibustion be applied?" The mother apologized for ignorance. Dairen ordered the administration of Renshen Chaihu Yinzi (Ginseng-Bupleurum Drink) first, administered several doses continuously, resulting in the passage of foul-smelling, fish-intestine-like stool. Thirsty for water, allowed whatever he wanted; ice-cold water was consumed in cups. Subsequently, thirty to forty bowel movements occurred, and the high fever subsided. Then Tongge Wan (Unblocking Chest Pill) and Niuhuang Wan were given, resulting in more than ten bowel movements, and the child fully recovered. In total, about fifty bowel movements occurred, consuming approximately one hu of ice water. If moxibustion had been performed, what would have happened?

"Shuijian" How to treat infants under 100 days with sudden chills and fever? Giving cold medicines causes milk to become cold and lead to vomiting/regurgitation; giving hot medicines worsens the condition. Without divine methods, negligence kills the child. Having extensively studied secret formulas from stone chambers, usage yields miraculous results. Song says: "Taohua Yinmo (Flower Ash) slightly over 1 qian, Gancao (Licorice) in soup enhances power; Lanhua (Indigo Flower) only needs a few characters (dried in shade), poured into mouth eliminates instantly." If purgation is excessive, resolve with cold porridge. If there is a boil like a finger-sized cotton swab, mix one pill with honey, apply to swab tip, insert into boil in morning, draw out pus; repeat if not healed.
