[Zhang Jingyue's Discussion] When infants have stagnation of evil heat in the heart and diaphragm, accumulation of phlegm and saliva that cannot be dispersed, or if the nursing mother drinks alcohol and eats meat, allowing toxic heat to flow into the milk, causing the infant's retained food to not digest, the evil heat and toxin attack the heart spirit, causing palpitations, rolling eyes, high fever without ceasing, and limb convulsions. This disease is called Tian Gou. In severe cases, the fingernails turn blue, resembling possession by spirits. Now collecting effective famous prescriptions from classics, the treatments are as follows.

First, grind the left half of the centipede with musk in a mortar into fine powder, put into the "Left" packet and store. Using a separate mortar, grind the right half of the centipede with musk into extremely fine powder, put into the "Right" packet and store. Do not let them contaminate each other. Indication: When the patient's eyes are hooked up, showing only the whites, accompanied by opisthotonos and inability to make sound.

[Xue Ji's Discussion] Tian Gou, when attacking, features upward gaze of head and eyes, palpitations and high fever, eyes rolling backward, tears flowing but not stopping, hand and foot spasms, intermittent crying and laughing, resembling spirit possession. In severe cases, fingernails turn blue. This is due to the nursing mother consuming rich flavors, accumulating toxins in the stomach, causing heat-phlegm stagnation in the infant's heart and lungs, or external wind pathogen invasion. The method should resolve and benefit the pathogen, using Douteng Yin. If there is upper body asthma and coarse breathing, use Wu Xi Si Jun Zi Tang (Black Scorpion and Four Gentlemen Decoction).
Nei Gou (Internal Hook) presents with abdominal pain and frequent panting, black lips, scrotal swelling, stooping and opisthotonos, red eye corners. This is caused by wind received in the womb and external fright. If internal organs spasm painfully with wild shouting, or diarrhea with curled legs, once the internal syndrome appears, the external syndrome also appears. It is extremely difficult to regulate. Internal syndrome takes Jubao Dan, external syndrome takes Douteng Yin. If breast milk/food can be taken, it is treatable. For symptoms like abdominal pain, black lips, scrotal swelling, use Jubao Dan. For external fright, internal organ spasms, use Douteng Yin. If caused by nursing mother drinking strong wine and eating rich flavors, accumulating toxins in the stomach, use Jiawei Qingwei San (Modified Stomach-Clearing Powder). If caused by nursing mother's depression and anger, accumulating heat in the liver, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Jiawei Guipi Tang (Modified Spleen-Restoring Decoction), both added with Loulu (Euphorbia Kansui Root). Mother and child take them together. Whenever the mother eats rich/greasy food, before feeding the child, squeeze out the old milk first, then let the child suck.
