[General Discussion] Hong Kong Foot (a disease characterized by swelling and weakness of the lower legs and feet, commonly known as beriberi or gouty arthritis in historical contexts) originates from dampness pathogen invasion. It subsequently combines with wind-cold, summer-heat, or heat pathogens to manifest. Therefore, symptoms initially appear in the legs and feet. The feet first become weak and unable to support weight, followed by generalized muscle spasms and convulsions, pain in the limbs and joints, foot swelling, insensitivity in the lower abdomen, palpitations, chest fullness, dry and difficult defecation, accompanied by fever, headache, and body aches. The presentation closely resembles Taiyang Disease (diseases affecting the Taiyang channel). People often misdiagnose it. Zhang Zhongjing classified Hong Kong Foot as a syndrome similar to cold damage diseases, establishing separate chapters to distinguish it, precisely for this reason. In the early stage, pressing the affected area causes heat and pain. Redness and swelling start from the feet and gradually ascend to the thighs; the pathological trend will inevitably rise upward. If the pathogen ascends to the heart, it is considered incurable, requiring urgent treatment.
[For all initial stages of Hong Kong Foot] Tu Wuyao (Linderae Radix, 1-2 liang, peeled). Scrape the surface into powder using a bowl slice, store in a porcelain vessel. Soak in 1 jin of good wine overnight. The next morning, remove the herbs while fasting, add a small amount of Moschus (She Xiang), and take the wine in three divided doses, warm. If Moschus is unavailable, take one or two additional doses; efficacy is achieved immediately after loose stools or diarrhea occur.
[Damp rot, ulceration, swelling, and pain of the toes] Ulcers on both feet preventing walking. Cang Zhu (Atractylodis Rhizoma, soaked in rice wash water for one day, dried, sliced, stir-fried with salt water, 3 qian). Huang Bo (Phellodendri Cortex, stir-fried with wine, 3 qian). Chuan Niu Xi (Cyathulae Radix, stir-fried with wine, 2 qian). Decoct in water, take on an empty stomach. For severe cases, take twice daily. After five to seven treatments, efficacy is invariably achieved. Externally, crush Gou Qi Ye (Lycii Folium, Goji leaf) to extract juice, apply with a goose feather to the affected area; or decoct Luobo Cai (Radish greens) with roots for washing, both are effective.
[Fang Ji Yin] Treats foot pain and shin swelling due to dampness. Bai Zhu (Atractylodis Macrocephalae Rhizoma, stir-fried with soil). Cang Zhu (prepared as above). Xi Jiao (Rhinocerotis Cornu, now typically substituted with Buffalo Horn Shui Niu Jiao). Sheng Di Huang (Rehmanniae Radix, stir-fried with wine). Huang Bo (stir-fried with wine, each 1 qian). Mu Tong (Akebiae Caulis). Fang Ji (Stephaniae Tetrandrae Radix). Bing Lang (Arecae Semen). Long Dan Cao (Gentianae Radix, each 8 fen). Gan Cao (Glycyrrhizae Radix et Rhizoma, 5 fen). Decoct in water, take hot. If phlegm is present, add Zhu Li (Bambusa Textilis Sap, Bamboo Juice) and Jiang Zhi (Zingiberis Recensis Juice, Fresh Ginger Juice). If constipation occurs, add Tao Ren (Persicae Semen). If urination is difficult, add Niu Xi. Dry Feng Xian Hua Ye (Impatiens Balfourii Folium, Balsam leaf), decoct for hot washing, and mash leaves to apply externally to the affected area.
[Hong Kong Foot with Swelling] Decoct Ma Bian Cao (Verbenae Herba, Verbena officinalis herb) for washing two to three times until healed. For elderly patients or post-illness deficiency-type edema, use Bu Zhong Yi Qi Tang (Decoction for Replenishing the Middle and Augmenting the Qi - see Deficiency Damage section). Add Yi Yi Ren (Coicis Semen, Coix Seed, 3 qian) and Niu Xi (2 qian). Frequent administration yields results.
[Hong Kong Foot Pain Relief] Niu Pi Jiao (Corii Corii Asini, Donkey-hide gelatin). Crush finely, stir-fry with Bran (Fu Pi) until puffed, grind into fine powder. Take 1 qian mixed with wine, twice daily, to relieve pain. Chi Xiao Dou (Vigna umbellata Seed, Adzuki bean powder). Nan Xing (Arisaematis Rhizoma, stir-fried and powdered). Mix evenly, apply with Jiang Zhi (Fresh Ginger Juice) to relieve pain. Ai Ye (Artemisiae Argyi Folium, Mugwort leaf, 8 liang, kneaded). Hua Jiao (Zanthoxyli Pericarpium, Sichuan pepper, 1 jin). Cao Wu (Aconiti Kusnezoffii Radix, sliced, 2 liang). Mix evenly, pack in cloth to form a mat-like cushion. Place over a heating stove to fumigate the feet, and tread upon the mat. The rising aroma of pepper provides immediate pain relief.
[Hong Kong Foot Ascending to Heart] One Bing Lang (Areca Seed) ground to powder. Mix with Tong Bian (Child's Urine) and Jiang Zhi (Fresh Ginger Juice), half a cup, taken orally.
[Hong Kong Foot Critical Condition] Symptoms include aversion to cold, fever, massive swelling of both feet, irritability, body pain, approaching death. Use Shan Mu Jie (Schisandrae Sphenantherae Caulis, Schisandra vine nodes, 3 liang). Bing Lang (Areca Seed). Da Fu Pi (Arecae Pericarpium). Ju Ye (Citri Reticulatae Folium, Tangerine leaf, each 1 liang). All cut finely. Decoct in six bowls of running water until reduced to two bowls. Divide into three doses. For young, robust individuals, consume all within one day.
(Note: This condition is not caused by wind pathogen but by rebellious Qi. Bing Lang moves Qi to the highest level; Da Fu Pi moves Qi in the chest and diaphragm; Shan Mu Jie moves Qi in the limbs and joints; Ju Ye moves Liver Qi. When Qi moves, the disease resolves. The dosage must be substantial to overcome the pathogen. Users should adjust according to suitability.)
[Liu Huo (Flowing Fire)] Redness, swelling, and migratory pain in the legs and feet. Huo Luo Dan (Pill for Unblocking the Collaterals) taken with wine is highly effective. Externally, apply Hai Zhe Pi (Metapenaeus Ensis Periostracum, Jellyfish skin), selecting thin pieces, paste onto the affected area. Replace when dry. Alternatively, use Jian (Alkali/Soda, any amount, finely powdered). Mix with water and apply. When dry and frost-like residue forms, wipe off and reapply. Or, use Mei Tan (Coal dust, finely powdered), mix with Mi Cu (Rice Vinegar) and apply. These are excellent methods for pain relief. Decocting Shan Mu Xie (Schisandra wood shavings) for washing is also effective.
[Kidney Wind Accumulation causing Heavy Waist and Knees] Wei Ling Xian (Syneilesidis Rhizoma, Peucedani Radix, stir-fried and powdered). Form pills with Honey (Mi Wan) the size of Tung Zi (Tung tree seeds). Take 70-80 pills with wine at Wu Geng (approx. 3-5 AM). At dawn, slight passage of green pus-like foul matter indicates removal of wind-damp accumulation. If no bowel movement occurs, take 100 pills the next day. Consume congee after purgation to tonify. After recovery, take several doses of Ping Bu Yao (Balanced Tonifying Medicine) to prevent recurrence. (Note: Wei Ling Xian dispels various winds and unblocks the twelve channels. It causes mild laxity without severe diarrhea. Avoid tea and wheat products during medication.)
[Kidney Wind Attacking Feet, Knees, and Legs, Swollen like a Gourd] Gan Suan (Kansui Radix, 1 liang). Mu Die Zi (Momordicae Semen, Balsam Apple Seed, one male, one female). Remove shells, grind to powder, mix evenly. Use two pig kidneys, split open, insert 1 qian of powder. Wrap tightly in wet paper, roast in fire until cooked. Chew thoroughly at Wu Geng (approx. 3-5 AM), swallow with Mi Yin (Rice Drink). Purge accumulated fluid by dawn. Consume congee afterward to assist. If only the left leg is affected, use the left kidney; if the right, use the right. Use only 1 qian of powder. Even for chronic conditions lasting years, administration yields efficacy.
